[AB0 blood group-associated biological variability in the content of metabolites].

The performed studies provided new evidence for biological variability in clinically important analytes, estimated a possible association of the metabolic features and the conservative genetic attribute--the ABO blood group. Certain features in the levels of protein, carbohydrate, lipid, and mineral metabolic parameters were revealed in relation to various blood groups.

[Professional selection and successful residency training in anesthesiology-resuscitation].

The professional carrier of 356 residents, the 1960-2007 graduates of the Saint Petersburg Medical Academy of Postgraduate Education, was studied to specify approaches to professional selection of anesthesiologists. Thirty (8.4%) graduates did not work as anesthesiologists at all. It should be noted that most (n = 24) of the 30 graduates had successfully completed the residency. The anesthetic activities were generally altered under the influence of factors that are not related to successful education, namely financial, social, familial, etc. In this connection, long-term forecasting underlying professional selection appears to be of doubtful feasibility. The selection made within the first year of training (a follow-up by teachers) is valid. However, it is impossible to consider each residency graduate to be a well-trained anesthesiologist. An intermediate decision to solve this problem is to establish the new nationally accepted order: a graduate may start his/her professional career only at a university clinic or other large hospital especially licensed for this purpose, by being supervised by senior colleagues. Permission for fully independent practice should be given not earlier than after 2-3-year work at the above mentioned clinics.

[Polioxidonium in comprehensive therapy of recurrent herpetic stomatitis in children with allergic diseases].

In the study with participation of 96 children of 1-7 years age with recurrent herpetic stomatitis and allergic diseases therapeutic efficacy of Polioxidonium was investigated. Polioxidonium was used in combination with traditional methods and with He-Ne-laser irradiation. Local oral cavity immunity, indexes of cellular immunity (in the reaction of lymphocytes blast-transformation with phytohemagglutinin), indexes of humoral immunity (IgG and IgE) were studied, complement C(3)-component was determined. Polioxidonium in combination with traditional methods and with laser therapy for recurrent herpetic stomatitis treatment in children with allergic diseases influenced favourably upon clinical course of allergic diseases, let achieve stable remission, increased efficacy of their treatment, promoted immunological status and shorten the terms of medical rehabilitation.

[Training of anesthesiology-resuscitations physicians in clinical residency: resident's experience].

An educational process in clinical residency is discussed in terms of the results of anonymous questionnaire survey: the respondents were 77 residents in anesthesiology. A significant proportion (28%) of the trainees felt knowledge shortage and even more (47%) were diffirent in their own professional future. There were serious problems regarding practical training organization. Half of the trainees pointed out patients' inaccessibility. In the residents' opinion, the best clinical teacher is a staff anesthetist (42%) or assistant professor (39%). The vast majority (84%) of the trainees believe that one tutor should teach more than two trainees. The ways of solving these or those problems in clinical residency training and the prospects for primary postgraduate education in anesthesiology are discussed.

[Changes of the immune status of children with relapsing herpetic stomatitis, suffering from allergic diseases, during licopide treatment]. / Izmeneniia na fone lecheniia likopidom immunnogo statusa detei s retsidiviruiushchim gerpeticheskim stomatitom, stradaiushchikh allergicheskimi zabolevaniiami.

The efficiency of licopide in combination with traditional treatment was evaluated in 59 children aged 1-14 years suffering from allergic diseases and relapsing herpetic stomatitis. Immunological studies were carried out before and after a course of therapy: local oral immunity, cellular immunity (in the blast transformation test with phytohemagglutinin), lymphocyte populations and subpopulations, humoral immunity values (IgA, IgM, IgG, and IgE), and C3 complement component were evaluated. The results indicate that licopide in complex with traditional methods normalized the immunological values, had a favorable impact on the course of relapsing herpetic stomatitis, and led to a stable remission of allergic diseases (p<0.001).

[Efficacy of nondrug correction of hypertension in general medical practice]. / Effektivnost' nemedikamentoznoi korrektsii arterial'noi gipertenzii v obshchei vrachebnoi praktike.

Effectiveness of nondrug correction of elevated blood pressure was assessed in a group of hypertensives selected from participants of a study of prevalence of cardiovascular risk factors conducted in one of Saint Petersburg districts. Patients were divided into 3 subgroups in which the following interventions were used: diet with decreased sodium content, aerobic physical exercise, or combination of these methods. Blood pressure lowering achieved with these methods (-11 mm Hg systolic and -6.8 mm Hg diastolic) was maintained throughout 2 years of observation.

[Hemodynamic features of anesthesiological maintenance of operations on the lungs in oncological practice when using high epidural anesthesia]. / Gemodinamicheskie osobennosti anesteziologicheskogo obespecheniia operatsii na legkikh v onkologicheskoi praktike pri ispol'zovanii vysokoi épidural'noi anestezii.

The investigation was aimed at studying the effects of epidural anesthesia with local anesthetics alone and in combination with Fentanyl on blood circulation (a comparison of effects of Markain with Naropin) against the background of the infusion of Diprivan by the purposeful concentration during anesthesiological maintenance of operations on the lungs in oncology. The investigation was carried out in 111 patients operated upon for lung cancer. Mildly reduced parameters of systemic hemodynamics when using local anesthetics with Fentanyl were revealed and more pronounced effects on blood circulation when using local anesthetics alone. Hemodynamic effects of Markain were not reliably different from those of Naropin. Thus, the both medicines can be equally used for epidural anesthesia as components of anesthesiological maintenance during operations on the lungs that provides an acceptable level of the antinociceptive protection.

