OBJECTIVE: To identify predictive factors for postoperative continence in female Golden Retrievers following cystoscopic-guided laser ablation of intramural ectopic ureters (CLA-EU). ANIMALS: 41 client-owned female entire Golden Retrievers with uni- or bilateral intramural ectopic ureter(s) were retrospectively enrolled. METHODS: Patients were diagnosed with ectopic ureters with a combination of ultrasonography and cystoscopy. CLA-EU was performed for all dogs so that each ureteral opening was considered to be in an appropriate position by a single operator. All dogs had short-term follow-up 4 weeks and long-term follow up > 10 weeks after the procedure via telephone, which included urinary continence scoring. Clinical factors and ultrasonographic and cystoscopic findings from initial presentation were evaluated to identify predictive factors for postoperative continence. RESULTS: Short-term urinary continence was achieved in 46.3% of dogs with no additional medical therapies. Presence of historical urinary tract infections prior to CLA-EU (OR, 0.130; 95% CI, 0.020 to 0.621; P = .018) was negatively correlated and ureteral dilatation (OR, 34.260; 95% CI, 1.813 to 2,143; P = .043) was positively correlated with likelihood of urinary continence. Long-term urinary continence was achieved in 63.4% of dogs, and presence of historical urinary tract infections was negatively prognostic (OR, 0.173; 95% CI, 0.023 to 0.856; P = .048). CLINICAL RELEVANCE: Female Golden Retrievers undergoing CLA-EU have similar outcomes to those reported for other mixed-breed cohorts with > 30% of dogs failing to regain urinary continence. Historical urinary tract infections were significantly associated with both short- and long-term urinary continence in our population.

BACKGROUND AIMS: In a previous pilot study of HLA-matched sibling donor hematopoietic cell transplantation (HCT), the authors determined the feasibility of day 4 versus day 5 granulocyte colony-stimulating factor (G-CSF)-mobilized peripheral blood stem cell (PBSC) collection compared with a historical cohort. Given identified differences in the PBSC product (day 4 cohort with significantly lower infused total nucleated, mononuclear and CD3 cells compared with other collection cohorts), the authors performed a follow-up study to determine long-term post-HCT outcomes, including detailed characterization of chronic graft-versus-host disease (GVHD). METHODS: This was a prospective observational study, and the authors collected data on chronic GVHD, staging, sites of involvement and treatments. Performance status, incidence of relapse, overall survival and duration of immunosuppressive therapy (IST) were also evaluated. Data were examined retrospectively. To account for differences in length of follow-up among cohorts, the authors also determined performance status and chronic GVHD staging, sites and treatment at 2 years post-HCT. RESULTS: At 2 years post-HCT, the overall survival rate was 71.7% in the day 4 cohort compared with 61.5%, 52% and 56% in the day 5, 2-day and historical cohorts, respectively (P = 0.283). The cumulative incidence of chronic GVHD was 65.2% in the day 4 cohort versus 46.4% in the day 5 cohort, 51.1% in the 2-day cohort and 65% in the historical cohort (P = 0.26). There was no significant difference in the maximum overall stage of chronic GVHD (P = 0.513), median number of sites involved (P = 0.401) or cumulative incidence of discontinuation of IST (P = 0.32). Death from chronic GVHD was less common in the day 4 and day 5 cohorts compared with the 2-day and historical cohorts, though this did not reach statistical significance. CONCLUSIONS: The authors' preliminary results demonstrated that collection of allogeneic matched sibling donor PBSCs on day 4 of G-CSF was feasible, reduced donor exposure to growth factor and was associated with an initial cost savings. Importantly, the authors now demonstrate that transplantation of day 4 mobilized PBSCs is not associated with any adverse outcomes post-HCT, including late effects such as chronic GVHD. Further investigation of donor G-CSF collection algorithms is merited in other HCT settings, including unrelated and mismatched related donors.

