RESUMO

Zirconia, a crystalline oxide of zirconium, holds good mechanical, optical, and biological properties. The metal-free restorations, mostly consisting of all-ceramic/zirconia restorations, are becoming popular restorative materials in restorative and prosthetic dentistry choices for aesthetic and biological reasons. Dental zirconia has increased over the past years producing wide varieties of zirconia for prosthetic restorations in dentistry. At present, literature is lacking on the recent zirconia biomaterials in dentistry. Currently, no article has the latest information on the various zirconia biomaterials in dentistry. Hence, the aim of this article is to present an overview of recent dental zirconia biomaterials and tends to classify the recent zirconia biomaterials in dentistry. This article is useful for dentists, dental technicians, prosthodontists, academicians, and researchers in the field of dental zirconia.

BACKGROUND: Although there are a few studies which compare fast and slow sintering in normal zirconia crowns, it is essential to compare the cracks and load-bearing capacity in zirconia screw-retained implant crowns between regular and speed sintering protocols. This research aimed to compare the surface structure, cracks, and load-bearing capacity in zirconia screw-retained implant crowns between regular sintering (RS) and speed sintering (SS) protocol with and without cyclic loading (fatigue). METHODS: A total of 60 screw-retained crowns were fabricated from zirconia (Katana STML Block) by the CAD/CAM system. Then, 30 crowns were subjected to the RS protocol and 30 crowns were subjected to the SS protocol. Cyclic loading was done in half zirconia crowns (15 crowns in each group) using a chewing simulator CS-4.8/CS-4.4 at room temperature. The loading force was applied on the middle of the crowns by a metal stylus underwater at room temperature with a chewing simulator at an axial 50 N load for 240,000 cycles and lateral movement at 2 mm. Scanning electron microscopy was done to study the surface of the crowns and the cracks in the crowns of the regular and speed sintering protocols, with and without fatigue. RESULTS: For the speed sintering group, the surface looks more uniform, and the crack lines are present at a short distance compared to regular sintering. The sintering protocol with a larger Weibull module and durability increases the reliability. It showed that the Speed group showed the maximum fracture load, followed by the regular, speed fatigue, and regular fatigue groups. The fracture load in various groups showed significant differences. CONCLUSIONS: It was found that the speed group showed the maximum fracture load followed by the regular, speed fatigue, and regular fatigue. The crack lines ran from occlusal to bottoms (gingiva) and the arrest lines were perpendicular to the crack propagations.

Translucent zirconia is becoming the material of choice for the esthetic restorative material. We aimed to evaluate the surface structure, phase determination, translucency, and flexural strength of 5Y-TZP Zirconia (Katana STML Block and Disc) between the regular sintering and the speed sintering with and without low-temperature degradation (LTD). A total of 60 zirconia discs (30 per group; regular sintering and speed sintering) were used in this study. A CAM machine was used to mill cylinders out of the zirconia blanks and then cut into smaller discs. For the speed sintering, the zirconia blocks were milled into smaller discs. The zirconia discs were subjected to regular and speed sintering with and without LTD. Scanning electron microscopy was used to characterize the zirconia specimens and the zirconia grain size. Furthermore, the zirconia specimens were analyzed for elemental analysis using energy dispersive spectroscopy and phase identification using X-ray diffraction. The zirconia specimens were subjected to translucency measurements and biaxial flexural strength testing. The results of the zirconia specimens were compared among the groups. Statistical analysis was completed using SPSS version 20.0 to detect the statistically significant differences (p value = 0.05). A one-way ANOVA with multiple comparisons was performed using Scheffe analysis among the groups. The speed sintering presented smaller grain sizes. The zirconia specimens with and without LTD in regular and speed sintering presented a similar surface structure. Regular sintering showed more translucency compared to speed sintering. Multiple comparisons of the translucency parameter were a significant difference (p value < 0.05) between the various groups except for the comparison between speed sintering and speed sintering LTD. The regular sintering showed bigger gain sizes and slightly more translucency compared to speed sintering. The speed sintering showed higher biaxial flexural strengths compared to regular sintering. This shows that speed sintering can be considered a suitable method of sintering zirconia.

