Results of relaparotomies in 219 wounded with gunshot abdominal wounds got in local wars were analyzed. The main causes of relaparotomies were diagnostical, tactical and technical errors made during primary operations. The final outcome of the treatment depends upon the terms and volume of reoperations. Favourable results were frequent often in performing the programmed sanitation of the abdominal cavity which should be fulfilled in cases of diffuse peritonitis. The elaborated method of early diagnosis of postoperative complications, use of programmed sanitation of the abdominal cavity and restricted volume of reoperations allowed the frequency of relaparotomies to be decreased from 14.8 to 12.1% and lethality after them from 53.2 to 25%.

Considering the experience obtained by the collaborators of the Fuild Surgery Chair (Medical-Military Academy) there was suggested the following conception of using surgical strengthening in the Troops medical-and-evacuation security. By strengthening the battalion medical station with physician, including the surgeon the pre-medical care is rendered; in the case of evacuation delay the emergency measures of first aid are conducted. In the regiment medical station the surgical team renders the first medical aid in whole volume and emergency measures of qualified surgical aid respectively; in the leading medical institution the qualified and specialized emergency surgery as well as specialized medical aid in whole volume are rendered by specialized surgical group.

The article presents experimental grounds and clinical experiences for using a plasma device in 58 patients. Sufficiently high efficiency and safety of the method used during surgical procedures for arrest of bleedings from parenchymatous organs is proved.

Results of experimental application of plasma surgical device UMPR-20 in operations on the stomach, intestine and liver in acute and chronic experiments are described. Coagulating properties of the plasma jet were detected responsible for reliable hemostasis in dissection of the parenchymatous organs. As compared with other surgical devices (electroknife, ultrasonic scalpel, laser "Scalpel-1" device) the zone of injury of the underlying tissues is much less and makes up 0.2-0.3 mm. The plasma surgical device UMPR-20 has good prospects in using during operations on parenchymatous organs and for treatment of their injuries.

The article presents comparative morphological data on the healing of operative wounds on the stomach of rabbits made by an ordinary, laser and ultrasound scalpels. In all the cases the wound was sutured by a one-row interrupted sero-muscular stitches. Clear distinctions in the healing of postoperative gastric wounds made by different scalpels were established. The formation of a zone of coagulative necrosis, followed by rejection of necrotic masses was invariably noted after the laser and ultrasound scalpels. These zones were not so considerable after dissecting the stomach with an ordinary scalpel. No incompetent sutures, failure of their hermetic quality or any other complications in the process of scar formation were detected in all the series.

Clinical observations and experiments in animals enabled the authors to conclude that after operations on hollow organs of the abdominal cavity the healing of wounds made by a usual scalpel developed more favorably than of those made by a laser scalpel.

The results of treatment of 115 patients with vascular injuries were studied within the terms from 3 to 20 years after the operation. In 7 patients the vessels were ligated, in the remainder--repaired. The functional state of the extremity and blood vessels was studied using oscillo- and rheography, angiography, ergometry and other methods. In 81 patients (70.5%) the extremity function was normal, in 24 (20.8%)--moderately restricted, and in 10 (8.7%)--poor.

161 observations of the extremity vein injuries, including 141 cases with associated arterial injuries and 11 cases with isolated traumas, as well as 9 cases of replantation of the extremity in complete tears are analysed. Venous reconstruction was performed in 37 patients, in the remainder veins were ligated. Indications to reconstructive procedures and ligation of injured veins are discussed.