Ultrasonography is a widely available diagnostic modality for the identification of dogs with suspected ureteral ectopia; however published studies detailing its sensitivity and specificity are currently lacking. The aim of this retrospective, descriptive, diagnostic accuracy study was to evaluate the sensitivity and specificity of ultrasonography for the diagnosis of ureteral ectopia in incontinent dogs presenting to a referral institution, using cystoscopy as the gold standard. Medical records of urinary incontinent dogs presenting to a single institution (n = 38) were retrospectively reviewed for the presence of ureteric insertion abnormalities and concurrent urinary tract abnormalities. Ultrasonographic findings were compared with those from cystoscopic examination to determine diagnostic accuracy. The relationship between the presence of concurrent urinary tract abnormalities and ureteral ectopia was assessed using an independent samples t-test and Mann-Whitney test. Statistical significance was set at P ≤ 0.05. Ultrasonography had a sensitivity of 93.5%, specificity of 100%, and diagnostic accuracy of 95% when identifying dogs with ureteral ectopia. When classifying individual ureters as ectopic or non-ectopic, sensitivity was 87.8% and specificity was 86.7%. Dogs with ureteral ectopia had significantly more concurrent urinary tract abnormalities on ultrasound than unaffected dogs (P = 0.004). Ectopic ureters were associated with significantly more concurrent ipsilateral upper urinary tract ultrasonographic abnormalities than unaffected ureters (P < 0.001). Ultrasonography performed by an experienced ultrasonographer is a sensitive and specific screening tool for canine ureteral ectopia, which eliminates the need for heavy sedation, general anesthesia, and advanced imaging, although it should not be relied upon as the sole diagnostic modality for the assessment of individual ureters.

BACKGROUND & OBJECTIVES: Emotion regulation is an integral component of multiple physical and psychological functions, and problems with emotion regulation are thought to be involved with the development of psychological disorders. Most laboratory research on emotion regulation has been limited to investigating short-term impact during a single session. DESIGN & METHODS: In the current study, we investigated the potential delayed consequences of the short-term use of two emotion regulation strategies: mindfulness and suppression. To do so, a two-session design was implemented, with 56 undergraduate participants viewing images under various regulation instructions at Time 1, returning one week later for a recognition memory test for these images at Time 2. Images were rated for valence and arousal at both time points. RESULTS: We hypothesized that images encoded mindfully would elicit lower levels of anxiety upon re-exposure, while suppression would increase levels of anxiety. Results did not support original hypotheses: although participants reported being calmer and happier at re-exposure, this effect did not vary by strategy. High-anxiety participants reported higher arousal levels regardless of Instruction condition or Time Point. CONCLUSIONS: These findings suggest that short-term use of emotion regulation does not impact feelings of calmness long-term, when exposed to anxiety-provoking stimuli.

CASE DESCRIPTION: A 7.75-year-old sexually intact male Welsh Terrier was examined because of cervical soft tissue wounds and an inability to maintain hemoglobin oxygen saturation without oxygen supplementation following a dog attack. CLINICAL FINDINGS: A 2-cm-long penetrating wound that extended into a large open pocket was identified on the left ventral aspect of the dog's neck. The dog was anesthetized and underwent advanced imaging, the findings of which suggested that the trachea was intact. However, when the cuff of the endotracheal tube was deflated during the dog's recovery from anesthesia, sudden oxygen desaturation occurred. Given no radiographic signs of deteriorating lung injury, a tracheal tear was suspected. For rapid confirmation of a tracheal tear, without the need for additional advanced imaging, the oxygen concentration at the skin wound was investigated by use of an oxygen analyzer. When the dog was breathing 100% oxygen, the analyzer identified a higher oxygen concentration at the edge of the penetrating wound, compared with the concentration of oxygen in room air; the leakage of oxygen-rich gases from the airway through the wound confirmed the presence of a tracheal tear, immediately indicating the need for surgical exploration and repair. TREATMENT AND OUTCOME: Surgical repair of the tracheal tear with a left sternothyroideus muscle flap was successfully performed. CLINICAL RELEVANCE: For this dog, an oxygen analyzer was used to confirm the presence of a tracheal tear, suggesting that application of an oxygen analyzer may be useful in the emergency management of neck trauma cases.

