RESUMO

Clinical and clinico-pathoanatomical comparisons (60 observations) have shown that in patients with primary cutaneous melanoma and its metastases treated by modern methods the tumour metastasis to the brain occurs in late periods of the disease at the stage of the neoplastic process generalization. Histological examinations of the melanoma metastases of various localization revealed a much lesser differentiation of the tumour cells and a greater vascularization of the tumour. The pronounced vascularization is characteristic for metastases of any localization, both visceral and cerebral, at the stage of the process generalization.

Under examination there were 37 patients with uterine, and 3 with testicle chorioepithelioma with metastases to the brain (35 patients) and the spinal cord (5 patients). The particular malignancy of the tumour which rapidly involved the vessels was the cause of perifocal hemorrhages and hemorrhages to the brain matter. This makes the clinical course of the disease especially grave. Twenty-eight patients died: most of them had multiple metastases to the brain. Twelve patients were dismissed: 5 of them with a certain improvement of the state and 7 with the disease still progressing.

RESUMO

A description of 21 patients suffering from lympho- and reticulosarcomas with metastatic affections of the cerebral and spinal meninges (20 patients) and an intracerebral solitary tumour (one patient) is presented. In 17 cases the diagnosis was confirmed on autopsy. The neurological picture was similar to the clinical picture of basal (7 patients) or cerebrospinal (9 patients) meningitis. Isolated spinal lesions were infrequent (4 patients). The cerebrospinal liquor showed, as a rule, a protein-cellular dissociation; in individual cases a high blastic pleocytosis was observed. A favourable effect on the disease course was produced by methotrexate which was injected endolumbarly.
