RESUMO

BACKGROUND: Basal cell carcinoma (BCC) is the most common malignancy in whites, but it rarely occurs in dark persons. OBJECTIVE: To report a BCC on the hairy scalp of an Asian Indian female with no obvious risk factors except previous scalp trauma. METHODS: We review the English literature concerning BCC in Indians, and compare this with data for North American blacks and whites; and reports of BCC arising in areas of prior trauma. RESULTS/CONCLUSION: Skin cancer accounts for 1-2% of malignancies in blacks and Indians, compared with one-third of neoplasms in whites. BCC comprises 75% of skin cancers in whites, but squamous cell carcinoma represents 60-65% of skin cancers in blacks and Indians. Although most BCCs occur in sun-exposed areas in whites, blacks, and Indians, a significant percentage also develop in photoprotected areas. Trauma may be a significant risk factor for BCC, either with actinic damage or alone, as in our case.

RESUMO

Superficial basal cell carcinoma typically presents as an erythematous, scaling, mildly infiltrated patch, which may have a "pearly" border. Because these lesions share some of the clinical features of dermatitis, they are occasionally misdiagnosed for many years. We report a case of basal cell carcinoma of thirteen years' duration that presented clinically as eczematous dermatitis. We believe that this reaffirms the importance of histologic examination of longstanding skin lesions that fail to respond to standard therapy.

BACKGROUND: Keratoacanthoma is a benign epithelial tumor that occurs on the sun-exposed skin of elderly people. Development of KAS on functionally and cosmetically important areas such as the face poses the therapeutic challenge to minimize functional and cosmetic damage. Oral isotretinoin (Accutane, Roche Laboratories) has been reported to be an effective treatment of multiple as well as solitary keratoacanthomas. CASE REPORT: A 45-year-old man with a large keratoacanthoma on the nasal ala, recurrent after surgical excision, was treated with 1 mg/kg/day (80 mg/day) oral isotretinoin (Accutane). Cessation of tumor progression was evident within 1 week after initiation of therapy, and tumor regression was evident within 2 weeks. Tumor size diminished rapidly over the ensuing weeks and complete resolution at 12 weeks was confirmed by biopsy. CONCLUSION: An initial trial of oral isotretinoin is an alternative to immediate surgical excision for the treatment of large keratoacanthomas in instances when tumor removal would cause considerable cosmetic deformity.

RESUMO

Various types of running sutures have been described for use in dermatologic surgery. The running vertical mattress suture is a time-saving technique that can be used alone or in combination with any running-suture technique. It is useful for wound eversion, providing hemostasis, and closing wounds under mild tension.

RESUMO

We report an unusual case of pyogenic granuloma that appeared after the application of liquid nitrogen and 27 percent salicylic acid (Duoplant, Stiefel Inc.) to a verruca vulgaris. Only two cases of pyogenic granuloma following cryotherapy appear in the literature and pyogenic granuloma as a complication of salicylic acid application has not been documented.

RESUMO

Tissue expansion in dermatologic surgery has gained wide acceptance. We report the case of a patient with basal cell carcinoma in whom a Foley catheter was used for immediate intraoperative tissue expansion. Wound tension was decreased significantly and primary closure obtained. This technique is appropriate for office surgical procedures and provides an alternative approach to wound closure with flaps and grafts. The Foley catheter is an inexpensive and disposable tissue expander.

Focal acantholytic dyskeratosis (FAD) is considered an incidental histological finding of unknown etiology. It has been described in association with only few pathological conditions. To the best of our knowledge, we report the first case of focal acantholytic dyskeratosis occurring in condyloma acuminata.

RESUMO

Bowen's disease and other primary cutaneous malignancies are uncommon in patients with psoriasis. Primary malignancies of the umbilicus are also unusual. Plaques of psoriasis and Bowen's disease may appear morphologically similar, requiring examination of a biopsy specimen for definitive diagnosis. We report on a patient with psoriasis who experienced Bowen's disease of the umbilicus that was initially believed to be psoriasis.
