RESUMO

Type 2 Diabetes Mellitus (T2DM) is a growing global concern having reached the epidemic proportion in most countries. Achieving glycaemic control decreases the risk of macrovascular and microvascular complications in T2D. To achieve and maintain good glycaemic control, lifestyle changes and pharmacological treatment is necessary. Pharmacological management of T2DM have some limitations as it may lead to vit-B12 deprivation, hypoglycemia, increased cardiovascular risk etc. Treatment algorithm that include integration of Ayurvedic treatment & diet, Panchakarma therapies and Yoga will ensure good glycaemic control and reduce the progression of complications. Diagnosed cases of T2DM without intervention of conventional pharmacological drugs will be safe and beneficial. A female patient aged about 52 years visited to diabetic specialty OPD with the following complaints: increased micturation frequency, vaginal itching, fatigue and increased thirst. Patient reported weight gain in past 1 year, was not on any conventional antidiabetic medication. Her HbA1c status confirmed diagnosis of T2DM. Patient was treated with a course of nityavirechana (medicated purgation) and Sarwanga Udwartana (Dry medicated powder rubbing whole body) for the duration of 7 days along with ayurveda medicines for 1 year. Patient was prescribed Ayurvedic diet and Yoga practices throughout the intervention. This case report reveals the protocol based integrated Ayurveda and Yoga practices for diagnosed case of T2DM without causing any untoward effect along with reversal of the diabetes.

RESUMO

Post-irradiation angiosarcoma arising in the irradiated breast after breast-conserving surgery is uncommon though being reported with increasing frequency. Such cases are reported mainly in hormone-receptor positive patients who had received hormonal therapy along with radiation. We describe a rare case of post-irradiation angiosarcoma in a 71-year-old hormone-receptor negative female who did not receive hormonal therapy.

RESUMO

Tuberculosis of the colon commonly involves the ascending,transverse, or sigmoid colon while rectal involvement in tuberculosis is uncommon and poorly characterized. We report a six-year-old male from Nepal who presented with abdominal pain and difficulty passing stool for two years. On per rectal examination, palpation revealed a circumferential rectal mass. On further evaluation, CT scan showed mural thickening and luminal narrowing in the ano-rectum. Colonoscopy with biopsy showed caseating granuloma and positive acid fast bacilli culture consistent with tuberculosis. After starting anti-tubercular therapy, the patient's abdominal pain resolved and the patient was able to pass stool normally within two weeks. Colonoscopy three months after starting treatment showed complete resolution of the mass. Gastrointestinal tuberculosis should be considered in cases of children from endemic areas who present with a rectal mass.

BACKGROUND: Phyllodes tumors are spectrum of tumors ranging from benign to malignant. Malignant spectrum pose a management challenge for clinicians due to high risks of recurrence and metastasis. Malignant phyllodes tumor with brain, lung and adrenal metastases at the same time is rare. CASE PRESENTATION: A 37 years-old unmarried female presented with an ulcerated huge lump in left breast with severe bleeding. Trucut biopsy showed necrosis with spindle cell proliferation with atypia for which she underwent modified radical mastectomy with final diagnosis of malignant phyllodes tumor. Three months after surgery, patient presented with headache, which on further evaluation showed masses in lung, right adrenal and brain. CONCLUSION: We presented a rare case of malignant phyllodes tumor with clinical, imaging and histological findings with metastases to multiple sites early in course and poor outcome of the patient despite margin negative resection.

RESUMO

Hemangiomas of nipple of the breast are rare lesions and are limited to case reports in the literature. Such pathology in a lactating mother are clearly a hindrance preventing the child from breastfeeding with its benefits. We report a rare case of a bleeding nipple hemangioma in a 24 years old lactating mother with the clinical, imaging and histopathological findings.