Engagement in potentially beneficial activities is assumed to be based partially on perceived costs and benefits. Mental fatigue may be one factor that affects perceived costs. We examined age differences in both chronic and situational mental fatigue, and their relation to task perceptions and engagement levels. Younger (M age = 32.6) and older (M age = 73.1) adults completed questionnaires measuring subjective mental fatigue, physical and mental health, and motivational states, along with several assessments of cognitive ability. In addition, assessments of effort expenditure (systolic blood pressure) and task difficulty were collected during performance of a cognitively demanding memory-scan task. Two components of chronic mental fatigue (CMF) relating to affect and motivation were identified. Although the structure of CMF did not vary with age, CMF was differentially predictive of engagement and appraisal levels across groups. As CMF-Affect levels increased, older adults' task-difficulty appraisals increased more than those of younger adults. In addition, CMF-Affect was positively associated with levels of engagement in the young, but negatively associated in the old as memory load increased. Older adults also exhibited higher levels of situational fatigue than did the young, as evidenced by increased levels of effort expenditure as both time on task and objective demands increased. However, little relationship existed between chronic and situational mental fatigue in either age group. These findings suggest that the affective aspect of CMF is particularly consequential in older adults, with both task appraisals and engagement levels being negatively affected when fatigue is high.

Clonal hematopoiesis of indeterminate potential (CHIP) is an age-associated condition defined by the presence of a somatic mutation in a leukemia-associated gene in individuals who otherwise have no evidence of a hematologic malignancy. In the allogeneic hematopoietic cell transplantation (HCT) setting, clonal hematopoiesis (CH) mutations present in donor stem cells can be transferred to recipients at the time of HCT. Given that the consequences of donor-derived CH in HCT recipients are not entirely clear, we sought to investigate clinical outcomes in patients with engrafted donor-derived CH using a matched cohort analysis of both related and unrelated donors. Of 209 patients with next-generation sequencing performed before and after HCT, donor-derived CH mutations were detected in 15 (5.2%). DNMT3A was the most commonly mutated gene (9 of 15; 60%); mutations in SF3B1, CSF3R, STAT3, CBLB, TET2, and ASXL1 were also identified. Donor-derived CH was not associated with delayed neutrophil or platelet engraftment, and there was no impact on conversion to full donor chimerism. No patients with donor-derived CH experienced relapse, in contrast to 15.6% (7 of 45) in the matched control cohort without CH (P = .176). Donor-derived CH was not associated with worse overall survival; however, patients with donor-derived CH were more likely to develop chronic graft-versus-host disease (GVHD) necessitating systemic immunosuppressive therapy (IST) (P = .045) and less likely to discontinue IST (P = .03) compared with controls without donor-derived CH. We conclude that donor-derived CH does not have an adverse impact on relapse, survival, or engraftment outcomes but may potentiate a graft-versus-leukemia effect, as reflected by increased chronic GVHD necessitating IST.

A 5 yr old male British blue shorthair cat was presented collapsed with hypotension, hypothermia, and cranial abdominal pain. Abdominal ultrasound and abdominocentesis revealed hemoperitoneum and a suspected hepatic mass. Cytology of fine-needle aspirates of the mass was inconclusive, so exploratory celiotomy was performed for diagnostic and therapeutic purposes. An enlarged, hemorrhagic papillary process of the caudate lobe was identified, with twisting of the vasculature at the hilus, so a complete lobectomy was performed using a thoracoabdominal stapler. Histopathology was consistent with liver lobe torsion with no evidence of hepatocellular neoplasia identified. This report describes the first case of hemoperitoneum secondary to liver lobe torsion without evidence of hepatocellular neoplasia in a cat and demonstrates a successful outcome following surgical management with liver lobectomy.

