RESUMO

The purpose of this manuscript was to provide a better understanding of patients with intralenticular foreign bodies (FBs) and also to review the reported cases, including clinical presentation, diagnosis, management, and visual outcome. A 50-year-old male was referred to our clinic with suspected intraocular FB. Under slit-lamp examination, a full-thickness corneal wound with localized corneal edema at the temporal lower peri-limbal area was revealed. Seidel test did not indicate any wound leakage. The corresponding iris was depigmented, but there was no penetrating hole. The anterior chamber was deep with cells, but the lens, vitreous, and fundus were normal. B-scan ultrasonography and orbital computed tomography were performed, but no intraocular FB was detected. On the 2nd day, a zonal cortical cataract and posterior subcapsular cataract formed rapidly. Left-eye bare vision dramatically decreased from 20/100 to counting fingers. One month later, the patient received elective extracapsular cataract extraction. A fine metal thread was completely embedded in the lens; the lens and FB were removed together during the operation. The posterior capsule was not injured; an intraocular lens was implanted in the capsular bag. Two months postoperatively, left-eye vision had returned to 20/25. No adverse events were noted during the follow-up period. In addition to the case report, some 28 previously reported cases of intralenticular FB are reviewed here. Patient demographics, time and course of management, and visual outcome are all summarized and compared.

The alpha angle is a parameter extensively used to assess for cam-type femoroacetabular impingement (FAI) in a 2D image of the hip. As this angle requires estimation of the axis of the femoral neck, the drawing of this axis often results in measurement errors due to subjective judgment, influencing inter-rater and intra-rater agreements. In the present study, sampling points were captured from the edges of a femoral neck and head in the 2D image, and the best curves of the two were fitted respectively by using the curve fitting method. The morphology of the femoral neck was outlined by two polynomials, and the femoral head was represented by an equation of a circle. By means of the proposed method, the results reveal that the inter-rater ICCs in X-ray and MRI were respectively 0.905 and 0.969, and the intra-rater ICCs in X-ray and MRI were respectively 0.892 and 0.840. The Bland-Altman plot shows that the values obtained by the proposed method and the conventional method were not consistent; nevertheless, the linear regression analysis indicated the two measurement results had a significant association (p < 0.001). This study provides a repeatable and agreed α angle measuring method, which contributes to identifying normal and abnormal femoral head-neck morphologies. The proposed numerical method would contribute to diagnose early FAI.

RESUMO

Myeloid sarcoma is known as a tumor mass of myeloblasts or immature myeloid cells occurring in an extramedullary site. When ophthalmic areas are involved, it is usually located in the orbits and noted at or after the diagnosis of an underlying leukemia. We report a 38 year-old woman who had isolated conjunctival myeloid sarcoma without any other clinical signs and symptoms. Acute myeloid leukemia (AML) was diagnosed after a thorough examination. The image studies revealed no orbital or subcutaneous involvement. The patient had complete remission of AML after systemic chemotherapy. We reported this case to emphasize the unusual presentation of a conjunctival nodule of uncertain origin, particularly if it is salmon- pink and grows rapidly. The patient should undergo prompt evaluation for underlying hematological disease even if there are no ocular or systemic symptoms.
