RESUMO

A newly developed technique to reduce the local air contamination level in operating rooms is described. After physical and hygienic evaluations a mobile clean field-equipment has been used at 97 traumatic operations. At 66 operations about 300 aerobiological samples of the contaminated room air and simultaneously of clean field-air at the wound site were collected. It is shown that the bacterial level of clean field-air is comparable with those levels in a sterile operating enclosure. The advantages of the new technique and first experiences under practical operating conditions are discussed.

RESUMO

Direct current under certain conditions will be show a positive effect on the longitudinal growth of long bones during growing-age. Examinations were made in 8 to 10 week old rabbits in a right to left attempt. The electrodes were implanted in the midshaft of tibia. The stimulation was performed by direct current of 20 and 30 microamperes. The quantitative evaluation of 12 oxytetracyclinelabled pairs of bone showed a statistically significant increase of longitudinal growth on the electrically treated side as compared with control. Possible reasons leading to bone growth by electrical stimulation are discussed.

RESUMO

The examinations were made in grown up rabbits of different races in a right to left attempt. A defect like a slot was placed transcondyloid and sagittal by an osteotomy in the condyle of the femur. The fixation of the condyle was performed by a screw poled as kathode. The anode was planted in the extensor muscles of the femur. Direct current from a battery on the neck of the rabbit was running subcutan by stranded wires. The stimulation was performed by direct current of 10 and 17,5 microamperes for two, three and four weeks. The quantitative survey of 44 pairs of bone showed a significant acceleration of bonegrowth by direct current indicated by a higher weight of the new grown bone in the osteotomy slot. The bones, stimulated by 10 and 17,5 microamperes showed no significant differences among themselves. Possible reasons leading to bonegrowth are discussed.

RESUMO

The dynamic slide osteosynthesis of Pohl - a single acting method - is compared to other methods of operations. 103 patients with intertrochanteric fractures of the femur were treated by "Pohlsche Laschenschraube". The average age of patients was 76.7 years. The average stay in our hospital lasted 43 days. 16.5% of the patients died. 27 patients were examined two years after operation. The further study is based on clinical documents.

RESUMO

Report about a rare complication after total hip endoprosthesis with a simultaneous rupture of the cement mount in the acetabulum (due to a trauma) and a fracture of the femur. The x-rays and the intraoperative findings were interpreted erroneously; therefore the diagnosis was delayed. A second operation was needed to reimplant the acetabulum. During the same operation a wrong position of the prosthesis (outward rotation) had to be corrected.

RESUMO

Degenerative changes of the femoropatellar joint are being observed increasingly more often. These may be due to various disturbances of the gliding movement and gliding plane. The article describes the clinical and x-ray symptoms and the therapy; in most cases, additional surgery will be necessary besides antepositioning according to the technique described by Bandi.--Follow-up examinations of 32 patients showed that in 28 of 35 surgically treated knee joints, a positive influence had been exercised on the complaints, load capacity and walking distance.

RESUMO

36 arthrodesis of the wrist have been controlled. The operative technique could be divided into two groups. On the first group a stable osteosynthesis and cortico-cancellous bone graft have been used. The second group has been treated by other internal rigid fixation. 13 patients of the first and 14 of the second group have been followed up. It was possible to obtain an arthrodesis by all methods. The stable kind of arthrodesis makes the handposition secure, the postoperative immobilization in cast much shorter and avoids the danger of pseudarthrosis.

RESUMO

In the OHH, during the years 1969 to 1974, 28 patients were operatively treated because of delayed callusformation or pseud-arthrosis, following a fracture of the shaft of the humerus. It was possible to inquire about and to re-examine 20 patients. The operative treatment had, when possible been achieved by osteosynthesis through plates (25 cases). Each of the so treated cases lead to healing of the bone, additional nerve lesion did not in one case occur because of the operation. Objectively seen, there existed in 7 patients a slight, final restriction of movements, for the abduction and outside rotation in the shoulder joint. 6 patients complained of being sensitive to weather changes. The different treatment method of the fracture of the humerus and pseudarthrosis was discussed.

RESUMO

High tibial osteotomy has proved its value in the treatment of gonarthrosis with or without axis deformity. The thrust of weight-bearing and other stresses is lessened on the degenerated tibial condyle and transferred to the more normal condyle. The stable fixation by means of external fixation allows early movement of the knee joint.-R-ferences to operative technique, indication, complications and after-treatment.

RESUMO

For differentiated indication for operative treatment of avascular necrosis of the femoral head following points have to be considered: age, sex and profession of the patient, precedent treatments, state of contralateral and neighbouring joints. -- Methods of operation: Osteotomy for redistribution, filling with spongious bone and muscle-pedicle-bone graft, transplantation of homeoplastic osteochondral cups of the femoral head, Smith-Petersen-Cup arthroplasty- alloarthroplastic replacement of the femoral head, total replacement of the hip joint, resection of the femoral head and angulation-osteotomy of the diaphysis, arthrodesis of the hip joint. These methods are discussed with regard to personal conditions, surgical techniques, postoperative treatment as well as their advantages and disadvantages. There is no causative treatment of avascular necrosis of the femoral head. Therefore it is especially important to use the different methods appropriate to the special situation.

RESUMO

The results of 42 cases of compression arthrodesis of the knee joint during a 5.5 years period are reviewed. Main indications were degenerative, rheumatoid and posttraumatic arthrosis and tuberculosis of the knee joint. Other differential indications and the operative procedure following the Charnley-Müller method are described. The complications - pseudarthrosis, infection, false rotation - are discussed.
