[Efficiency of combined methods of hemorroid treatment using hal-rar and laser destruction]. / Éffektivnost' kombinirovannoi metodiki lecheniia gemorroia s ispol'zovaniem HAL-RAR i lazernoi destruktsii uzlov.

AIM: To develop the combined method of treatment of hemorrhoids with arterial ligation under Doppler control and laser destruction of internal and external hemorrhoids. MATERIAL AND METHODS: The study included 100 patients with chronic hemorrhoids stage II and III. Combined method of HAL-laser was used in study group, HAL RAR-technique in control group 1 and closed hemorrhoidectomy with linear stapler in control group 2. Сomparative evaluation of results in both groups was performed. RESULTS: Combined method overcomes the drawbacks of traditional surgical treatment and limitations in external components elimination which are inherent for HAL-RAR. Moreover, it has a higher efficiency in treating of hemorrhoids stage II-III compared with HAL-RAR and is equally safe and well tolerable for patients. This method does not increase the risk of recurrence, reduces incidence of complications and time of disability.

[Laparoscopic peritoneoanal resection of rectum with coloanal anastomosis].

65 laparoscopic peritoneoanal resections were performed. 80 patients, who underwent the traditional open surgery, comprised the control group. The mean operative time for the laparoscopic procedure was 202+/-47.7 min. Conversion was performed in 2 cases. A delayed transversostomy was performed in 10 cases because of the coloanal anastomosis ischemia. Lethality rate was 3%. 5-year survival was 76.9% after the radical laparoscopic procedure.

[Minilaparotomy in surgical treatment of calculous cholecystitis]. / Minilaparotomiia pri khirurgicheskom lechenii kal'kuleznogo kholetsistita.

A set of instruments devised by the CAN company was used in performing cholecystectomy in 15 patients. It proved adequate for the operative situation and allowed surgical manipulations to be carried out through a 30-50 mm minilaparotomy. Cases in which the approach had to be widened or intraoperative complications occurred were not encountered. The indices of external respiration function and the volumetric parameters decrease a little but are rapidly restored during the postoperative period. This confirms the mild traumatic character of the operation and is consistent with the absence of pulmonary complications and negligible manifestation of postoperative intestinal paresis.