BACKGROUND: Guidelines recommend treatment with 4-5 days of granulocyte colony-stimulating factor (G-CSF) for optimal donor peripheral blood progenitor cell (PBPC) mobilization followed by day 5 collection. Given that some autologous transplant recipients achieve adequate collection by day 4 and the possibility that some allogeneic donors may maximally mobilize PBPC before day 5, a feasibility study was performed evaluating day 4 allogeneic PBPC collection. METHODS: HLA-matched sibling donors underwent collection on day 4 of G-CSF for peripheral blood (PB) CD34+ counts ≥0.04 × 106/mL, otherwise they underwent collection on day 5. Those with inadequate collected CD34+ cells/kg recipient weight underwent repeat collection over 2 days. Transplant and PBPC characteristics and cost analysis were compared with a historical cohort collected on day 5 per our prior institutional algorithm. RESULTS: Of the 101 patient/donor pairs, 50 (49.5%) had adequate PBPC collection on day 4, with a median PB CD34+ cell count of 0.06 × 106/mL. Day 4 donors were more likely to develop bone pain and require analgesics. Median collected CD34+ count was significantly greater, whereas total nucleated, mononuclear and CD3+ cell counts were significantly lower, at time of transplant infusion for day 4 versus other collection cohorts. There were no significant differences in engraftment or graft-versus-host disease. Cost analysis revealed 6.7% direct cost savings for day 4 versus historical day 5 collection. DISCUSSION: Day 4 PB CD34+ threshold of ≥0.04 × 106/mL identified donors with high likelihood of adequate PBPC collection. Day 4 may be the optimal day of collection for healthy donors, without adverse effect on recipient transplant outcomes and with expected cost savings.

OBJECTIVE: To describe surgical techniques, caval occlusion times, and short-term outcomes in dogs undergoing adrenalectomy with caval venotomy. STUDY DESIGN: Retrospective case series. ANIMALS: Dogs undergoing adrenalectomy with caval venotomy between October 1, 2010 and May 31, 2018. METHODS: Medical records of dogs undergoing adrenalectomy with caval venotomy were reviewed for signalment, perioperative management, surgical details, perioperative complications, mortality, and histopathology. Computed tomography images were reviewed to describe tumor morphology and signs of thrombus extension. RESULTS: Nineteen dogs had adrenal tumor thrombi extending into the prehepatic (14 dogs, 74%), hepatic (3 dogs, 16%), and posthepatic (2 dogs, 11%) caudal vena cava. Tumors occurred in left (11) and right (8) adrenal glands. Median caval occlusion was 6.5 minutes (range, 2-25). Two to six vascular tourniquets were used. Venotomy closure was performed under full caval occlusion in 11 dogs and by using a partial occlusion clamp in 8 dogs. Left ureteronephrectomy was performed in 5 dogs. Perioperative mortality rate was 21% (4 dogs). CONCLUSION: Extension of caval tumor thrombus beyond the hepatic hilus did not preclude a good outcome. Longer caval occlusion than has been previously reported was tolerated in some cases. Number of vascular tourniquets used reflected the location of phrenicoabdominal vein insertion on the cava and length of the caval tumor thrombus. Venotomy closure under full occlusion was often required for right adrenal tumors. When required, ureteronephrectomy was left sided. CLINICAL SIGNIFICANCE: Dogs with adrenal tumors extending beyond the hepatic hilus and those requiring a long caval occlusion time can survive adrenalectomy.

