RESUMO

Reinke oedema is a benign condition due to fluid collection within subepithelial space of the true vocal fold. This phenomenon is predisposed by chronic smoking, alcohol consumption and contribution of laryngopharyngeal reflux. Although benign, it may be life threatening when bilateral vocal folds are affected causing airway obstruction. In this report, awake tracheostomy was performed for a 65- year- old lady with stridor secondary to severe Reinke oedema. The importance of establishing a definitive airway is highlighted here in securing a difficult airway. Atopic dermatitis (AD) is a systemic inflammatory disorder. However, the relationship between severity of AD and metabolic and atopic conditions is not well studied. The aim of this study was to elucidate the relationship between severity of AD and comorbidities, such as metabolic conditions (increased body mass index, hypertension, hyperlipidaemia, and type II diabetes mellitus) and other atopic manifestations (allergic rhinitis, asthma and food allergies). A retrospective population-based national study of a cohort of young adult males conscripted into military service in Singapore over a 5-year period was performed, utilizing military electronic medical records. A dose-dependent effect was found between the association of comorbidities and severity of AD. Metabolic and atopic conditions were significantly correlated with having moderate-to-severe AD. An association of metabolic and atopic conditions with severity of adult AD was found, which suggests that chronic severe AD may result in increased disease burden and morbidity.

RESUMO

Imatinib mesylate is a tyrosine kinase inhibitor used in the treatment of oncological conditions, including chronic myeloid leukemia and gastrointestinal stromal tumors. The most frequent dermatological side effect reported is pigmentary abnormalities. We report a case series of three Asian Chinese females with preexisting acquired dermal melanocytosis that progressed after initiation of imatinib treatment, and concurrently developed generalized hypopigmentation of the skin. All three patients had similar histological findings on skin biopsy. It is postulated that the KIT/SCF pathway has a central role in the pathogenetic mechanism. Therefore, it is important for physicians to be aware of this potential side effect of paradoxical pigmentation in patients treated with imatinib.

RESUMO

Subungual squamous cell carcinoma is a rare entity and difficult to diagnose as its clinical presentation may resemble benign conditions. This case report highlights the need to maintain a high clinical index of suspicion, and recommends a practical approach for subungual conditions. Dermoscopy and a biopsy for histology are important adjuncts to clinch the diagnosis.

INTRODUCTION: This study aimed to report the injury or disease patterns, challenges, key observations, and recommendations by the Singapore Armed Forces (SAF) team that embarked on an Humanitarian Assistance and Disaster Relief (HADR) mission in the aftermath of the April 2015 Nepal earthquake. METHODS: The SAF medical team that provided HADR assistance to Nepal consisted of personnel from the SAF, Singapore¢s Ministry of Health and the Royal Brunei Armed Forces. Upon arrival in Kathmandu, Nepal, the SAF medical team was assigned to the Gokarna district by the local health authorities. In addition to providing primary healthcare, the medical facility was equipped to perform resuscitation and minor procedures. We also assembled mobile medical teams (MMTs) that travelled to various remote areas of the country to deliver medical aid. RESULTS: A total of 3,014 patients were managed by the SAF medical team. Of these patients, 1,286 (42.7%) were men. 574 (19.0%) patients sustained earthquake-related injuries or illnesses, while 2,440 (81.0%) sustained non-earthquake-related injuries or illnesses. The team treated a total of 447 (77.9%) adults and 127 (22.1%) paediatric patients with earthquake-related injuries or illnesses. A significant number of patients developed exacerbations of underlying medical conditions. 2,161 (71.7%) patients were treated in our main facility in Gokarna, while 853 patients (28.3%) were treated by our MMTs. CONCLUSION: The ability to transport healthcare personnel and essential medical equipment within a short time allowed the SAF medical team to provide crucial medical care in the aftermath of the 2015 Nepal earthquake.

BACKGROUND: Hypohidrosis is a risk factor for developing heat-related illnesses. OBJECTIVE: To determine the prevalence of hypohidrosis in heat injury. METHODS: Soldiers from the Singapore Armed Forces who developed heat injury between January 1, 2012 and December 31, 2013 were invited to participate in the study. Subjects were induced to sweat through exercising in a temperature- and humidity-regulated room, and then atomized starch-iodine powder was administered over their whole body to detect hypohidrosis. If this was found to be present, investigations to elucidate the cause of anhidrosis were performed. RESULTS: Out of 65 males, 30 consented to participation. One was excluded because an exogenous cause resulted in heat injury. Nine (31%) demonstrated hypohidrosis. Of these, 1 (11%) had miliaria profunda, 2 (22%) had acquired idiopathic generalized anhidrosis and 6 (67%) manifested a new phenotype which we termed acquired symmetrical hypohidrosis (ASH). CONCLUSION: Thirty-one percent of healthy soldiers who developed heat injury had hypohidrosis. Concurrently, a new clinical phenotype of ASH was identified.

BACKGROUND: Systematically planning appropriate medical coverage for mass-participation running events is a challenge that has received relatively little attention in the medical literature, despite its potentially severe consequences. In particular, the literature lacks quantitative information on running events that medical planners can utilize for decisions on medical resource allocation and deployment. METHODS: Using a case-study approach, this study provides a detailed quantitative medical services utilization profile for the Singapore Army Half-Marathon, constructed from participant and casualty data spanning three years and comprising over 80,000 data points. Casualty rates for participants of varying age and sex in different running events were also estimated using a multivariate logistic regression model. Qualitatively, planning processes and practices were described and discussed. RESULTS: The quantitative profile yielded three main findings. Firstly, the analysis reveals that the gross Medical Usage Rate had remained fairly stable at between 16.9 and 26.0 casualties per 10,000 participants over the three years. Secondly, comparing injury types, musculoskeletal and soft-tissue injuries were the most commonly-presented injuries. Thirdly, more casualties presented at the race end-point as compared to the along the race routes. The regression analysis showed that, of the four modeled variables, the longer event distance (21 km vs. 10 km) had the largest effect on the likelihood that a participant would become a casualty. Conversely, being of an older age, being male, and running in a non-competitive event were each associated with lower casualty risk. CONCLUSIONS: The stable and intuitive casualty patterns detailed in this study provide a strong basis for further quantitative research on the medical aspects of running events, as well as for mass-participation sporting events in general. The qualitative aspects of this report may serve as a useful resource to medical planners for running events.