RESUMO

This case report presented a fully digital workflow in esthetic dental restoration. A 51-year-old female patient was referred to BDMS Wellness Clinic due to a fracture of the maxillary left central incisor. An immediate dental implant was planned to restore tooth 21 with esthetic crown lengthening of upper front teeth and new zirconia crowns for teeth 11, 12, 13, 21, 22, and 23. Digital impression was made using a digital scanner (PRIMESCAN®, Dentsply Sirona, Bensheim, Germany); surgical guide (Cerec Guide 3, Dentsply Sirona, Bensheim, Germany) was designed by using a designing software (Galileos Software, Dentsply Sirona, Bensheim, Germany) and was milled by using a milling machine (MCXL milling machine, Dentsply Sirona, Bensheim, Germany) to create a precise surgical guide. 3D smile design was done by using the Digital Smile Design (DSD) program, the crown lengthening guide was designed according to DSD designed by using a designing software (Inlab 19 Software, Dentsply Sirona, Bensheim, Germany), and the guide was fabricated by a 3D printer (FormLabs Form 2, Formlab, MA, USA). Provisional crowns (splinted) for teeth 11, 12, 13, 21, 22, and 23 were milled by using polymer (VIPI BLOCK TRILUX®, VIPI Industria, Pirassununga, SP, Brazil) using a milling machine (MCX5, Dentsply Sirona, Bensheim, Germany). The zirconia crowns were designed by using software (Inlab19 Software, Dentsply Sirona, Bensheim, Germany) and milled using the same milling machine. At the implant position, Ti-base is cemented by using an abutment (Multilink Hybrid Abutment, Ivoclar Vivadent, Schaan, Lichtenstein, Germany) with zirconia coping (Cercon base white shade, Dentsply Sirona, Bensheim, Germany) utilized as the abutment. The zirconia crowns (Cercon Xt, Dentsply Sirona, Bensheim, Germany) were sintered and characterized and then cemented. The patient was satisfied with the esthetic outcome of the treatment.

Background: COVID-19 has spread worldwide and caused severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) led to numerous dead cases. However, with the new COVID-19 outbreaks, there is a shortage of personal protective equipment (PPE) especially N95 masks worldwide including Thailand. This issue had placed the health professional in great need of an alternative mask. Aim: This study aimed to measure the fit factor of 3D printed frames by quantitative fit testing (QNFT) to find an alternative facemask by using a mask fitter together with 2 different kinds of the American Society for Testing and Materials (ASTM) level 1 surgical mask. Materials and Methods: Two commonly used surgical masks (Sultan Com-Fit Super Sensitive Ear Loop Mask or "White Mask Group," not water-resistant, and Sultan Blue Com-Fit Super High Filtration Ear Loop Mask or "Blue Mask Group," water-resistant) with and without 3D printed frame covering. The fit performance was measured by a quantitative fit test (QNFT) device (PortaCount, model 8048, TSI Incorporated, Minnesota, USA) accepted by the Occupational Safety and Health Administration (OSHA). The PortaCount device, which is based on a miniature continuous flow condensation nucleus counter (CNC), assesses the respiratory fit by comparing the concentration of ambient dust particles outside the surgical mask to the concentration that has leaked into the surgical mask. The ratio of these two concentrations (Cout/Cin) is called the fit factor. A fit factor of a 3D printed frame of at least 100 is required and considered as a pass level. Results: We found that the mask fitter improves the overall performance of surgical masks significantly. The improved performance is comparable to that of N95. Conclusion: The mask fitter improves the performance of surgical masks. The authors suggested that further study on frame material, shape, and expanded sample size would be beneficial to society.

