RESUMO

Development of renovascular hypertension was investigated in 68 patients. Interrelated defects found in components of renal pressor-depressor system may be a contributing factor in the disease onset. Contralateral pressor-depressor system is the first to start regulating deranged regional and general hemodynamics in functional failure of the kidneys. In organic lesion occurring in the affected kidney later than in the contralateral one, there is compensatory depressor activation upon the distress of the pressor function. This may be aimed at maintenance of adequate microcirculatory blood flow and metabolism.

RESUMO

The results of long-term (1 to 6 years) follow-up of 43 patients operated on for vasorenal hypertension were compared to preoperative data. The predictive value of the duration of arterial hypertension, patient's age, plasma renin activity coefficient and quantitative intrarenal flow data obtained at angiography with a simultaneous droperidol test, was determined. Intrarenal flow of the contralateral kidney was shown to be the most valuable predictive parameter for the prognosis of the hypotensive effect of the operation. Predictive use of other parameters, such as the duration of arterial hypertension, patient's age, plasma renin activity coefficient yielded higher rates of false-positive and false-negative results.

RESUMO

Advantages of stepwise surgical correction are illustrated by a review of late results of surgical treatment for arterial hypertension due to bilateral vascular lesions or heterolateral combinations of vascular and parenchymatous renal disorders. The procedure yields better results, so that repeated surgery on the contralateral side may be avoided or postponed until the next relapse of hypertension in some patients. The timing of the contralateral operation in the context of stepwise treatment depends on hypotensive effect of the first correction that is conditioned by the state of terminal arterial bed, and the interstitium of both kidneys.
