RESUMO

Chronic pain often has an unknown cause, and many patients with chronic pain learn to accept that their pain is incurable and pharmacologic treatments are only temporarily effective. Complementary and integrative health approaches for pain are thus in high demand. One such approach is soft touch, e.g., adhesion of pyramidal thorn patches in a pain region. The effects of patch adhesion on pain relief have been confirmed in patients with various types of pain. A recent study using near-infrared spectroscopy revealed that the dorsolateral prefrontal cortex (DLPFC), especially the left side, is likely to be inactivated in patients experiencing pain relief during patch treatment. Mindfulness meditation is another well-known complementary and integrative approach for achieving pain relief. The relation between pain relief due to mindfulness meditation and changes in brain regions, including the DLPFC, has long been examined. In the present review article, we survey the literature describing the effects of the above-mentioned complementary and integrative treatments on pain relief, and outline the important brain regions, including the DLPFC, that are involved in analgesia. We hope that the present article will provide clues to researchers who hope to advance neurosensory treatments for pain relief without medication.

Pain in the elbow, shoulder, knee, lower back, and various other joints is relieved by adhesion of pyramidal thorn patches. To elucidate the pain relief mechanism induced by the patches, we established a quantitative method for estimating the pain reduction and investigated the brain regions that change in association with pain relief. We first attempted to quantify the pain relief using transcutaneous electric stimulation (TCES) and a visual analog scale (VAS), and then applied near-infrared spectroscopy (NIRS) to the prefrontal cortex, including the dorsolateral prefrontal cortex (DLPFC) and the orbitofrontal cortex (OFC). We also examined the salivary oxytocin levels, which are thought to reflect oxytocin secretion levels from the posterior pituitary in the brain. Application of pyramidal thorn patches to pain regions decreased the pain degree estimated using TCES and VAS. Oxyhemoglobin levels were likely to be decreased in the left DLPFC on the basis of NIRS measurements during patch treatment, suggesting that the left DLPFC is involved in pain relief. On the other hand, the salivary oxytocin levels varied widely. A potential reason for the varying salivary oxytocin levels is its utilization in the pain region as an analgesic agent. Our results suggest that the left DLPFC will become a target brain region for pain therapy.

In some types of meditation, such as mindfulness and Zen, breathing is the focus of attention, whereas during an excessive, short-period of anaerobic exercise, the muscles become the focus of attention. Thus, during both efforts, one's attention is focused on a certain feature of the body. Both meditation and exercise generally provide mental refreshment to humans. We hypothesized that the same brain regions are activated by both efforts in humans. To examine this hypothesis, we engaged participants in 3 tasks: meditation, exercise, and a control task. After each task, the participants underwent a 2-back test to concentrate their thoughts, while changes in their blood hemoglobin levels were simultaneously monitored using near-infrared spectroscopy (NIRS). Seventeen participants (20-24 years of age; 11 men, 6 women) were enrolled. We applied a fast-Fourier transform (FFT) analysis to the NIRS wave data and calculated the correlation coefficients of the FFT data between (1) meditation and control, (2) exercise and control, and (3) meditation and exercise, at the orbitofrontal cortex (OFC) and dorsolateral prefrontal cortex (DLPFC), brain areas that are generally involved in mental refreshment. A significant difference in the correlation coefficients between the OFC and DLPFC was detected in the meditation and exercise analysis, and signal source analysis confirmed that the NIRS waves spread from the right and left OFC edges (i.e., right and left temples) toward the center. Our results suggest that both meditation and exercise activate the OFC, which is involved in emotional reactions and motivation behavior, resulting in mental refreshment.

RESUMO

The aim of this study was to investigate the link between response styles and depression in university students. Ninety-two university students participated in a questionnaire survey, withi a follows-up survey administered 4 weeks later. Structural equation modeling was conducted with analysis of moment structures. The model describing the causal relation from response style to depression fit the data best. These results indicate that the likelihood of future depression is increased by negative rumination response and decreased by distraction response for mood changing.

