RESUMO

We present a case report of patient with systemic vasculitis with affection of heart, skin, gastrointestinal tract and rare involvement of central nervous system. Diagnosis of systemic vasculitis was based on clinical manifestations, blood hypereosinophilia and brain magnetic resonance imaging. Immunosuppressive therapy led to regression of symptoms including initially present neurologic manifestation.

RESUMO

AIM: To present a rare case report about a woman with a right heart ventricle malignant melanoma metastasis who presented with chest pain and that was successfully detected by 67Ga-citrate single photon emission computer tomography (SPECT). MATERIAL AND METHOD: A 73-years-old woman with a past history of excision of a malignant melanoma from the right calf, 6 years later, was admitted to hospital with chest pain. The examination excluded myocardial ischemia. The chest CT and MR imaging detected a large tumour mass in the right heart ventricle. The CT and MR finding was evaluated as a suspect benign, mesenchymal intracardiac tumour. The 67Ga-citrate scintigraphy for restaging of malignant melanoma was performed. The double-head gamma camera VariCam (Elscint) with infrared body countouring and the large field of view was used. The gamma camera was fitted with medium-energy, parallel-hole collimators. Images were evaluated by processing system Xpert-Pro (Elscint). RESULTS: The whole body planar scintigraphy from anterior and posterior view was negative without pathological uptake of 67Ga-citrate, but the subsequent chest SPECT revealed oval focus (4 x 5 x 4 cm) of the pathological uptake in the right heart ventricle. The SPECT slices and whole body planar scans were evaluated as the suspect isolated metastasis of a malignant melanoma in the right ventricle (it was proven by biopsy) without others metastases. The woman underwent percutaneous transcatheter biopsy from the right heart ventricle and malignant melanoma metastasis was proven. CONCLUSION: The chest SPECT improved the sensitivity of 67Ga-citrate scintigraphy. The suspect malignant character of cardiac involvement was proven by 67Ga-citrate scintigraphy.

RESUMO

Magnetic resonance imaging is a modern imaging technique that is characterized by high resolution and variable tomographic slices. The development of magnetic resonance technology in last decade led to the expansion of this method in many fields of medicine. In cardiology, the imaging is focused on the heart, aorta, pulmonary, coronary and renal arteries. Dynamic imaging is used for the evaluation of the kinetics and the function of the ventricles. Static imaging serves for the assessment of the myocardial wall in patients with cardiomyopathies and coronary artery disease. The quality of static imaging can improve paramagnetic contrast agent that increasingly accumulates in areas of acute necrosis, fibrosis or infiltration of the myocardium. Magnetic resonance imaging can also diagnose intracardiac tumors and thrombi, valvular heart disease and pericardial disorders. Despite of wide spectrum of diagnostic applications, the clinical use of magnetic resonance imaging is reduced by limited availability and high cost of the examination.

RESUMO

Authors present the possibilities of different examination methods in cases of intraocular expansions and specific diagnostic methods to determine the actual etiology of the intraocular process in the posterior segment in particular. Of special concern to authors are cases, as is e.g. vitreous hemorrhage, in whose the standard examination methods are not useful. Pars plana vitrectomy with intraocular biopsy of the tissue for histological and cytological examination is very effective method to determine the proper diagnosis. The case report concerns with a 58 years old woman with vitreous hemorrhage and suspect tumor-like lesion, diagnosed by means of ultrasound examination. The pars plana vitrectomy revealed extensive preretinal organized hemorrhage with intravitreal hemorrhage. Samples for cytological examination were taken.

RESUMO

Percutaneous vertebroplasty is a therapeutic, interventional radiologic procedure originally developed in France by Galibert, Deramond et al. (1987). The technique consists of the percutaneous puncture of the affected vertebral body, followed by injection of bone cement into a vertebral body for the relief of pain, and the strengthening of the bone. The procedure was used initially to treat aggressive hemangiomas, but it then was extended to the treatment of osteolytic metastases, multiple myeloma and osteoporotic compression fractures refractory to medical therapy. In this article we review the current techniques, indications for this procedure, preoperative and postoperative evaluations.