OBJECTIVES: The aim of this study was to describe and evaluate the configurations and management of feline skull fractures and concurrent injuries following head trauma. METHODS: Medical records and CT images were reviewed for cats with skull fractures confirmed by CT that were managed conservatively or with surgery. Details of signalment, presentation, skull fracture configuration, management, re-examination, and complications or mortality were recorded and analysed. RESULTS: Seventy-five cats (53 males, 22 females) with a mean age of 4.8 ± 3 years met the inclusion criteria. Eighty-nine percent of cats had fractures in multiple bones of the skull, with the mandible, upper jaw (maxilla, incisive and nasal bones) and craniofacial regions most commonly affected. Temporomandibular joint injury occurred in 56% of cats. Road traffic accidents (RTAs) were the most common cause of skull fractures, occurring in 89% of cats, and caused fractures of multiple regions of the skull. RTAs were also associated with high levels of concurrent injuries, particularly ophthalmic, neurological and thoracic injuries. A more limited distribution of injuries was seen in non-RTA cats. Equal numbers of cats were managed conservatively or surgically (47%). Mortality rate was 8% and complications were reported in 22% of cats. Increasing age at presentation and presence of internal upper jaw fractures were risk factors for development of complications. No risk factors were identified for mortality. CONCLUSIONS AND RELEVANCE: RTAs were the most common cause of feline skull fractures and resulted in fractures in multiple regions of the skull and concurrent injuries occurred frequently. Problems with dental occlusion were uncommon post-treatment. An increased risk of implant loosening and malocclusion was seen with palatine and pterygoid bone fractures and hard palate injuries. This study provides useful additional information regarding feline skull fractures, concurrent injuries and management techniques following head trauma.

Tibial plateau levelling osteotomy (TPLO) is commonly performed for surgical management of cranial cruciate ligament (CCL) disease. It has been suggested that small dogs may have steeper tibial plateau angles (TPAs) than large dogs, which has been associated with increased complication rates after TPLO. A retrospective study was performed to assess the rate and nature of long-term complications following TPLO in small dogs with TPAs>30°. Medical records were reviewed for dogs with TPAs>30° treated for CCL rupture by TPLO with a 2.0 mm plate over a five-year period. Radiographs were assessed to determine TPA, postoperative tibial tuberosity width and to identify any complication. Up-to-date medical records were obtained from the referring veterinary surgeon and any complications in the year after surgery were recorded. The effects of different variables on complication rate were assessed using logistic regression analysis. Minor complications were reported in 22.7 per cent of cases. This is similar to or lower than previously reported complication rates for osteotomy techniques in small dogs and dogs with steep TPAs. A smaller postoperative TPA was the only variable significantly associated with an increased complication rate. No major complications were identified.

CASE DESCRIPTION A 6.8-year-old neutered male Labrador Retriever-Poodle mix was evaluated because of continued left pelvic limb lameness unresponsive to conservative management 1 year after undergoing tibial plateau leveling osteotomy (TPLO). CLINICAL FINDINGS A jerking lateral movement of the left stifle joint was detected during walking. Orthopedic examination revealed a pivot-shift phenomenon (PSP). Palpation elicited no signs of discomfort over the TPLO plate or caudomedial aspect of the stifle joint. Radiography revealed complete bone fusion at the osteotomy site and only mild joint effusion. TREATMENT AND OUTCOME Arthrotomy was performed at the medial aspect of the stifle joint, revealing no meniscal tears. The previously applied plate was removed, and an extracapsular, synthetic, ligament-like biomaterial was placed to counteract internal tibial rotation, thereby eliminating the PSP. Six weeks later, lameness had improved considerably with no evidence of PSP; 8 weeks after surgery, the dog was assessed for sudden-onset lameness in the right pelvic limb. Cranial cruciate ligament rupture was suspected, and TPLO was performed. The PSP was identified intraoperatively, so an extracapsular implant was placed. Six weeks later, the dog had only mild lameness and no evidence of PSP in either pelvic limb. In a follow-up telephone conversation 1 year later, the owner reported no obvious lameness or gait abnormalities. CLINICAL RELEVANCE Use of an extracapsular implant effectively eliminated the PSP following TPLO in the dog of this report and can be considered as an intra- or postoperative option for dogs with PSP that responds poorly to conservative management.
