RESUMO

While the older adult population continues to grow, psychologists specializing in geropsychology remain a small fraction of the workforce. Facing this reality, it is essential for training programs to better monitor whether their students receive adequate training to serve older adults. The current study describes a brief, logistically feasible, and cost-effective program evaluation conducted within one APA-accredited clinical psychology doctoral program. The evaluation consisted of an online survey of doctoral students (n = 99) that explored their field training experiences with older adults, coursework related to aging, and their overall interest in working with older adults. Students reported significantly less coursework and significantly less field work focused on serving older adults when compared to other age groups. However, students reported a high level of interest in learning about aging, with a total of 73.74% of participants (n = 73) reporting at least some current interest. A number of institutional actions resulted from this evaluation, which demonstrate how a brief evaluation can yield data that is practically useful. This case example provides a useful model for others to follow, and may support other institutions' efforts to evaluate, monitor, and make potential improvements to aging-related training in their own programs.

RESUMO

BACKGROUND: Eliciting life stories is an important component of person-centered care that may aid in understanding how an individual's unique background and narrative impacts their health. Such life stories, especially when gathered from older military Veterans, may include traumatic events, raising the urgency that clinicians be prepared to provide care that is trauma-informed and ensure Veteran's experiences are acknowledged and treated effectively. METHODS: We examined the prevalence and type of potentially traumatic and/or adverse life events spontaneously shared by 231 older Veterans participating in a life story intervention while receiving care within the U.S. Veterans Health Administration system. Veteran life stories were extracted from the electronic medical record and deductive qualitative content analysis was used to code potentially traumatic or adverse life experiences based on a codebook adapted from the Brief Trauma Questionnaire and Adverse Childhood Experience questionnaire. RESULTS: A majority (71.0%) of Veterans described at least one traumatic disclosure in their life story. Among narratives with a disclosure of any type, more than half (53.0%) included descriptions of combat trauma, 21.5% noted a history of life-threatening illness, and 9.5% reported having been in a serious accident. Fewer noted adverse childhood experiences (19.5%). CONCLUSIONS: Elicitation of life stories among older Veterans may advance person-centered care and life stories frequently include disclosure of potentially traumatic and/or adverse life events. These findings underscore the normative experience of traumatic events among older Veterans and highlight the importance of developing age-sensitive trauma-informed care competencies.

RESUMO

BACKGROUND AND OBJECTIVES: The ongoing workforce shortage in geropsychology requires continued efforts to understand the factors that influence whether or not students choose to enter this field. The current study builds on prior research by replicating the Working with Older Adults Scale (WOAS) model. Further, the scale was expanded to add professional attitudes, adultist concerns, and number of known professionals in the field of aging. RESEARCH DESIGN AND METHODS: Graduate students in clinical psychology (n = 117) completed the WOAS and new items as part of a larger survey of graduate students in an American Psychological Association-accredited doctoral program. Structural equation modeling was used to first replicate the existing WOAS model and then expanded to include new antecedents in a second model. RESULTS: Results were largely similar to past research validating the WOAS, with the exception that Perceived Behavioral Control was not a significant predictor of Intention in this population. In the expanded model, new items improved the overall model fit and exhibited significant indirect paths to predict intention to work with older adults. DISCUSSION AND IMPLICATIONS: These findings further validate use of the WOAS and expand scholarly understanding of the factors influencing the choice of a career in aging. Individuals positioned to encourage careers in aging may wish to address adultist concerns, explore attitudes about such careers, challenge unhelpful assumptions, and provide greater exposure and access to mentors in the field as part of recruitment efforts.

RESUMO

OBJECTIVES: This article describes the program evaluation of a group intervention combining reminiscence with literary creative writing instruction. METHOD: This 10-week, 1-hour group was completed seven times in an outpatient geriatric mental health clinic. Each session introduced a different reminiscence theme and creative writing practice. Feasibility and acceptability were assessed through group monitoring and qualitative feedback. Effectiveness was assessed with pre- and post-test depression screening (PHQ-9) and qualitative feedback. RESULTS: A total of 34 veterans participated in the groups, with a mean group size of 4.86 (SD = .69). Participants were 85% male (n = 29) and 15% female (n = 5) with a mean age of 70.89 (SD = 8.30). To increase accessibility, several adaptations were made. There were statistically significant reductions in PHQ-9 depression scores (p = .005). Veterans reported qualitative improvements including increased motivation and self-expression. CONCLUSIONS: This intervention was feasible and acceptable to the veterans being served. Randomized controlled research is needed to better understand efficacy. CLINICAL IMPLICATIONS: Clinicians may consider ways to augment or adapt reminiscence interventions to meet the needs of their patient population. Clinicians may find it useful to integrate creative writing instruction into reminiscence interventions.
