The effect of palatal wall modifications and height of a maxillary central incisor on the resistance form of metal-ceramic crowns are compared. Dies with adequate preparation height show no crown dislodgement in groups with a palatal wall, palatal wall/groove, and no palatal wall. This study suggests the use of a palatal groove improves the resistance to dislodgement in short maxillary anterior tooth preparations.

Primary and secondary reconstruction of mandibular discontinuity defects with vascularized flap is currently the standard of care in many institutions. The most commonly used donor site for such flaps is fibula. Fibula provides enough bone length, allows 2-team approach, and has low donor site morbidity and abundant periosteal blood supply. The placement of endosseous implants in the vascularized fibula flap also facilitates functional dental rehabilitation. This clinical report describes the prosthetic rehabilitation and the complications of 2 mandibular discontinuity defects treated with vascularized fibula flap and implant-supported fixed prosthesis.

This study sought to evaluate the effect of the linguoaxial wall, a lingual auxiliary groove, and the cervicoincisal dimension of the anterior tooth preparation on retention. A simulated maxillary central incisor was prepared following "ideal" guidelines. The linguoaxial wall of these preparations was modified and the samples were divided into three groups: preparations with a linguoaxial wall (Group 1), preparations with a linguoaxial wall and groove (Group 2), and preparations with no linguoaxial wall (Group 3). Ten metal dies and crowns were fabricated from each preparation group. A universal testing machine subjected cemented crowns to force along the long axis of the dies and load-to-dislodgement was measured. All preparations were shortened by 3.0 mm and crowns for the reduced-height groups were fabricated, cemented, and subjected to similar force. The preparations with the groove produced higher mean retention than the other groups. Among the reduced-height groups, the preparations with the groove also produced higher mean retention than the other groups. There were no differences in mean retention for the short and long preparation heights of each group.

The "All-on-Four" concept-tilting the distal implants in the edentulous arches improves the prosthetic support-increases the inter-implant distance and provides better implant anchorage in the bone by using longer implants. Computer milling of a solid block of titanium also provides frameworks with improved fit and fewer technical challenges than conventional cast or noncast approaches. This clinical report describes a method of restoring an edentulous mandible with the "All-on-Four" immediate function concept and a milled titanium framework. The patient in our clinical report has reported for follow-up visits for 1 year and is satisfied with the outcome of the treatment. No discernable clinical and radiographic changes were noted around the dental implants. To date, there have been no prosthetic complications. The patient is scheduled for quarterly follow-ups to determine the effectiveness of home care.

Replacing the missing maxillary anterior teeth with dental implants and maintaining the soft tissue structure and alveolar profile is an esthetic challenge for the restorative dentist. This clinical report describes the immediate provisional placement and prosthetic rehabilitation of 2 missing maxillary central incisors with scalloped dental implants.
