RESUMO

In 10 patients with frequent ventricular extrasystoles there were studied the antiarrhythmic effectiveness and side effects of a new drug bonnecor at a single intravenous administration in doses from 15 to 60 mg (0.17-0.91 mg/kg). It was found that the drug exerted the antiarrhythmic effect at dosages of over 0.4 mg/kg and side effects and toxic effects occurred at dosages of over 0.7 mg/kg. The optimal dose for a single intravenous administration was regarded to be the dose of 0.6 mg/kg. The mechanisms of action of bonnecor were studied during intracardiac electrophysiological investigation. The drug was shown to suppress the conduction of excitation along the atrioventricular node, the His-Purkinje system, the ventricular myocardium and the abnormal pathways of conduction. Thus, bonnecor may be referred to as an agent of class I according to the classification of Vaughan Williams. When administered intravenously (0.6 mg/kg) bonnecor was found to interrupt and prevent the recurrent development of atrioventricular tachycardia and also supraventricular tachycardias at the presence of the abnormal pathways of conduction.

RESUMO

An analysis of data of contrast ventriculography before and after volume load tests in 19 males suffering of chronic alcoholism with disorders of intraventricular conduction revealed signs of alcohol involvement of the heart with signs of "latent" insufficiency of the right and left ventricles manifested in an increased rate of circular shortening of myocardial fibers, terminal diastolic and terminal systolic volumes, terminal diastolic pressure.

Effect of a new antiarrhythmic drug allapinin was studied in 21 patients with chronic ventricular extrasystoles by an acute drug test with single oral dose of the drug controlled by 12-hour Holter monitoring. In 9 cases the dose was 50 mg. Maximal total reduction of ventricular ectopic complexes (VEC) per hour was 67 +/- 28% (mean value); in 5 cases (55%) it exceeded 90% of basal level. At peak of action, PQ interval increased by 15%, QRS--by 14%, QT was not significantly changed. In 14 cases the dose was 75 micrograms. Maximal VEC number reduction per hour was on the average 89 +/- 18% and in 12 cases (88%) exceeded 90%. PQ increased by 22%, QRS--by 23% and QT by 9% though JT was not changed (average peak values). It is thus concluded that single oral dose of allapinin has pronounced antiarrhythmic effect in patients with VEC and influences ECG intervals in the way characteristic for class IC antiarrhythmic drugs. The degree of the effect depends on the dose.

RESUMO

The authors analyse the results of their 10 years experience of surgical treatment of cardiac arrhythmia in 827 patients. Survival in the course of 3 years amounted to 73% of all the patients, in the course of a 5 year period--to 67%. The article presents experimental data substantiating the effectiveness of transvenous laser micro-transsection and cryoablation of atrioventricular connection and His-bundle in treating supraventricular tachycardias.

RESUMO

A double blind randomized cross-over study was conducted to analyze antiarrhythmic activity of the new Soviet drugs allapinin , ethacizine in comparison with mexitil. Altogether 33 patients with ventricular disorders of cardiac rhythm of high gradations after Lown B. were investigated including patients with paroxysmal ventricular tachycardia. The authors managed to establish and compare the group antiarrhythmic efficacy and safety of each drug, and to assess a degree of selective efficacy of each of them.

Antiarrhythmic effect of ethacizine, a new Soviet compound, was compared to those of etmozin and Ritmilen. Ethacizine was given to 30 patients with chronic ventricular rhythm disorders, including those with frequent extrasystoles, bigeminy, paired extrasystoles and runs of ventricular tachycardia. Coronary patients prevailed in the sample. Holter's ECG monitoring was used for the assessment of treatment efficiency. Ethacizine showed the best anti-arrhythmic effect (81%), suppressing frequent extrasystoles as well as high-grade extrasystoles in the highest proportion of cases. The lowest efficiency (38%) in combination with the highest incidence of side effects were associated with Ritmilen, while etmozin kept somewhat in between (58% response rate).

RESUMO

The results of bicycle ergometry in 115 patients with various paroxysmal rhythm disorders are reported. A comparative analysis of the rate of provoked arrhythmia paroxysms in patients with supraventricular tachycardia (SVT) and paroxysmal atrial fibrillation (PAF) is presented. The exercise test proved to be of diagnostic value in 18.5% of the SVT patients and in 16.4% of those with PAF.

The paper deals with a comparative analysis of the results of using 2 methods of investigation (daily Holter's monitoring of ECG and bicycle ergometry in patients with paroxysmal heart rhythm disturbances (group 1) and in those with extrasystole (group 2). The method of the Holter monitoring proved to be diagnostically significant in 77.2% of the first group and in all the patients of second one. In 40.9% of patients with paroxysmal rhythm disturbances and 64.7% of those with extrasystole, arrhythmias were provoked by trials with a dosed physical exercise. The comparison of both the methods revealed a concurrence of the results in 40% of the cases. A joint application of both the methods was found to enhance their diagnostic value.