Dentin matrix protein 1 (DMP1) contains a large number of acidic domains, multiple phosphorylation sites, a functional arginine-glycine-aspartate (RGD) motif, and a DNA binding domain, and has been shown to play essential regulatory function in dentin and bone mineralization. DMP1 could also orchestrate bone matrix formation, but the ability of DMP1 on Ti to human mesenchymal stem cell (hMSC) conversion to osteoblasts has not been studied. There is importance to test if the DMP1 coated Ti surface would promote cell migration and attachment to the metal surface and promote the differentiation of the attached stem cells to an osteogenic lineage. This study aimed to study the human mesenchymal stem cells (hMSCs) attachment and proliferation on DMP1 coated titanium (Ti) disks compared to non-coated disks, and to assess possible osteoblastic differentiation of attached hMSCs. Sixty-eight Ti disks were divided into two groups. Group 1 disks were coated with dentin matrix protein 1 and group 2 disks served as control. Assessment with light microscopy was used to verify hMSC attachment and proliferation. Cell viability was confirmed through fluorescence microscopy and mitochondrial dehydrogenase activity. Real-time polymerase chain reaction analysis was done to study the gene expression. The proliferation assay showed significantly greater cell proliferation with DMP1 coated disks compared to the control group (p-value < 0.001). Cell vitality analysis showed a greater density of live cells on DMP1 coated disks compared to the control group. Alkaline phosphatase staining revealed higher enzyme activity on DMP1 coated disks and showed itself to be significantly higher than the control group (p-value < 0.001). von Kossa staining revealed higher positive areas for mineralized deposits on DMP1 coated disks than the control group (p-value < 0.05). Gene expression analysis confirmed upregulation of runt-related transcription factor 2, osteoprotegerin, osteocalcin, osteopontin, and alkaline phosphatase on DMP1 coated disks (p-value < 0.001). The dentin matrix protein promoted the adhesion, proliferation, facilitation differentiation of hMSC, and mineralized matrix formation.

Recently, translucent zirconia has become the most prevalent material used as a restorative material. This study aimed to compare the crown fracture load of the four most common different translucent zirconia brands available in the market at 1.5 mm thickness. Standardized tooth preparations for a full ceramic crown were designed digitally with software (AutoCAD) by placing a 1.0 mm chamfer margin and 1.5 mm occluso-cervical curvature for the crown sample manufacturing. Stylized crowns were chosen to control the thickness of the crown. The axial and occlusal thickness were standardized to 1.5 mm thickness except at the central pit, which was 1.3 mm thick. The STL file for the tooth dies was prepared using software (3Shape TRIOS® Patient Monitoring, Copenhagen, Denmark). The tooth dies were printed with a resin material (NextDent Model 2.0, Vertex-Dental B.V., Soesterberg, The Netherlands) using a 3D printing software (3D Sprint® Client Version 3.0.0.2494) from a 3D printer (NextDent™ 5100, Vertex-Dental B.V., Soesterberg, The Netherlands). The printing layer thickness was 50 µm. Then, a total of twenty-eight (N = 28) stylized crowns were milled out of AmannGirrbach (Amann Girrbach GmbH, Pforzheim, Germany) (n = 7), Cercon HT (Dentsply Sirona, Bensheim, Germany) (n = 7), Cercon XT (Dentsply Sirona, Bensheim, Germany) (n = 7), and Vita YZ XT (Zahnfabrik, Bäd Sackingen, Germany) (n = 7). Following sintering the crowns, sandblasting was performed and they were bonded to the tooth dies with the resin cement (RelyX U-200, 3M ESPE, Seefeld, Germany) and permitted to self-cure under finger pressure for 6 min. The crowns were loaded on the occlusal surface in a universal testing machine (MTS Centurion) with a stainless-steel ball indenter (7 mm radius) with a loading rate of 1 mm/min to contact the stylized crowns on each of the four cusps until failure. A rubber sheet (1.5 mm thickness) was positioned between the crown and indenter, which helped with the load distribution. Statistical analysis was done using SPSS version 20 (IBM Company, Chicago, USA). The fracture loads were analyzed using Dunnett's T3 test, and the number of cracks was analyzed using the Mann-Whitney U test among the groups. The significant level was set at p value = 0.05. The mean fracture loads were 3086.54 ± 441.74 N, 4804.94 ± 70.12 N, 3317.76 ± 199.80 N, and 2921.87 ± 349.67 N for AmannGirrbac, Cercon HT, Cercon XT, and Vita YZ XT, respectively. The mean fracture loads for the surfaces with the greatest number of cracks (excluding the occlusal surfaces) were on the lingual surface for AmannGirrbach and Cercon HT, on the distal and mesial for Cercon XT, and on the buccal for Vita YZ XT. We found that the AmannGirrbach had the most overall cracks. Cercon XT had the greatest number of occlusal cracks and appeared to be the most shattered. Cercon HT had the least number of cracks. In conclusion, Cercon HT presented the best strength properties, the highest fracture load, and no visible cracks. AmannGirrbach presented the lowest strength properties.

