RESUMO

Cataract extraction in patients with chronic uveitis may be hazardous, although recent studies have indicated an improvement in prognosis using lensectomy-vitrectomy techniques in selected cases of uveitic cataracts. The results of lensectomy-vitrectomy in 12 eyes with uveitic cataract are reported. All patients had improvement in vision. No significant "flare-up" of the underlying uveitis occurred. The importance of extensive preoperative evaluation is emphasized. The major cause of decreased acuity postoperatively was persistent cystoid macular edema, a complication of chronic inflammation.

RESUMO

Cataract extraction in patients with chronic uveitis may be hazardous. RESUMO

A 71-year-old patient developed acute pupillary block glaucoma two years after uncomplicated extracapsular cataract surgery with primary implantation of a Choyce-style anterior chamber lens. He presented one week after the onset of pain. The attack was relieved with argon laser iridectomy. The presumed mechanism was rotation of the intraocular lens due to blunt trauma, occluding the previously patient single surgical iridectomy site. We felt that in this case, laser iridectomy was a good, safe alternative to surgical iridectomy.

RESUMO

We studied four cases in which the differentiation between a noncalcified suprasellar aneurysm and tuberculum meningioma, and a calcified suprasellar aneurysm and craniopharyngioma, could not be established by computed tomography scan alone. Cerebral angiography clearly showed both aneurysms.

RESUMO

One hundred and seventy Black diabetic patients who regularly attend the Baragwanath Diabetic Clinic were studied. Just over 50% of patients suffered from hypertension, and 10% had angina pectoris. Retinopathy was present in 25% of the patients. The incidence of hypertriglyceridaemia and hypercholesterolaemia was much lower than has previously been found among White diabetic patients in Johannesburg. Attention is drawn to the lack of adequate health education given to Black diabetic patients and also to the reliance on oral hypoglycaemic agents in preference to dietary therapy. It is suggested that there is a need for health educators from the patient's own ethnic group if these problems are to be overcome.

RESUMO

A case is presented of a patient who developed ocular symptoms and a skin rash after treatment with oxprenolol (Trasicor) for 2 months. The lesions improved on withdrawal of the drug and her eye symptoms became worse when she was again treated with oxprenolol. Patients who receive beta-adrenergic blocking drugs must be watched carefully for side-effects.
