RESUMO
The paper provides the results of differential neogilurythmal therapy in 20 patients with high-grade atrial and ventricular premature contractions in the presence of coronary heart disease. The detection of cardiac arrhythmias and evaluation of the antiarrhythmic efficacy of neogilurythmal were performed by Holter monitoring and transesophageal electrophysiological study. After the baseline studies, the antiarrhythmic efficacy of the drug was evaluated during an acute drug test and then during a 8-day course of the therapy. In the acute drug test, the dose of neogilurythmal was 50% of the daily dosage. The studies indicated that neogilurythmal in a dose of 80 mg/day was beneficial in affecting both the atrial and ventricular extrasystolic arrhythmia. The agent failed to alter heart rate, sinus nodal function and atrioventricular conduction. Thus, neogilurythmal is low toxic and produces no adverse effects when given in the definite dosage range.

Noninvasive monitor-aided transcutaneous determination of oxygen tension--TcPO2 (oxymonitor SM 361, Hellige, FRG) was performed in 32 patients aged 34 to 70 with chronic obstructive pulmonary diseases (COPD) during bicycle ergometry testing of moderate power (300 kgm/min) for 5 min. Respiration biomechanics was investigated before testing in all the patients using the method of total plethysmography (Pulmorex, Fenyves Gut, Switzerland). During testing all the patients were divided into 2 groups with relation to variations of TcPO2 values. A conclusion was made that the type of TcPO2 changes during exercise testing of COPD patients gave an opportunity for distinguishing between different types of bronchial obstruction.

A method for determination of uneven ventilation in the lungs by the transcutaneous measurement of oxygen tension in arterial blood (TcPO2) is suggested. The degree of uneven ventilation was estimated by the time of a decrease in the TcPO2 till point of departure (desaturation time) after oxygen inhalation. The TcPO2 was measured by an oxymonitor (Hellige, Model SM 361). Twelve normal controls and 24 patients with chronic obstructive lung diseases were examined. The results showed that desaturation time in controls was 3.4 +/- 0.14 min, and in patients with chronic obstructive lung diseases it was significantly prolonged till 8 +/- 0.75 min; those data were in a good correlation with helium dilution time. The method may be recommended as a diagnostic test for determination of uneven ventilation in the lungs.
