[2 years' experience with treatment of uveal melanoma using the Leksell gamma knife]. / Dva roky zkuseností s lécbou uveálního melanomu Leksellovým gama nozem.

In the last years, the therapy of ocular melanoma with Leksell's gamma-knife was indicated at several clinics in Prague. It referred to bulky tumors and to unfavourably localized tumors so that only enucleation of the eyeball was the only alternative in our conditions. The results of the treatment of 11 patients have been evaluated in this article. Some other patients treated with this method have not been included in this sample for various reasons (short-term follow-up, combination of radiotherapy with gamma-knife with other therapeutic modality etc.) Gradual decrease in tumor size developed in 10 patients during follow-up. The decrease in tumor size has been observed on repeated ultrasound examinations and magnetic resonance imaging. The growth of tumor was reappeared in one patient, which led to the necessity of enucleation of the eyeball. The complications in other patients were numerous but not serious to such extent that would lead to enucleation. Marked deterioration in visual acuity connected with the treatment occurred in 6 patients. The results achieved by this method are comparable with those achieved by other therapeutic modalities and with the results from other clinics employing gamma-knife for the treatment of uveal melanoma. The results lead to the conclusion that this is an effective method of treatment of uveal melanoma and that this method is able to go through further development, especially in combination with other therapeutic modalities.

[Implantation of the Iovision silicone lenses]. / Implantace silikonových cocek "Iovision".

The authors report on 80 patients subjected to a cataract operation performed by phacoemulsification with implantation of silicone lenses Nordan-Varifocal manufactured by Iovision Co. In the methodical part the authors describe briefly the operation and implantation of the lens. They discuss also peroperative and postoperative complications, results of visual acuity and astigmatism during the six-month period following operation.

[Therapy of malignant melanoma of the uvea]. / Moznosti terapie maligních melanomu uvey.

The author gives an account of different therapeutic possibilities as regards treatment of malignant melanomas of the uvea. She describes factors which influence the selection of therapy and indications of different methods.

[Prognostically important features of uveal melanoma in histopathologic examinations]. / Prognosticky významné rysy uveálního melanomu v histopatologickém vysetrení.

The prognostic value of various morphological features of the uveal melanoma (i.e. histological type, maximal distance and shape of the tumour, the extension of the tumour, it's site in various parts of the uvea and the mitotic activity and pleomorphism within the tumour) is described. The description and evaluation is based mainly on the histopathologic examination of enucleated globe. The possibilities of the clinical examination or using of specialized methods are mentioned.

[Phacoemulsification. The method of choice in cataract surgery]. / Fakoemulzifikace. Metoda volby v chirurgii katarakty.

Complications and problems in the first 300 patients operated on by phacoemulsification technique are described. In the first 100 cases the procedure began with the creation of the corneoscleral incision and the anterior capsule was opened in a "can opener" fashion. In the second group of 200 patients we preferred the scleral incision, and the anterior capsule was opened mainly by capsulorhexis. The phacoemulsification was predominantly performed in the capsular bag or in the iris plane. The wound was closed with interrupted sutures. The most common and the most serious complication as well in the first 100 cases was posterior capsule rupture (11%). Then the percentage of this most common complication was diminished significantly with increasing experience of the surgeon (2.5% in 200 cases). The same problems which the authors had in their first operations appeared again, though in a smaller amount as soon as they started to work with phacoemulsification as a method of choice of cataract surgery.

[Decentration of posterior chamber intraocular lenses]. / Decentrace zadnekomorových nitroocních cocek.

In a group of 300 patients with a posterior chamber lens in 19% its decentration was found, in 2.3% the iris capture syndrome. The author discusses possible causes of decentration of the implanted lens, possibilities of its therapy.

[Inflammatory intraocular complications after placement of retropupillary lenses]. / Zánetlivé nitroocní komplikace po aplikaci zadnekomorových cocek.

In 194 patients aged 32-81 years 200 retropupillary lenses were administered. 129 lenses were implanted into the sulcus ciliaris, 71 lenses into the lenticular capsule. The surgical technique was the same in both groups. The authors divide inflammatory postoperative complications they encountered into two major groups: cellular reactions on the anterior surface of the lens and fibrinoid reactions with the formation of membranes or synechiae. The authors discuss possible reasons for the development of these complications and possibilities of therapy.
