BACKGROUND: Treatment options for attention deficit hyperactivity disorder (ADHD) in adults consist of psycho-education, cognitive behavioral therapy (CBT), pharmacotherapy or a combination thereof. AIM: To investigate the effect of CBT combined with pharmacotherapy on the quality of life in adults with ADHD compared to medication alone. METHOD: In this multicenter prospective cohort study a total of 627 patients were included, 305 where included in the medication only group and 322 in de combination group (CBT and medication). The Adult ADHD Quality-of-Life scale (AAQoL) was conducted at baseline and at the end of treatment. RESULTS: The quality of life as measured by the AAQoL increased significantly in both groups but was not significantly different between the two groups (p = 0.33). CONCLUSION: To our knowledge, this is the first study to describe the effect of CBT as an addition to ADHD drug therapy on the quality of life in adults. Contrary to our expectations, there was no significant effect of CBT as an addition to drug therapy on the quality of life.

RESUMO

In this paper, we discuss the use of role plays in ethics education for engineering students. After presenting a rough taxonomy of different objectives, we illustrate how role plays can be used to broaden students' perspectives. We do this on the basis of our experiences with a newly developed role play about a Dutch political controversy concerning pig transport. The role play is special in that the discussion is about setting up an institutional framework for responsible action that goes beyond individual action. In that sense, the role play serves a double purpose. It not only aims at teaching students to become aware of the different dimensions in decision making, it also encourages students to think about what such an institutional framework for responsible action might possibly look like.

This second section of the European Crohn's and Colitis Organisation (ECCO) Consensus on the management of Crohn's disease concerns treatment of active disease, maintenance of medically induced remission, and surgery. The first section on definitions and diagnosis includes the aims and methods of the consensus, as well as sections on diagnosis, pathology, and classification of Crohn's disease. The third section on special situations in Crohn's disease includes postoperative recurrence, fistulating disease, paediatrics, pregnancy, psychosomatics, extraintestinal manifestations, and alternative therapy for Crohn's disease.

RESUMO

This paper describes how nursing informatics content will be integrated into the education of Advanced Nursing Practitioners at Masters level. Changes in health care, especially the need to deliver evidence based nursing care and the growing amount of health data, information and knowledge, require well educated Advanced Nurse Practitioners to lead the nursing profession. The nursing informatics content for this level of nursing education is described and compared with national and international curriculum guidelines, including Nightingale work.

RESUMO

Eighty-eight nonstress tests (NSTs) were obtained on gravid women between 27 and 44 weeks' gestation to compare the results of a three-second acoustic stimulus with a traditional NST. Length of testing time was significantly decreased with the use of acoustic stimulation. Consequently, nursing time and equipment use were also considerably lessened per test. Previous studies indicated that the predictive value of the acoustic stimulation test is comparable or better than the traditional NST.

RESUMO

A total of 80 femoral head replacements for recent fractures were reviewed with special attention to local complications. In 50 cases a posterior approach was used by one surgeon, in 30 an anterolateral approach by the other surgeon. As there were no serious local complications in either group, we found no differences between the two approaches.

From retrospective study of the case records of 62 consecutive patients from whom 58 underwent abdominoperineal rectal excision for malignant or inflammatory bowel disease, it was found that management with primary closed perineum and closed pelvic peritoneum with closed vacuum drainage results in 84% primary perineal healing and short hospitalization. Perineal wounds were healed in 93% of patients within one month. Adequate exclusion of three sepsis promoting factors, e.g. dead space, devitalized tissue and micro-organisms should preclude perineal sepsis. The need for precise positioning of a large bore vacuum drain, Ch. 27, through a preoperatively determined stab-wound on the medial side of the thigh is stressed. To guarantee optimal drainage a three-stage procedure is required in a one-team management. The procedure is well tolerated as a palliation; patients with pre-existing massive perineal sepsis should be excluded.

After prolonged fasting the activity of histidine decarboxylase in the oxyntic mucosa of the rat stomach is low. Feeding or injection of gastrin or insulin rapidly raises the enzyme activity. It was earlier suggested that all enzyme-activating agents act through release of gastrin. This view has found experimental support in studies which show that in antrectomized rats the enzyme is activated by gastrin but not by gastrin-releasing stimuli like feeding or vagal excitation (insulin hypoglycemia). In the present investigation rats were subjected to a variety of treatments and serum gastrin concentrations and gastric histidine decarboxylase activities were measured. The main findings were as follows.1. Feeding raised the serum gastrin level and the enzyme activity in unoperated rats. In fasted antrectomized rats the serum gastrin concentration was low; in freely fed antrectomized rats it was at the same level as in fasted unoperated rats. In antrectomized rats the enzyme activity was low and not raised by feeding.2. Acid in the antrum inhibits the release of gastrin whereas an alkaline pH may facilitate such release. All treatments that blocked acid secretion, thereby raising the antral pH, also raised the serum gastrin concentration and concomitantly the histidine decarboxylase activity. Thus, vagotomy increased the serum gastrin level and the histidine decarboxylase activity in fasted rats. Treatment of fasted unoperated rats with atropine or hexamethonium had similar effects. Antral exclusion, which prevents HCl from reaching the pyloric glands, resulted in marked increase in the serum gastrin concentration and in the enzyme activity of fasted rats.3. Injection of insulin resulted in a rather slow, progressive increase in the serum gastrin concentration. The peak was reached after about 4 hr. The enzyme activity was also raised markedly and the peak response occurred about 1 hr later.4. An increase in the histidine decarboxylase activity was invariably preceded or accompanied by a raised serum gastrin level. With fasted or fed unoperated, vagotomized, antrectomized or antrally excluded rats, the correlation coefficient for the relation between enzyme activity and serum gastrin concentration was 0.69 (P < 0.05).5. Porta-caval-shunted fasted rats responded to feeding or injection of insulin with marked activation of gastric histidine decarboxylase. The response after feeding was at least 5 times higher in shunted than in nonshunted rats but serum gastrin was only slightly higher. Following antrectomy of porta-caval-shunted rats feeding no longer raised the enzyme activity. Thus, the enzyme-activating agent was of antral origin. In the shunted rats injection of pentagastrin induced an enzyme activation about 5 times that seen in intact rats. This response was not significantly reduced by antrectomy. In conclusion, we have observed a correlation between serum gastrin concentration and histidine decarboxylase activity. We have failed to obtain evidence for the existence of any physiological intermediate other than gastrin in the activation of histidine decarboxylase induced by feeding, vagal stimulation or inhibition of acid secretion.
