OBJECTIVES: To assess the risk variables related to the types of candidemia for each patient, who was admitted into the intensive care unit regardless of the patient with or without complete diagnosis of COVID-19, during the period of March 2019 to December 2022. METHODS: The evaluation comparison of demographic and clinical data of COVID-19 positive and negative patients with candidemia confirmed in blood, 113 cases were assessed. Variables such as gender, age, age of hospitalization, history of hospitalization, concurrently infection, The acute physiology and chronic health evaluation-II scores, comorbidity checking, intubation, central venous catheter use, parenteral nutrition use, steroid use, antibiotic use, lymphopenia, and laboratory variables were evaluated. Candida species distribution, antifungal susceptibility in blood culture were determined. RESULTS: Coronavirus disease-19 was present in 62.8% of cases confirmed candidemia, and these cases were significantly different from COVID-19 negative cases. Significance was found in more intubation, central venous catheter use, parenteral nutrition, and steroid therapy in Group 2. There was no significance with species distribution and associated infection. In total, COVID-19 positive had higher hemoglobin, aspartate aminotransferase, alanine transaminase, and white blood cell levels, which may be associated with the possibility of revealing and controlling candidemia. CONCLUSION: Candida albicans and Candida Parapsilosis (C. parapsilosis) are the species seen in infected COVID-19 patients, while C. parapsilosis and Candida tropicalis are found in non-COVID-19 ones. Risk factors were intubation, parenteral nutrition, central venous catheter, and steroid in the COVID-19 group.

Sustainable uses of the struvite (magnesium ammonium phosphate hexahydrate, MgNH4PO4·6H2O, MAP) recovered from the synthetic wastewater, as a high-quality slow-release fertilizer for the growth of nine medicinal plants and a fire-retardant barrier on the flammability of cotton fabric and wooden plate, were explored in this study. The previous experimental results demonstrated that under the optimal conditions, about 98.7% of [Formula: see text] (initial [Formula: see text] = 1000 mg/L) could be effectively and successfully recovered from simulated wastewater in the form of MAP precipitate. Rates of increase in total fresh weights, total dry weights, and fresh heights of plants grown in soil fertilized with the struvite were determined as 67%, 52%, and 12% for valerian; 121%, 75%, and 18% for cucumber; 421%, 260%, and 47% for dill; 314%, 318%, and 27% for coriander; 432%, 566%, and 30% for tomato; 285%, 683%, and 26% for parsley; 200%, 225%, and 9% for basil; 857%, 656%, and 92% for rocket; and 146%, 115%, and 28% for cress, respectively, compared to the control pots. The microstructure, elemental composition, surface area, thermal behaviour, and functional groups of the grown crystals were characterized using SEM, EDS, BET, TGA-DTG-DSC, and FTIR analyses, respectively. Flammability tests and thermal analyses concluded that the dried and crumbled/implanted form of struvite used as a fire-retardant barrier demonstrated a remarkable flame-resistant behaviour for both cotton fabric and wooden plate. Findings of this experimental study clearly corroborated the versatility of struvite as non-polluting and environmentally friendly clean product for the sustainable usage in different fields.

AIM: To evaluate maternal and cord blood irisin levels in pregnant women with gestational diabetes mellitus (GDM) and in obese pregnant women without GDM. METHODS: The study included 109 patients, with 34 patients in the GDM group, 40 in the obese non-GDM group, and 35 in the control group. Maternal serum irisin levels at the time of delivery were measured by an enzyme-linked immunosorbent assay kit. The correlation of serum irisin levels with metabolic parameters and anthropometric measurements was analyzed. RESULTS: There were significant differences between the study groups in terms of cord arterial, cord venous, and maternal serum irisin levels (P < 0.001, P < 0.01, P < 0.001, respectively). Cord arterial, cord venous, and maternal serum irisin levels were higher in the obese group compared to the control (P < 0.01, P < 0.01, P < 0.01, respectively) and the GDM group (P < 0.001, P < 0.001, P < 0.001, respectively). CONCLUSION: Elevation in irisin levels of women who have pregnancies complicated with obesity may be explained as part of the compensation mechanism against disturbed metabolic functions. Pregnant individuals with GDM have lower serum irisin levels in comparison to healthy pregnant women. In this regard, it is possible that the measurement of serum irisin levels may be utilized in the future for prediction, prevention, and treatment of GDM.

