Eight patients suffering from hemorrhagic fever with renal syndrome (HFRS) running a severe course complicated by pulmonary edema developed absolute hyperhistaminemia and hyperserotoninemia, histamine and serotonin accumulation in tissues. These amines inactivation in blood and lungs and excretion of catecholamines with urine got disturbed. High blood and lung tissue levels of biologically active substances resultant in enhanced permeability of the vascular wall and alveolar epithelium, hemodynamic disturbances due to hypoexcretory hypercatecholaminemia are thought to underlie the occurrence of this grave HFRS complication.

The study of tick-borne rickettsiosis in Northern Asia has made it possible to determine the clinico-epidemiological features of the modern course of this disease, which should be taken into consideration for the timely diagnosis of this infection. The laboratory verification of the diagnosis should be carried out, as recommended by the authors, in the agar precipitation and leukocytosis tests in their modification.

Clinical, laboratory and original data collected for many years served the basis for proposed guides on intensive care in severe hemorrhagic fever with renal syndrome (HFRS) involving acute cardiovascular and renal insufficiency, shock, rupture of the kidney, eclampsia, pulmonary edema, massive hemorrhage. The conclusion is made on the value of early diagnosis and adequate treatment of emergency states in reduction of HFRS mortality.

[Characteristics of endemic foci of hemorrhagic fever with renal syndrome in various regions of the USSR]. / K kharakteristike éndemicheskikh ochagov gemorragicheskoi likhoradki s pochechnym sindromom v raznykh regionakh SSSR.

Clinical, epizootological, and laboratory data for 9 endemic foci of hemorrhagic fever with renal syndrome (HFRS) were compared. The analysis covered 5120 cases of HFRS. It was established that in the foci, where the leading role is played by field mouse and Japanese field mouse, HFRS is caused by the 1st serotype of HFRS virus, and the clinical course of the disease is quite severe. In the natural foci with the predominance of redbacked vole, the 2nd serotype of the virus circulates and the disease is of moderate severity or mild. In each individual case, however, both serotypes of the virus may produce both mild and severe diseases.

In patients with North Asian tick-borne typhus fever the deficiency of the T-system of immunity, most pronounced in the acute period of the disease, is observed, which is manifested by low values of lymphocyte blast-transformation and rosette-formation. A decrease in the number of active E-rosette-forming cells depends on the severity of the disease. The shifts in the T-system of immunity, appearing in patients with North Asian tick-borne typhus fever, are compensated for comparatively quickly, the normalization of the functional activity of T-lymphocytes occurring sooner than the restoration of their number. The specific dynamics of the characteristics of cell-mediated immunity in patients with North Asian tick-borne typhus fever is accompanied by a comparatively mild course of the disease and its favorable outcome.

[Changes in carbohydrate metabolism indices in patients with hemorrhagic fever associated with renal syndrome]. / Izmeneniia pokazatelei uglevodnogo obmena u bol'nykh gemorragicheskoi likhoradkoi s pochechnym sindromom.

Metabolism of carbohydrates was impaired in 80 patients with hemorrhagic fever accompanied by renal syndrome, in which activation of glycolysis occurred due to a decrease in aerobic oxidation and development of metabolic acidosis. Lactacidemia and activation of LDH in blood serum were considered as criteria of the degree of hypoxia and of the severity of the disease. Estimation of lactate and pyruvate in blood serum and urine might be used for diagnosis and for evaluation of the treatment efficiency.
