RESUMO

A 54-year-old man developed pupillary block resulting from pupillary capture 2 months after uneventful phacomulsification and AcrySof intraocular lens (IOL) implantation. The IOL was placed in the bag through a 6.0 mm continuous curvilinear capsulorhexis. The glaucoma was treated with intravenous drip infusion of a hyperosmotic diuretic, followed by peripheral iridectomy, iridocapsular synechiolysis, and IOL repositioning. The IOL loops were located completely in the bag. However, pupillary capture recurred 3 weeks after the surgery, at which time intraocular pressure was normal. The flexibility of the IOL optic and its large overall length and rigid, low-angulated loops were the probable causes for the recurrence of the pupillary capture. The IOL was exchanged for a sulcus-fixated, single-piece poly(methylmethacrylate) lens with 10 degree angulated loops. Pupillary capture did not recur during the follow-up.

RESUMO

Technical characteristics of a new stereoscopic ophthalmic microendoscope system developed for the direct observation of intraocular structures are described herein. Our system includes a stereoscopic visualization mechanism and an image superimposing system that projects the endoscopic image into the eyepiece of the surgical microscope, allowing the surgeon to monitor 3-dimensional endoscopic and microscopic images simply by looking into the eyepiece of the surgical microscope. Stereoscopic visualization of the intraocular structure facilitates fine manipulation of the endoscope probe just above the retina or ciliary body, providing more reliable and safer control of the probe within the eye. Preliminary clinical experience has demonstrated the usefulness of this new system.

RESUMO

PURPOSE: To clarify the relationship between suturing an intraocular lens (IOL) and residual vitreous after vitrectomy and transscleral IOL suturing. SETTING: Toranomon Hospital, Tokyo, Japan. METHODS: Enucleated pigs' eyes were fixed to the observation device. Three methods for removing the crystalline lens and the vitreous were tested; and IOL was then sutured to the ciliary sulcus. Miyake's posterior approach and an endoscope were used to observe the movement of fluorescein-stained residual vitreous during these procedures. RESULTS: Considerable residual vitreous and extensive vitreous entwinement with the IOL were seen when IOL suturing followed anterior vitrectomy through a limbal incision. These were absent when IOL suturing followed careful pars plana vitrectomy and capsulectomy. CONCLUSIONS: Our findings indicate that suturing the IOL to the ciliary sulcus should be followed by the removal of as much anterior vitreous and lens capsule as possible to prevent such postoperative complications as tractional retinal detachment and cystoid macular edema.
