[Osteotomy of the humerus in the operative treatment of children with complications of obstetrical Duchenne-Erb paralysis]. / Primenenie osteotomii plechevoi kosti v operativnom lechenii detei s posledstviiami rodovogo paralicha tipa Diushena-Erba.

From 1987 to 1991, 33 children with sequelae to Duchenne--Erb type obstetrical paralysis of an upper extremity were operated on in the clinic. In 20 patients, the operations were performed on the tendinomuscular apparatus of the humeral articulation, in 13--on the humerus. The technique for performance of detorsive osteotomy of the humerus is described. Indications for the given method of operative rehabilitation are substantiated.

[The incidence and prognosis of obstetric paralysis of the upper extremity]. / Chastota i prognoz rodovogo paralicha verkhnei konechnosti.

A total of 122 babies with birth paralysis of the arm were treated at the department for diseases of the newborns in 1984-1987 in the town of Yaroslavl. The incidence of this suffering has made up 1.5 per 1000 of babies born alive. Characterization of the pregnancies, deliveries, and the newborns is presented. Risk factors in respect of such paralyses are distinguished. Late results of conservative treatment were followed up in 80 cases. Functional and cosmetic aftereffects of birth paralyses were seen in 27.5% of cases.

Within 1987-1989, surgical correction of sequelae to obstetrical paralysis of an upper extremity of the Duchenne-Erb type was accomplished in 26 children. In 11 cases, the operations of transposition and lengthening of the adductor brachial muscles were supplemented with a non-free transfer of a thoracico-dorsal flap on a neurovascular bundle to the position of a musculus deltoideus. With the aim of active functional influencing on a myotransplant at the early postoperative period, the neural stimulation and adaptive bioregulation with the outer feedback were used. For control, the continuous electroneuromyography was used. A good functional result was obtained.

Closed dislocations of finger bones is a rare injury in children. Most often complications are localized in the metacarpophalangeal articulation of the first finger. The main method of treatment of fresh dislocations is a one-step closed hand reposition. In case of its failure an open setting of the dislocation should be performed along with liquidation of the cause of interposition. The method of choice of treatment of neglected dislocations of fingers in children is the application of a compression-distraction apparatus.

[Restoration of the function of the hand after combined injuries of the flexor tendons, nerves and arteries of the distal part of the forearm in children]. / Vosstanovlenie funktsii kisti posle sochetannykh povrezhdenii sukhozhilii sgibatelei, nervov i arterii distal'nogo otdela predplech'ia u detei.

The authors observed 25 children with combined injuries of flexor tendons and vascular nervous fascicles on the level of the lower third part of the forearm and the carpal canal. During the initial operation the flexors and the nerves were restored and the nerves were ligated in all patients. Precision equipment was employed when putting in the epineural stitches. A complex of electrophysiological non-invasive and clinical methods of investigation of regional hemodynamics and the neuromuscular apparatus of the hand were employed during this study. Long-term clinical and functional results of treatment of this category of patients are presented.