OBJECTIVE: Immediate skin-to-skin contact (ISSC) and early breastfeeding are recommended for the wellbeing of the neonate. In this study, we aimed to evaluate the effect of ISSC and early breastfeeding on maternal oxidative stress and postoperative pain. METHODS: A total of 90 patients were randomized into two groups based on the timing of skin-to-skin contact and breastfeeding. Group 1 (n = 45) was provided ISSC and breastfeeding in the operating room during the cesarean section (C/S). Group 2 (n = 45) breastfed their babies 1 h after the C/S. As markers of oxidative stress, maternal serum levels of total antioxidant status (TAS), total oxidant status (TOS) and oxidative stress indices (OSI) were evaluated. Maternal oxytocin levels and the relation between these parameters and postoperative pain were also evaluated. RESULTS: The postoperative TAS levels were significantly higher, whereas TOS and OSI levels were lower in Group 1 than Group 2. Negative correlations between oxytocin level and postoperative TOS and OSI were observed, as was a positive correlation between oxytocin level and postoperative TAS. CONCLUSIONS: The effect of ISSC and early breastfeeding on mothers was documented for the first time in this study. Our results demonstrated ISSC and early breastfeeding during C/S reduce maternal oxidative stress.

OBJECTIVE: We aimed to identify a noninvasive marker for clinically significant fetal uropathies. To achieve this aim, we detected TGF (transforming growth factor)-ß1 serum level which rises in neonatal hydronephrosis, in pregnant patients with fetal hydronephrosis. MATERIALS AND METHODS: We evaluated 44 patients, all of whom were pregnant and had a gestational age between 20 and 30 weeks. Twenty-two patients had normal maternal renal ultrasound imaging and had a fetus with fetal hydronephrosis (Group A). The remaining twenty-two patients had normal maternal and fetal renal ultrasound imaging (Group B). The maternal serum levels of TGF-ß1 were measured with a sandwich enzyme-linked immunosorbent assay (ELISA) using a commercially available kit. RESULTS: The median value for the study group was 55.90 pg/mL (9.67 ± 574.45) and for the control group was 59.49 pg/mL (12.49 ± 402.04). There was no statistical difference in serum TGF-ß1 levels between the groups (p = 0.769 - Mann-Whitney U test). In the study group, the diameter of the right renal pelvis was 5.7 mm (5.1-8.9 mm), while the diameter of left renal pelvis was 5.75 mm (5.3-10.04 mm). CONCLUSION: In our study, the circulating TGF-ß1 levels were not statistically different in the fetal hydronephrosis group when compared to the controls. According to our study, TGF-ß1 is not useful in the detection and follow-up of fetal hydronephrosis. We therefore require further studies involving larger groups with moderate or severe fetal hydronephrosis to detect the usefulness of the serum levels of TGF-ß1 in pregnant women with fetal hydronephrosis.

BACKGROUND: The potential genotoxic activity associated with high strength real paracetamol (PCT) wastewater (COD = 40,000 mg/L, TOC = 12,000 mg/L, BOD5 = 19,320 mg/L) from a large-scale drug-producing plant in the Marmara Region, was investigated in pre- and post- treated wastewater by the Fenton process (COD = 2,920 mg/L, TOC = 880 mg/L; BOD5 = 870 mg/L). METHODS: The SOS Chromotest, which is based on Escherichia coli PQ37 activities, was used for the assessment of genotoxicity. The corrected induction factors (CIF) values used as quantitative measurements of the genotoxic activity were obtained from a total of four different dilutions (100, 50, 6.25, and 0.078 % v/v.) for two samples, in triplicate, to detect potentially genotoxic activities with the SOS Chromotest. RESULTS: The results of the SOS Chromotest demonstrated CIFmax value of 1.24, indicating that the PCT effluent (non-treated) is genotoxic. The results of the SOS Chromotest showed an CIFmax value of 1.72, indicating that the wastewater treated by Fenton process is genotoxic. CONCLUSIONS: The findings of this study clearly reveal that the PCT wastewater (non-treated) samples have a potentially hazardous impact on the aquatic environment before treatment, and in the wastewater that was treated by the Fenton process, genotoxicity generally increased.