BACKGROUND: We examined outcomes in patients treated for radioactive iodine-induced sialadenitis (RAIS) and xerostomia with sialendoscopy. METHODS: Data was prospectively collected for all patients undergoing sialendoscopy for RAIS from a single institution. Interventional details and intraoperative findings were recorded. Qualitative data were obtained through patient examination, telephone interviews, and use of a standard quality of life questionnaire, Xerostomia Questionnaire. Quantitative data were obtained from patients who underwent sialometry. RESULTS: Twenty-six patients (24 women and 2 men; median age, 43 years; age range, 19-57 years) underwent interventional sialendoscopy after conservative management of symptoms proved unsuccessful. Sialadenitis was present in 25 patients and xerostomia in 22 patients. Mucus plugging in the duct of the gland was the most common finding (22 patients) followed by stenosis (18 patients), inflammation (eight patients), and erythema (eight patients). Median follow-up time was 23.4±12.1 months. Sixteen patients (64%) reported complete resolution; seven (28%), partial resolution; one (4%), no change in symptoms; and one (4%), regression in RAIS-related symptoms. Patients subjectively noted the following regarding their xerostomia symptoms: seven (31.8%) had complete resolution; 10 (45.5%), partial resolution; four (18.2%), no change; and one (4.5%), regression. Statistical analysis of the available sialometry data revealed a statistically significant difference in saliva production at 6 months following sialendoscopy for unstimulated saliva production (p=0.028). CONCLUSION: Sialendoscopy is an effective treatment option for the management of RAIS and xerostomia refractory to conservative therapy and medical management. Patients in our cohort report durable improvement in symptoms after intervention.

PURPOSE: The purpose of this study was to examine gender disparities in prosthodontics by reviewing the trend of female authorship in prosthodontic journals and exploring the role of female leadership in prosthodontic organizations and Advanced Education in Prosthodontic (AEP) programs. MATERIALS AND METHODS: Three journals representing the prosthodontic specialty were selected to analyze the percentage of female dentist first and last (senior) authors for the years 1995, 2000, 2005, and 2008. Article inclusion criteria were restricted to the first or last authors who held at least a DMD/DDS/BDS degree and were from U.S. institutions. Data on female leadership in prosthodontic organizations and advanced education programs were collected, and the trends were studied. Descriptive statistics were used to analyze the data. A linear regression analysis was performed to investigate the proportion of female authorship compared to male in the dental literature. A Fisher's Exact Test was performed to contrast differences of female first and last authorship in the selected journals between years 1995 and 2008. RESULTS: Overall, there was no statistically significant linear increase in the proportion of either first or last female authorship compared to male authorship over time. With respect to each journal, the linear regression analysis showed that the increase of first female authorship was statistically significant (p= 0.016) compared to male authorship only in the Journal of Prosthetic Dentistry. The percentage of female presidents of prosthodontic organizations has been very limited. A similar trend was also observed in AEP program director positions. CONCLUSIONS: Over the past 13 years, female dentists' participation in prosthodontics literature authorship has not increased significantly in the United States. Furthermore, female involvement in prosthodontics leadership has been limited over the past decades.

The purpose of this study was to examine gender disparities in dental leadership and academics in the United States. Nine journals that represent the dental specialties and high published impact factors were selected to analyze the percentage of female dentists' first and senior authorship for the years 1986, 1990, 1995, 2000, 2005, and 2008. Data on appointment status and female deanship were collected from the American Dental Association (ADA) survey, and the trends were studied. The proportion of female presidents in ADA-recognized specialty organizations was also calculated. Overall, the increase in first female authorship was not statistically significant, but the increase of last female authorship was statistically significant in a linear trend over the years. The percentage of tenured female faculty members and female deans in U.S. dental schools increased by factors of 1.7 and 9, respectively, during the study period. However, female involvement in professional organizations was limited. Findings from this study indicate that female participation in authorship and leadership has increased over time. Nevertheless, females are still a minority in dental academics and leadership.
