RESUMO

A critically ill man suffered a respiratory arrest due to pulmonary air embolism after the exchange of central venous catheters over a guidewire. A pulmonary perfusion lung scan performed 90 min later demonstrated extensive perfusion defects which were interpreted as "high probability" for PTE. Pulmonary angiography 4.5 h later was normal. A second perfusion lung scan performed 24 h after the respiratory arrest was normal. Pulmonary air embolism can produce segmental (and larger) perfusion defects which may be indistinguishable from those caused by PTE. The rapid (24 h) resolution of the perfusion defects may help differentiate the two disorders.

RESUMO

Amiodarone (Cordarone) is an antiarrhythmic cardiac drug that is currently being evaluated in the United States. Pulmonary infiltrates developed in six of 34 patients who were receiving amiodarone in daily doses of 200-800 mg over a period of 1.5 to 12 months. Two patients died, one returned to normal after amiodarone had been discontinued for six months, and three have residual radiographic abnormalities (two continue to receive amiodarone with steroids). Radiologists should be aware of the history of amiodarone therapy and this possible serious side effect so that they can detect the earliest changes in the lung.

RESUMO

A 14-month-old child presented with severe respiratory distress from an anterior mediastinal mass. Since its gross appearance suggested malignancy, radical surgical excision was performed. In retrospect, secondary hemorrhage, fibrosis, and chronic inflammation were responsible for the atypical appearance of a benign cystic hygroma. Etiology, clinical presentations, diagnosis and recommended treatment of this relatively rare mediastinal lesion are discussed.
