BACKGROUND: The quality-of-life (QoL) perception by Parkinson's disease (PD) patients and their caregivers (CG) has not been studied in depth. OBJECTIVE: To examine patient/proxy agreements on the PD QoL Questionnaire (PDQ-39), the Scale of Quality of Life of Care-Givers (SQLC) and the Multidimensional Caregiver Strain Index (MCSI). METHODS: Patients with PD and their CG completed the above-mentioned questionnaires about themselves and each other. The intraclass correlations between their scores (paired t test) were compared. RESULTS: Twelve patient-CG pairs were studied. Agreements for QoL items were strong and comparable for the total scores of the PDQ-39, SQLC and MCSI questionnaires (75.4% ± 14%; 78.1% ± 14.1% and 78.2% ± 14.3%, respectively). Agreements ranged from moderate to strong (0.57-0.88, P≤.05) for the patients' physical condition (PDQ-39 items 3, 5, 6, 8, 12-15, 23, 24, 35), mental concentration (item 31) and depression (item 17). Disagreements were apparent in 20%-25% of the pairs and were particularly significant for PDQ-39 items #33 and #25 (embarrassment of patients in public and distressing dreams or hallucinations), in which the CG gave higher scores than the patients. CONCLUSIONS: Agreements between patients with PD and CG were generally good for most, but not all, of the PDQ-39, SQLC and MCSI domains.

BACKGROUND: Calcium supplements to the western-style diet may reduce the risk for colorectal neoplasia. Using rectal epithelial proliferation (REP) measurements as a biomarker of response to intervention, the authors evaluated the effects of 1-year calcium supplementation in adenoma patients and its possible interactions with the patients' dietary and lifestyle habits. METHODS: Consenting adenoma patients, without a family history of colorectal neoplasia, were randomly selected to receive 3.75 g calcium carbonate (1.5 g Ca2+) daily or to receive no treatment. All had their long-term dietary and lifestyle habits assessed and their REP labeling index (LI) evaluated before and at end of follow-up. The change in LI was compared between groups, and statistical associations were examined between mean nutrient consumption and treatment effect and between lifestyle and treatment effect. RESULTS: Fifty-two adenoma patients (33 treated and 19 untreated) completed intervention and follow-up. There were no significant differences between study groups in age, weight, cigarette smoking, or medication use. The LI decreased in 58% of calcium-intervened patients and in only 26% of nonintervened patients (P = 0.04); the mean LI x 100 (+/- standard deviation) of the former fell from 5.04 +/- 1.93 to 4.54 +/- 1.58, and rose from 4.32 +/- 1.58 to 4.93 +/- 1.58 in the latter (P = 0.04). A lower fat, a higher carbohydrate, fiber, or fluid intake each interacted with the calcium supplementation to decrease the LI (P = 0.02, 0.001, 0.02, and 0.08, respectively). CONCLUSIONS: Long-term calcium supplements significantly suppressed REP in adenoma patients, and long-term dietary habits contributed to this effect. Patient diet should be assessed when researchers use REP as a biomarker in calcium chemoprevention studies. Study results indicated that relevant dietary counseling may be useful in addition to calcium supplements in persons at increased risk for colorectal neoplasia.

BACKGROUND: HemoccultSENSA (HOS), the sensitive guaiac fecal occult blood test (FOBT) for colorectal neoplasia, is faulted for its low specificity, which might be improved by substituting or adding FlexSure OBT (FS), the immunochemical test for human hemoglobin. (Both tests are manufactured by Beckman-Coulter Inc., Primary Care Diagnostics, Palo Alto, CA.) The authors compared both FOBTs in an endoscopic study to determine which FOBT to recommend for a population-screening program. METHODS: Both FOBTs, without dietary restrictions, were prepared by 1410 screenees or nonbleeding symptomatic patients (3%). All underwent colonoscopy (51.8%) or flexible sigmoidoscopy (if asymptomatic and both FOBTs were negative). RESULTS: HOS sensitivity for significant neoplasia, cancers, or adenomas >/= 1 cm (20 cases) was similar to that of FS (50% vs. 35%, not significant). However, HOS specificity was lower (95% vs. 99%, confidence interval (CIs) 94-96 vs. 98-99, P < 0.05). In those 11 cases in which both HOS and FS were positive, specificity for significant neoplasia was 100% but sensitivity decreased to 25% (less than HOS alone, P < 0.05). HOS was more sensitive than FS for any neoplasia (55 cases), including adenomas < 1 cm (35% vs. 18%, CIs 22-47 vs. 8-28, P < 0.05), but less specific (96% vs. 99%, CIs 95-97 vs. 98-100, P < 0.05). CONCLUSIONS: Guaiac HOS, which does not require dietary restrictions, is significantly more sensitive for any colorectal neoplasm than the immunochemical FS; it identifies more adenomas with a specificity that is low but acceptable for population screening.