AIM: To determine whether there is a correlation between cardiac markers and peri-operative myocardial injury (PMI) and apoptosis in coronary artery bypass graft (CABG) surgery and to compare the efficacy of cardiac markers to detect PMI. METHODS: The study population consisted of 37 patients (24 male, 13 female, mean age 63.4 ± 8.9 years) undergoing elective CABG. Arterial and coronary sinus blood samples were collected just before aortic cross-clamping (pre-ACC) and after aortic declamping (post-ACC). Creatine kinase-MB isoenzyme (CK-MB) activity, and high-sensitivity cardiac troponin T (hs-cTnT), creatine kinase-MB isoenzyme mass (CK-MB mass) and cardiac troponin I (cTnI) concentrations were measured in blood samples. Myocardial injury and apoptosis were examined in atrial biopsies. RESULTS: CABG caused PMI and apoptosis in all cases. Concentrations and net releases of cardiac markers significantly increased after aortic declamping (p < 0.001 for CK-MB and CK-MB mass, p < 0.01 for cTnI, p < 0.05 for hs-cTnT). A positive correlation was found between apoptotic index (r = 0.611, p < 0.001 for cTnI; r = 0.806, p < 0.001 for hs-cTnT), myocardial injury score (r = 0.544, p < 0.001 for cTnI; r = 0.719, p < 0.001 for hs-cTnT) and cTnI and hs-cTnT values in the post-ACC period. A positive correlation was found between apoptotic index (r = 0.507, p < 0.001), myocardial injury score (r = 0.416, p = 0.010) and net release of hs-cTnT. Furthermore, a positive correlation was found between aortic cross-clamp (ACC) time (r = 0.448, p = 0.007), cardiopulmonary bypass (CPB) time (r = 0.342, p = 0.047) and net release of hs-cTnT. CONCLUSION: Although both cTnI and hs-cTnT may be specific and efficacious markers of myocardial apoptosis and injury occurring during CABG with CPB, hs-cTnT may be a more useful marker than cTnI to detect peri-operative myocardial apoptosis and injury.

The potential genotoxic activity in the surface waters of the Golden Horn Estuary was statistically evaluated utilizing a combination of appropriate parametric and non-parametric tests. The genotoxic activities that were associated with the water samples were determined by the SOS chromotest microplate assay. This assay utilizes ß-galactosidase activity, alkaline phosphatase activity, and four different solvent controls, to generate reliable results when corrected induction factors (CIF) are used as quantitative measurements of genotoxic activity. The CIF values were obtained from a total of 384 different genotoxic experiments that were grouped into subsets according to the respective seasons and the selected sampling locations. A total of 160 subsets were statistically compared to assess any possible differences between the pairs of groups, with 95% confidence limits. The findings of this study clearly indicate that some seasonal variations exist in the CIF values at several sampling sites. However, no potentially hazardous impact to the aquatic environment was found in the estuarine system.

A literature review has been undertaken to investigate the performance of the different anaerobic process configurations and operational conditions used in poultry and livestock waste treatment. The results of the extensive literature review showed that a wide range of different reactor volumes varying from 100 mL to 95 m3 were utilized in the investigation of anaerobic processing of poultry manure. Retention times studied were between 13.2 h and 91 days. Most of studies were carried out under mesophilic conditions maintained between 25 and 35 degrees C. Chemical oxygen demand (COD) removals and organic loading rate (OLR) ranged from 32 to 78%, and from 1.1 to 2.9 kg COD m(-3) day(-1), respectively. Biogas yields were achieved between 180 mL g(-1) COD added and 74 m3 day(-1) for a wide range of different reactor configurations. Up-flow anaerobic sludge blanket (UASB) seems to be a suitable process for the treatment of poultry manure wastewater and the liquid fraction of hen manure, due to its ability to maintain a sufficient amount of active biomass. The literature review showed that various reactor configurations such as fixed-film reactor, attached-film bioreactor, anaerobic rotating biological reactor, batch reactors, downflow anaerobic filter, fixed dome plant, UASB, continuously stirred tank reactor (CSTR), up-flow anaerobic filter (UAF), temperature-phased anaerobic digestion (TPAD), anaerobic hybrid reactor (AHR), and two-stage anaerobic systems are well suited to anaerobic processing of cattle manure. At both mesophilic and thermophilic conditions, high COD removals (87-95%) were achieved for treatment of cattle manure wastewaters. The COD and volatile solids (VS) reductions obtained were 37.9 to 94% and 9.6 to 92%, respectively. During the studies, OLR and retention times ranged between 0.117 and 7.3 g VS L(-1) day(-1) and between 0.5 and 140 days, respectively. In anaerobic processing of cattle manure, methane yields between 48 mmol CH4 L(-1) and 4681.3 m3 CH4 month(- 1) were found for the corresponding reactor volumes of 120 mL and 1300 m3, respectively. In anaerobic processing of swine manure, OLR ranged from 0.9 to 15.42 g VS L(-1) day(- 1) at mesophilic conditions (25-35 degrees C). The reactor volumes varied between 125 mL and 380 L. Temperature and retention times ranged from 25 to 60 degrees C, and 0.9 to 113 days, respectively. COD and VS reductions achieved were between 57 and 78% and between 34.5 and 61%, respectively. Moreover, methane yields were obtained between 22 and 360 mL CH4 g(-1) VS added. The results showed that UASB, anaerobic baffled reactors, CSTR, and anaerobic sequencing batch reactor (ASBR) were successfully utilized in anaerobic processing of swine manure at both mesophilic and thermophilic conditions.
