RESUMO
A 79-year old woman experienced a pubic bone fracture without severe dislocation as a result of a minor trauma. On the same day of the accident the patient developed symptoms of an acute abdomen. During laparatomy a traumatic small bowel perforation, caused by the fracture, was found. The patient died of peritonitis and complications independent of the original trauma three weeks after the accident.

Report on a case of acute vital complication of a conservatively treated lumbar vertebra fracture. Nine days after traumatic fracture of the first lumbar vertebra a 62-year old patient suddenly suffered from haemorrhagic shock caused by a minor rupture of the abdominal aorta. Before this dramatic event there were no symptoms pointing to this severe complication. Aetiology, diagnosis and therapy are discussed.

15 serious injuries of the cervical spine were treated by Halo-fixation devices. Practical procedure as well as advantages in clinical application are dealt with. This method seems to be a good alternative to surgical stabilisation.

This paper deals with problems and therapeutic principles of Smith-fractures. Fractures type II and III by Thomas are treated with T-plate osteosynthesis on principle. Methods are shown and the follow up of 15 surgically treated patients is reported. The good results in comparison with non operative treatment speak in favour of this method.

Sixty-eight samples were removed during autopsy from the right lateral thigh of seven female and ten male deceased persons between the ages of 50 and 82 years. These samples were examined with regard to their mechanical properties. For this purpose, two test series were used. The first series involved natural strips of skin and the second series comprised solvent-dried, gammasterilized and rehydrated strips of skin. The aim of the examinations was -to research the tensile strength of strips of human hide, -the effects of the conservation/sterilization process, -the possibility of a homological transplantation and -the dependency on age and sex. The tensile strength was found to be 29.70 kp mean average in the case of the male natural samples and 28.02 kp mean average for the female samples. The gamma-sterilized samples showed a marked loss in tensile strength of approximately 50%. The remaining tensile strength of 14.53 kp mean average is judged to be reasonably sufficient for use in homological transplants with gamma-sterilized strips of hide. There was found to be no dependency on age or sex.

16 patients with acromio-clavicular joint injuries were postexamined. On six of these a ligament suture and additional fixation by wiring had been applied. On the remaining 10, a replacement of the ligament with autologous corium following the modified Bunnell-technique had been performed. The achieved results are satisfactory in both groups. However, in our opion, the modified procedure following Bunnell is preferable to all other techniques.

Using 12 typical cases, which are tabulated, the diagnosis and therapy of combined bone and vascular injuries are demonstrated. Particular emphasis is put on a detailed presurgery diagnosis, if possible angiographic diagnosis. Early surgery, subject to a generous indication, with resection of the injured vascular segments and subsegment interposition of an autologous vein transplant is preferred. The correctness of this procedure is confirmed by the corresponding good postsurgery results.

On the basis of 3 personal observations the clinical picture, pathomorphology and therapy of the so-called "grease gun injury", are described. Clinically there is a difference between the acute stage, intermediary stage and chronic stage. In accordance with the course of disease are the pathomophological alterations, which are characterized by the development of an epithelioidcell-like granulation tissue. The injected oil is surrounded by histiocytic cell infiltrations, which, due to the chemico-toxic influence of this foreign substance are transformed into epithloidcells. Analogous to the pathogenesis of the tuberculoid granuloma this cellular metamorphosis is based on the action of long-chain fatty acids. It is important that the injured person is given immediate surgical treatment and operation. The aim of this being the entire removal of the oil or grease in order to avoid chronically granulating inflammation of the soft parts as well as the formation of scar tissue.
