RESUMO

Hydrocephalus is a common condition worldwide, and is frequently managed by diversion of cerebrospinal fluid (CSF), either externally with a drain or internally with a shunt. An external ventricular drain (EVD) can be an essential treatment modality, but is associated with a risk of infection, most commonly caused by Staphylococcal species, which can result in meningitis or ventriculitis and a delay in the definitive management of the hydrocephalus. Here, we report the case of a patient who required an EVD to manage post-operative hydrocephalus following a craniotomy and microvascular decompression for trigeminal neuralgia. He subsequently developed EVD-associated infection with a vancomycin-resistant Enterococcus faecium (VRE), which was treated successfully with a 2-week course of intravenous linezolid monotherapy. The authors believe this to be the only described case of successful treatment within this time frame of a CSF VRE infection associated with indwelling foreign material.

RESUMO

We report a case of a 60-year-old Caucasian man with a history of alcohol excess who presented to the emergency department with a 72-hour history of abdominal pain, profuse diarrhoea and vomiting. He was admitted to the intensive care unit (ICU) 12 hours later in extremis with severe sepsis and multiorgan failure. Collateral history from the patient on admission to ICU identified that he had been bitten by a dog 3 days prior to his symptom onset. Provisional microscopy and Gram staining from peripheral blood cultures taken on admission revealed the presence of long, thin Gram-negative bacilli in the anaerobic bottle only. This was later identified as Capnocytophaga canimorsus The patient survived the septic episode and was discharged to level 2 care 9 days later under the care of the renal physicians for ongoing renal dialysis.

RESUMO

Urinary tract infections are the most commonly acquired bacterial infections, but the significance of growths from catheter-related specimens, particularly heavy mixed growths, is uncertain and can pose a dilemma for the clinical laboratory responsible for processing and authorising reports, as well as a diagnostic dilemma for clinicians. Furthermore, inappropriate processing and reporting of samples may lead to inappropriate treatment of patients, which can result in adverse effects as well as increased laboratory and clinical costs. This short communication summarises recent evidence and guidelines on the matter.

RESUMO

Use of purified water for the final rinse stage of disinfected endoscopes is an important element of quality control. We describe the detection and management of Aspergillus fumigatus contamination of a new reverse osmosis unit supplying 10 automated endoscope reprocessor basins. Prompt detection and reaction to this contaminant were possible because of the introduction of a comprehensive program for microbiological monitoring of rinse waters, which included total viable counts, endotoxin, conductivity, and Pseudomonas spp.