Mechanisms of Dohsa-hou (a holistic psycho-rehabilitation method) were investigated by comparing the effects of attention to body sensation and muscular relaxation. Participants were randomly divided into the following three groups. The Dohsa + Attention (D + A) group practiced Dohsa-hou by focusing attention on somatic sensations. The Dohsa (D) group practiced Dohsa without focusing attention. The control group did not practice Dohsa. All the participants were assessed using the Decentering Scale, the Self-Acceptance Scale, and the Depression and Anxiety Mood Scale before and after the intervention. The results indicated that both the D + A and D groups had increased self-acceptance and decreased anxiety scores. However, in comparison to the control group, the decentering score increased in the D + A group (p < .05), whereas the Depression score decreased in the D group (p < .05). These findings suggest that attention to body sensations is effective in increasing decentering, whereas muscular relaxation is effective in decreasing depressed mood.

Introducing the ways of cultivating mental balance, B. A. Wallace and S. L. Shapiro attempted to build bridges between Buddhism and psychology. Their systematic categorization of Buddhist teachings and extensive review of empirical support from Western psychology are valuable for future study. However, it remains a matter of concern that some more profound parts of Buddhist philosophy can be disregarded by focusing only on practical aspects of Buddhism within the context of mental health. In this comment, the authors briefly address four substantial themes to be considered: reality, identity, causality, and logicality. They suggest that the way to interpret Buddhism as techniques for well-being would certainly be viable in encouraging the study of Buddhist teachings in psychology. Yet, such attempts should not result in superficial imports and applications of Buddhist practices but give due weight to the deeper philosophical issues to build more solid bridges between Buddhism and psychology.

The present study developed and evaluated the Positive Automatic Thoughts List (PAL) to explore the roles and function of self-talk in a Japanese population in positive situations. In Study 1, 22 items were chosen to construct the PAL. Five factors were identified, I . Expressions of Positive Emotion, II. Positive Expectations of Future and Self, III. Positive Self-Evaluation, IV. Having Social Support, and V. Hope for Maintaining Positive Mood. All these factors had high degrees of internal consistency. Factors I and V have never been identified in previous studies, these factors may characterized Japanese positive automatic thoughts. In Study 2, the concurrent validity of the PAL was examined. The results showed that the PAL total scores were significantly correlated with the Self-rating Depression Scale (r= -.29) and the Automatic Thoughts Questionnaire-Revised (Positive: r=.46, Negative: r= -.16). Study 3 confirmed the clinical validity of the PAL, discriminating between healthy undergraduates and depressed outpatients. We discussed that Factor I could be closely related to the increase of positive affect and to the maintenance of depressive states.

The purpose of this study was to compare the psychological effects of the heaviness and the warmth Standard Exercises (SE) with those of the first Space Exercises (SP) in Autogenic Training (AT) on college students. This study was conducted by the repeated practice of two experimental groups (SE, SP) and one control group (CT). The participants in the experimental groups practiced once a week, for three weeks. All the participants were assessed with State-Trait Anxiety Inventory-Trait Anxiety (STAI-T), Autogenic Training Clinical Effectiveness Scale (ATCES), and Rosenberg's Self-Esteem Scale. The results showed that the SE group showed more decreases in STAI-T and more increases in ATCES than the SP group, while the more neurotic, showed more increases in self-esteem in both the SE and SP groups. It is possible that non-clinical adults with high neuroticism might be able to enhance their STAI-T and ATCES in the SE group than in the SP group if their SE trainers respected their bodily abilities in readily responding to changes in sensations.

The purpose of the present study was to develop a scale measuring clinical effectiveness of autogenic training. In Study 1, 167 undergraduates completed a survey of items concerning physical and mental states, which were thought to vary in the course of autogenic training. With item and factor analyses, 20 items were selected, and the resulting scale (ATCES) had high discrimination and clear factor structure. In Study 2, reliability and concurrent and clinical validity of the scale were examined with three groups of respondents: 85 mentally healthy, 31 control, 13 clinical persons. The scale showed a high test-retest correlation (r = .83) and alpha coefficient (alpha = .86). ATCES had a Pearson correlation coefficient of r = .56 with General Health Questionnaire (GHQ-12), and r = .73 with trait anxiety (STAI-T). And ATCES successfully discriminated the mentally healthy and clinical groups in terms of clinical effectiveness. These results demonstrated high reliability and sufficient concurrent and clinical validity of the new scale.