When using the peroxidase-sensitive guaiac fecal occult blood test, Hemoccult Sensa (HOS), dietary restrictions are recommended so as to reduce its false positivity from ingested peroxidases. As dietary restrictions could reduce compliance for annual retesting, we reexamined their need. HOS was given to 944 persons in an endoscopic screening/follow-up program. Only the first 403 received instructions regarding a low peroxidase-containing diet for three days before and during HOS collection. Development was delayed 3-14 days after HOS preparation, and there were no significant differences in duration before HOS development in examinees with or without colorectal neoplasia and having or not having a positive test. Of the 901 persons without endoscopic evidence of neoplasia, 377 had received and 524 had not received instructions on dietary restrictions. Their HOS positivity was 7.2% and 5.5%, respectively (P = 0.26). When using HOS, delaying development by 3-14 days allows breakdown of ingested peroxidases, so dietary restrictions are not needed in our population, which has a low red meat intake.

A new immunochemical test for stool Hb, FlexSure OBT, was compared with the immunochemical HemeSelect and guaiac Hemoccult II and Hemoccult SENSA tests. Blinded development of test cards smeared with stools having added human blood showed better analytical sensitivity of FlexSure OBT (0.2 ml blood/100 g feces), than Hemoccult SENSA (> or =0.5 ml) or Hemoccult II (> or =1.0 ml). All four stool tests were prepared by 403 subjects having endoscopic examinations. The guaiac tests and FlexSure OBT were easy to prepare and develop. The positivity rate of Hemoccult SENSA was 8.7%, Hemoccult II 6%, FlexSure OBT 4.2%, and HemeSelect 3.4%. In this mainly asymptomatic (97%) population, 98% were free of clinically significant neoplasia (five had cancers, three had adenomas > or =1.0 cm). Sensitivity for cancers or adenomas > or =1.0 cm was similar for all tests (62.5-86%, NS) and Hemoccult SENSA had the lowest specificity (92% vs 95-98%, P < 0.05); but both Hemoccult II and Hemoccult SENSA had significantly lower predictive positive values (21% and 14%) than either FlexSure OBT (29%) or HemeSelect (50%) (P < 0.05). If both Hemoccult SENSA and FlexSure OBT were positive in the same subjects (1.7%), sensitivity for cancer or adenomas > or =1.0 cm (50%) was not significantly better than guaiac tests, but specificity (99.2%) and predictive positive (57%) values were improved (P < 0.05). In this population, guaiac tests were as sensitive as immunochemical tests for clinically significant colorectal neoplasia, but with significantly lower predictive positive values. A combination of a sensitive guaiac test (Hemoccult SENSA) and a specific confirmatory test for human Hb (FlexSure OBT) provided high specificity, comparable to HemeSelect.

Guaiac tests for faecal occult blood are used for colorectal neoplasia screening. Specificity may be improved by using an immunochemical test for human blood. We evaluated and compared, within an endoscopic study, an immunochemical test for human stool albumin, BM-Test Colon Albumin, with the guaiac test, Hemoccult SENSA. Both tests were given to 527 screenees who had had a low-peroxidase diet before and during the tests. All had a colonoscopic (59%) or flexible sigmoidoscopic (41%) examination. Both tests were easy to perform and develop. They were of similar sensitivity (35-30%) for adenomas > or = 1.0 cm in diameter or cancers, but Colon Albumin had a higher specificity (90%) than Hemoccult (85%; P < 0.05). The latter, however, had a higher sensitivity for neoplasia of all sizes (25% vs 20%; NS) but lower specificity (85% vs 90%; P < 0.05). Colon Albumin was positive in 11.2%, while Hemoccult SENSA was positive in 16.7% and appears very sensitive to dietary peroxidases. Positivity was reduced to 7% by changing the methodology protocol. Overall, the guaiac test is more sensitive but less specific than the immunochemical test for colorectal neoplasia. However, the technical and diagnostic limitations of these tests must be appreciated, and because of their high positivity rate neither is suitable for mass screening. A more specific test for neoplasia is needed.
