RESUMO

This paper is concerned with psychic processes within the mind of the analyst; its thesis is that the analyst's internal picture of what his troubled patient can become, or what he will be like when he is well, will affect what the analyst does with the patient and thus the outcome of the treatment. These fantasies on the part of the analyst are probably inevitable in spite of attempts at "even hovering attention." The patient also fantasizes a treatment outcome. The interplay of these conscious or unconscious fantasies and expectations, which may or may not be spoken aloud, will have an important effect on the course of the analysis.

Nineteen patients, each hospitalized with a major depressive episode, were deprived of sleep for one night. Ten patients responded with clear improvement in depressive symptoms; the substantial clinical change was transient, usually lasting one day. Those who responded had significantly higher initial depression ratings (P less than .01) and tended to be older than nonresponders who experienced mild increases in irritability, fatigue, and discomfort following sleep deprivation. Amine metabolites, 5-hydroxyindoleacetic acid (5HIAA), and homovanillic acid (HVA) were not substantially affected by sleep deprivation, although there was a significant interaction of clinical response and direction of 3-methoxy-4-hydroxyphenylglycol (MHPG) change. Sleep deprivation thus produces acute, but only transient improvement in a selected group of severely depressed patients; it appears to be an important tool in the study of the affective disorders.

RESUMO

The authors found a 60% incidence of difficulties in sexual functioning in 57 male patients who had taken thioridazine. Ejaculatory problems were the most frequent dysfunction; a third of these patients experienced retrograde ejaculation. The incidence of sexual difficulties in 64 patients who had taken other major tranquilizers was only 25%. None of the latter group experienced retrograde ejaculation-most had difficulty achieving and maintaining erection. The authors recommend consideration of the possibility of sexual dysfunction in patients given thioridazine.
