RESUMO
The article focuses on the possibility of our employing the drug preparation lypodox, a lyposomal form of doxorubicini hydrochloridum, of the Byolek firm, in treating of Hodgkin's disease and non-Hodgkin's lymphomas. A comparative analysis was performed of efficacy and ill effects of lyposomal and free forms of doxorubicini hydrochloridum. It has been shown that by clinical effectiveness the drug preparation lypodox, doxorubicini hydrochloridum in a lyposomal form, is superior to doxorubicini hydrochloridum in a free form in resistant and aggressive forms of Hodgkin's disease and non-Hodgkin's lymphomas. Side reactions developing in the wake of lypodox administration do not threaten the life and health of patients; these do not preclude us from employing the drug in a clinical setting.

A study was made of the natural resistance status in inhabitants of Kiev over ten years after the Chernobyl power plant accident. The results obtained showed that small doses of ionizing radiation exert a negative effect on the body inducing morphological changes in leucocytes, lowering the blood absolute content of large granule-containing lymphocytes, causing decline in the level of natural killer activity, changes in leucocyte as well as lymphocyte counts. In inhabitants of Kiev, post-Chernobyl changes in the studied parameters-tend to get more pronounced with time.

The time and degree pattern of the primary response manifestations did not in all instances correlate with subsequent severity of radiation pathologies, which fact may be explained by different levels of bodily exposure to alpha and beta-emitting radionuclides, products of combustion of different substances, psychological status of the people. There is a wave-like change in the parameters characterizing the cellular composition of blood.

Some aspects were studied of pathologies in those liquidators who sought to find out connection between health disorders and restorative work at the Chernobyl Power Plant through the Kiev Regional Expert Medical Counsel in 1990-1996. Those disorders in whose pathogenesis the radiation factor acts together with factors of nonradiation nature, namely of the nervous, cardiovascular systems, and digestive organs, rank first in the structure of pathologies during the first ten years of the postradiation period. The incidence rate of malignancies among those liquidators who had applied to the Council is on the increase. In the structure of oncopathologies prevalence, the leading place is taken up by tumours of digestive organs, respiratory system, and urogenital organs.

Results are submitted of use of propes during different phases of treatment in 36 patients with malignant lymphomas. The above drug is associated with no complications or side effects, inhibits the development of the tumor process. The employment of propes permits enhancing both natural resistance of the organism and its killer activity. It is an effective aid to cytostatic therapy as a way for prophylaxis of hypoplasia of haemopoiesis which can be successfully used in other malignant tumors.

Polychemotherapy as one of the chief modes of treatment of Hodgkin's disease needs to be updated time and time again. Among approaches toward optimization of chemotherapeutic effects of the drug preparations in question is their selection tailored to the individual with regard to the results of the preliminary investigation into sensitivity to cytostatics. At the department of systemic tumor diseases, treatments with regard to individual sensitivity to chemotherapeutic drug preparations were administered to 190 patients with lymphogranulomatosis. The above treatments appeared to be associated with significant improvement of the immediate results, in the patients developing drug resistance included.

Polychemotherapy is a mainstay in the treatment of lymphoproliferative diseases. At the department of systemic tumour diseases there have been examined as many as 63 patients with malignant lymphomas; individual sensibility was studied to antitumour drug preparations as recommended by V. P. Evtukh, A. S. Zverkova (1984). After courses of polychemotherapy devised with taking in account the individual sensibility, the immediate results and a parameter characterizing the relapse-free survival got improved as compared with treatments administered according to standard strategies, particularly so in patients with non-Hodgkin's lymphomas presenting with high degree malignancy in the stage of dissemination.

The conducted investigations showed an apparent immunomodulating action of the studied natural preparations on natural resistance of patients with malignant lymphomas. The modulation effect was the greatest in patients taking esberitox during remission and while receiving radiotherapy. Administration of Ney Tumorin to patients with lymphogranulomatosis during remission appeared to be associated with substantial increase in the content of large granular lymphocytes and level of natural killer activity (almost two--fold) immediately after the end of the treatment and this tendency was to be still seen one--two months later. Pirogenal had marked stimulatory effect on natural resistance of patients with granulomatosis in the postradiation period. The studied preparations were not associated with side effects. General condition improved after their administration, as per subjective experience.

Lennert's lymphoma is a malignant lymphoproliferative disease first described by K. Lennert in 1966. Morphological manifestation of the disease is the presence of Hodgkin-, Berezovsky-Sternberg cells which are arranged at the background of lymphoid series cells, neutrophils, epithelioid cellular elements in the histological preparation. The clinical presentations of the disease include rapid progression, the presence of general toxic symptoms. Optimal treatment option of these conditions includes, in the authors' judgement, 1-3 courses of polychemotherapy at the start of the treatment, remote gamma-therapy to the area of the lesion foci, with subsequent use of several courses (not fewer than three) of polychemotherapy.

We have summarized the results of cytogenetic studies of peripheral blood lymphocytes conducted in the institutions of Kiev and Kharkov in persons irradiated after the accident of the Chernobyl NPP. The average level of chromosomal aberrations and the appearance of cytogenetic markers characteristic of their irradiation effect was higher in the examinees in comparison with the control. The increase in the number of aberrations in certain Kiev residents as well as the development of lymphogranulomatosis in some liquidators of the accident were detected. The increase in the frequency of chromosomal aberrations in liquidators of the accident was accompanied by the reduction in the level of reparative DNA synthesis of blood lymphocytes. The necessity to expand cytogenetic examination in persons that participated in the liquidation of the accident on the ChNPP and Kiev inhabitants is discussed.

A study carried out among residents of the city of Kiev indicates that combined chronic radiation after the Chernobyl accident in small doses resulted in a reduction of the natural killer activity level, decrease of the number of leucocytes, lymphocytes and in their qualitative disorders. With time after the Chernobyl accident there occurred a deterioration of the above values.

A trial has been done for the first time to use historadiography for detection of spontaneously incorporated radionuclides in the body of persons who participated in liquidation of the Chernobyl atomic station disaster. The effect of incorporated radiation on the gastric mucosa, blood and bone marrow was studied. 88% of patients showed different degrees of accumulation of radionuclides in mucosal epithelium and submucosal layer of the stomach.

The histogenesis, classification, terminology of histiocytic lymphomas is analyzed. The possibilities are shown of their diagnosis evaluating lectin histochemistry data which is of major importance for the prognosis of the clinical course of the disease.

It was established that adequate treatment during latent period of acute radiation sickness reduces to a significant degree the level of genetic changes in the lymphocytes of the peripheral blood of the victims. After treatment the signs and symptoms of the disease were less manifested.
