RESUMO

The uterine appendages in 87 females aged from 17 to 53 years were surgically removed because of inefficacy of conservative treatment for purulent inflammation. There were signs of chronic recurrent inflammation in the uterine tubes or in the tubes and ovaries as shown by the operative material studied light and electron microscopically, immunomorphologically. Immune mechanisms as well as structural changes (reduction, vasculitis) of the vascular bed and muscular layer (sclerosis) of the tubes played a role in the pathogenesis of chronic inflammation. The detection of the antibodies against Chlamydobacterials in 60% of the patients implies their role in pathogenesis of chronic inflammation of the uterine appendages.

RESUMO

Lung, immune organs, hypophysis and adrenals of 74 children aged from 1 month to 1 year who died from acute pneumonia are studied. It is found that fatal pneumonia in one-year-old children developed as a rule against the grave premorbid background. Late hospitalization resulted in death of 47% of the patients within first 24 hrs after admission to hospital. Invasion of pathogenic agents results in secondary immunodeficiency as a response to stress during the development of general adaptation syndrome. Preventive measures are recommended for the prophylaxis of respiratory infections in subjects with abnormal premorbid background, early hospitalization and immunomodulating treatment under the control of immune system and adrenal cortex.

RESUMO

The results of program-controlled hemodialysis (PCH) and cadaver kidney transplantation (CKT) were evaluated morphologically using 36 sections obtained from patients with end-stage chronic renal failure. For PCH-treated patients the findings indicated development of mixed secondary hypertension of the lungs associated with atherosclerotic involvement of the pulmonary artery and its branches, arteriolar rearrangements and visceral angiosclerosis. Morphologically, a successful CKT outcome resulted in late postoperative (in the time interval of 5 months--4.5 years) reduction of the signs typical for pulmonary vasculature affection with hypertension of the lesser circulation. This can suggest possibility of a reversible course of pulmonary hypertension in relevant patients.

RESUMO

Discrepancies between clinical and pathological diagnoses (classes 1-3) attributed to subjective factors have been analyzed for the period 1981-1985. The investigation pooled the data from three largest Moscow hospitals: S. P. Botkin, No. 1 and 7. Subjective misdiagnosis comprises from 34 to 45.6% of overall diagnostic discrepancies. As a rule, it resulted from inadequate clinical examination, underestimation of clinical evidence, overestimation of anamnestic data, misinterpretation of laboratory findings and those of consulting physicians as well as provided by adjuvant investigations. The errors of the above kind are most typical for patients over 60. The authors refer these diagnostic discrepancies to iatrogenesis.

RESUMO

The analysis of 550 records of autopsies of patients who had suffered from chronic renal insufficiency and had been treated by transplantation of corpse kidney, programme hemodialysis, and conservatively, was carried out. Significant differences in the structure of complications and causes of death in these kinds of correction of the terminal stage of chronic renal insufficiency were established. Each of them has its own most frequent causes of complications and fatal outcomes associated both with the main disease and the type of treatment used.
