RESUMO

Combined therapy with preoperative course of remote neutron therapy was given to 29 patients with oral cavity cancer. All the patients had stage III of morphologically verified squamous cell carcinoma. Partial tumor regression after radiotherapy was observed in 39.5 +/- 8.9% patients. Complete regression was not achieved. There was no clear negative effect of preoperative course of remote therapy on the postoperative period in all patients. Local complications as wound edge disjunctions were registered in 13.4% of cases. On day 14-16 after surgery speech therapy rehabilitation was performed.

RESUMO

Reversible aggregation of the peripheral blood erythrocytes was studied in patients with stages III-IV lung cancer and patients with stages II-III tumors of the head and neck (cancer of the tongue, larynx, and buccal mucosa). The parameters examined were as follows: U0 and Ud--minimal and maximal stability of aggregates, tau---red cell aggregation half-period, A--the amplitude of photometric signal (in mm) characterizing the count of erythrocytes, Ia--aggregation index, and K--integral coefficient. The findings indicate an appreciable intensification of reversible aggregation of red cells in cancer: a significant increase of U0, Ud, A, Ia, and K and decrease of tau.

RESUMO

Scanning electron microscopy of the relief of the surface of peripheral red blood cells in patients with tumors of the head and neck (cancer of the tongue, larynx, and fundal buccal mucosa) of stages II-III and with stages III-IV lung cancer revealed similar changes, characterized by a manifest drop in the count of discocytes and increased share of transitional, prehemolytic, and degenerative forms of cells.

RESUMO

The efficacy of low-energy helium-neon and copper vapor lasers for prevention and treatment of postoperative and irradiation complications was assessed in 195 patients with locally advanced tumors of the head and neck. The control group included 118 patients. Intravenous laser irradiation of the blood was associated with a higher percentage of wound healing by first intention and better course of the postoperative period. Laser treatment of skin irradiation fields was shown to improve skin tolerance to the neutron beam. The study failed to establish tumor growth stimulation by the laser irradiation in terms of recurrence and metastasis development. The data obtained showed low-energy laser irradiation to offer promise for prevention and treatment of postoperative and irradiation complications.

RESUMO

The results of investigations evidence a high efficacy of intravascular laser irradiation of the blood in the treatment of maxillofacial pyonecrotic processes. Such treatment is conducive to improvement of the immunologic reactivity, microcirculation, and regeneration. The method is simple, that recommends it for outpatient practice.

RESUMO

The paper is concerned with the data on late radiation injuries of normal tissues and critical organs in a group of 35 patients after fast neutron therapy in a period of 12 mos.-3.5 yrs. The value of radiation exposure of critical organs and tissues was considered for patients with head and neck tumors in relation to methods of fast neutron therapy and photon radiation with 60Co after a radical program.

RESUMO

An analysis of primary reconstructive surgical treatment of 254 patients with carcinoma of the mouth mucosa and oral pharynx was made. The character of operative procedures for different localizations of the tumour is described. The primary reconstructive surgical treatment was shown to reduce the amount of postoperative complications to 11.4%, to keep intact the natural ways of respiration and nutrition at the early postoperative period. If without recidivation, about 70% of the patients continued working.

Defects resulting from surgical treatment for maxillo-facial tumors of various origin were studied. Procedures for reconstruction versus site are discussed and immediate results are presented. Reconstruction should be performed in early postoperative period to assure normal food passage.

The paper discusses the results of treatment of 128 patients with head and neck cancer in whom fast neutrons (Y-120; approximately 6.2 MeV) were used as a single procedure or in combination with surgery. Tumors of different histomorphologic structure displayed unequal sensitivity to fast neutrons. Mean disease-free survival in patients with complete regression of tumor was 24 months following neutron therapy alone, 18.5 months after combined neutron-photon treatment and 5.5 months only in controls. Relapse rates were 20 and 15% in patients with pre- and postoperative neutron treatment, respectively, as compared to 52% in controls.

RESUMO

Surgical intervention was performed in 16 patients with advanced tumors of the accessory sinuses and nose receiving neutron teletherapy. The 1st group (10 persons) was treated by combined therapy including preoperative irradiation with fast neutrons: 2 fractions a week at a dose of 1.2-1.4 Gy, total focal dose of 10 Gy corresponding to an equivalent dose of 40 Gy. Surgical intervention was performed in 10-15 days. In the 1st group during the post-operative period there were no complications, the period of wound healing was not increased as compared to that in the use of routine methods of combined therapy of tumors of the above site. In the 2nd group (6 persons) surgical intervention was performed 2-3 mos. after a radical course of combined neutron-photon therapy. The frequency of postoperative complications was 67%. The period of post-operative wound healing considerably increased.

RESUMO

The results of therapy of 55 head and neck tumor patients using fast neutrons of the Y-120 cyclotron of the Research Institute of Nuclear Physics, Tomsk Polytechnical Institute, were analyzed. For design of courses of neutron and combined neutron-photon therapy formulas of DMF calculation were used in cases of homogeneous and arrhythmic dose fractionation. The authors presented some data on methods of therapy with fast neutrons, dose fractionation schedules, a focal dose value, and time of treatment. Complete tumor regression was noted in 50-54% of cases using fast neutrons alone or in combination with photon radiation. A high sensitivity to neutron radiation of nonepidermoid carcinomas as compared to epidermoid cancer types was established with relation to tumor morphological structure.

The paper is concerned with the first results of treatment of 46 patients with tumors of the skin, oral mucosa and other sites using the Soviet small-size PMB-6E betatron with the energy of fast electrons at 6 MeV. The performance of the unit and different variants of dosimetric therapeutic plans of the fast electron beam are presented. Electron therapy with the energy of 6 MeV was mostly used in patients with head and neck tumors. Intra-oral irradiation with fast electrons was given to 4 patients with cancer of the moving part of the tongue and 5 patients with cancer of the fundic oral mucosa, the soft palate and the buccal mucosa in the form of combined or concomitant therapy. Summary focal doses in irradiation with fast electrons were 45-63 Gy for a combined or radical therapeutic course. A noticeable tumor regression was observed in patients with cancer of the skin, vermilion border and breast cancer recurrences. Though there were very few cases the results of concomitant and combined radiotherapy of cancer of the oral mucosa were promising. The first results of clinical application of the small-size PMB-6E betatron have shown its high efficacy for the treatment of superficial tumors.
