RESUMO

A case study of an acute tuberculous bronchiolitis (a rarely encountered form of generalized miliary tuberculosis), whose aggravated progressing resulted in fatal outcome 2 months after the onset of the initial disease signs, is described. The prevalence of non-specific infectious-and-allergic manifestations is a clinical-and-morphological peculiarity within the case study, which confirms a diversity of morphological changes occurring in the organism in tuberculosis infection. The etiological- and pathogenetic specific features of acute tuberculous bronchiolitis in children and in adults are under discussion; in particular, a possible role of a preceding viral infection is evaluated. It is one of the cases of hematogenic generalization in primary tuberculosis of adults, observed by the authors, when an involvement of the pulmonary and parenchymatous organs as well as of arachnoids and substances of the cerebrum was registered. The presence of generalized vasculitis with fibrinoid swelling and with necrosis of collagenous fibers of medium (and with the development of pan-vasculitis in the discussed case) is typical of all such observations.

RESUMO

The clinical picture of acute miliary tuberculosis is characterized in 120 patients followed up by the author in 1975-1996. The patients' age varied from 17 to 62 years. Variants of clinical forms of miliary tuberculosis, specific features of the course tuberculous meningoencephalitis in miliary tuberculosis are given. In the past 3 years, the clinical picture of the disease has aggravated and its extremely severe malignant forms appeared.

RESUMO

Clinical, x-ray and morphological characteristics are presented for acute tuberculous pneumonia in 56 patients. The diagnosis was established intravitally in 33, postmortem in 23 of them. The disease started acutely and terminated fulminantly in all the patients. A morphological study stated predominance of exudative-necrotic reaction without phasic and productive changes, the presence of M. tuberculosis in the tissues.

RESUMO

The results of ++clinico-serological examination of 81 subjects with meningeal diseases are given; among them 53 had tuberculous meningitis. The authors applied complement fixation (CFT) and indirect hemagglutination tests (IHT) with tuberculin. The high diagnostic significance of CFT and low efficiency of IHT were established in tuberculous meningitis detection. It is expedient to determine simultaneously the antituberculous antibodies in the blood serum and cerebrospinal fluid of the examined patients. Simultaneous performance of CFT during the follow-up increases its diagnostic potentialities to 88.6%. Detection of complement-binding antibodies only in CFT is, in most cases, a sign of isolated tuberculous meningitis. Detection of specific antibodies only in the blood serum of tuberculous meningitis patients is an indirect evidence of generalized process.

RESUMO

The article describes the versions of meningeal tuberculosis in 300 patients observed by the author from 1975 to 1990. In the majority of the patients, the onset of the disease was gradual and its course, typical. For an acute onset of the disease, certain types of serous meningitis (6.7%), influenza (2.3%), acute psychosis (3%), purulent meningitis (1.3%), craniocerebral injury (1%), seriously disturbed cerebral circulation (4%) and lethargic encephalitis (0.66%) were identified. Chronic forms were mainly characterized by fibroplastic processes, local focal neurologic symptoms and congestive changes in the eye ground. The full-scale picture of the disease was sometimes observed at the terminal stage only. In 5% of the patients, such neurologic symptoms, as aphasias and paralyses, were prevalent in the clinical picture. The final diagnosis of a follow-up and repeated examination of the cerebrospinal fluid.