[Effect of biologically active components of natursil on the course of reparative processes in the gastrointestinal mucosa in experiments and in patients with stomach and duodenal ulcers]. / Vliianie biologicheski aktivnykh komponentov natursila na techenie reparativnykh protsessov v slizistoi obolochke zheludochno-kishechnogo trakta v éksperimente i u bol'nykh iazvennoi bolezn'iu zheludka i dvenadtsatiperstnoi.

The outcomes, obtained in experiment, testify about high reparative ability of a natursilum. Natursilum has growth efficiency of conventional treatment of an ulcer of a stomach and duodenal intestine patients both on end results, and on terms of an adhesion increases. Is rotined, that natursilum has more expressed antiulcerous effect, than oil of hippopheae. The transendoscopy spraying of an ulcerative defect has not advantages before application of a drug inside. The specificity of a natursilum structure determines character of effect on exchange processes.

[Use of the new non-depolarizing myorelaxant, Nimbex, for anesthesiologic maintenance in high-risk thoracic operations]. / Primenenie novogo nedepoliarizuiushchego miorelaksanta Nimbeksa pri anesteziologicheskom obespechenii torakal'nykh operatsii vysokogo riska

The article presents main results of the open randomized investigation of the effectiveness and safety of the new non-depolarizing myorelaxant Nimbex included in the composition of anesthesiological maintenance of thoracal operations of high risk. The authors give data of its influence on neuromuscular conduction and main hemodynamic indices during different periods of the operative intervention. The assessment of the intubation condition, depth of the neuromuscular block and of the dynamics of reestablishment of the neuromuscular conduction is shown. A conclusion is made of the possibility to use the myorelaxant in question as a preparation of choice for the maintenance of thoracal operations in high risk patients.

[The use of Oxadol in tablet form for the treatment of chronic pain syndrome in late-stage cancer patients]. / Primenenie tabletirovannogo oksadola dlia lecheniia khronicheskogo bolevogo sindroma u onkologicheskikh bol'nykh pozdnikh stadii zabolevaniia.

Chronic pain management with tablets of oxadol, a non-narcotic central-action analgetic, was studied in 30 patients, focusing on combinations with "basic" drugs of the opiate and opioid group. Combined administration of tramal and oxadol for moderate pain relief raised the analgesic effect and improved certain parameters of quality of life. Possible side-effects and indications in cases of well-advanced tumor were established.

[Clinical and laboratory grounds for using solcoseryl in high-risk patients]. / Kliniko-laboratornoe obosnovanie ispol'zovaniia solkoserila u bol'nykh vysokogo riska.

In 56 patients aged from 58 to 82 years with the III and IV degrees of risk operated upon for malignant tumors of organs of the thoracic and abdominal organs Solcoseryl was included in the complex of anesthesia measures. It was given intravenously or as inhalation in dosage of 1000 mg. It was found that using Solcoseryl considerably reduced the amount of postoperative complications in high risk patients. It led to lower level of lipid peroxidation and activation of the antioxidant system and thus may be considered as an important component of therapy at the intra- and postoperative periods.

[Postoperative oxadol analgesia in oncologic patients]. / Posleoperatsionnoe obezbolivanie oksadolom u onkologicheskikh bol'nykh.

Oxadol (nefopam hydrochloride), a central analgesic, was used for postoperative pain relief in patients operated on the abdominal and pelvic organs for cancer. The analgesic effect of oxadol was sufficient for arresting moderate postoperative pain. Although the drug exerts rather many side effects, none of them notably deteriorated the clinical status of patients.

Study of pharmacokinetics of lidocaine, trimecaine, and clofelin demonstrated rapid resorption of these drugs in the postoperative period. Comparison of two methods of epidural anesthesia in high-risk patients (ASA 3) subjected to urologic surgery showed the objective advantages of adrenergic regional anesthesia, ensuring the stability of vital functions during surgery and allowing operations in awaken patients with adequate spontaneous respiration.

[Clofelin in the anesthesiological support system for surgical operations]. / Klofelin v sisteme anesteziologicheskogo obespecheniia khirurgicheskikh operatsii.

The article presents clinical experience of using Clofelin in 251 patients as a part of general anaesthesia, in postoperative period and in patients with peripheral vascular disease. The experiments on 14 rabbits chinchilla have been done prior to peridural use of Clofelin. The control for adequate use included estimation of hemodynamics parameters, cortizol, vasopressin and B-endorphin levels in plasma on different anaesthesia. The analgetic activity of Clofelin in postoperative period in patients with peripheral vascular disease was estimated by subjective tests using 5 steps of pain estimation scale. The clinical trial demonstrated the efficacy of intravenous and peridural use of Clofelin during surgery and in postoperative period.

Clinical investigations performed in 735 patients have shown that in addition to the improvement of organisation of the medical aid and the universally recognized resuscitation measures for trauma shock, the prophylactics and treatment of intoxication, metabolic, immune and other disorders developing in the patients soon after trauma should be started as early as at the prehospital step.

[Epidural analgesia with clofelin as a component of general anesthesia]. / Epidural'naia analgeziia klofelinom kak komponent obshchei anestezii.

A clinical experience with using peridural analgesia with Clophelin in the composition of general anesthesia in 35 patients operated upon because of surgical, gynecological and urological diseases is presented. Clinical application of Clophelin in anesthesiological maintenance was preceded by an experimental study in rabbits. Control of adequacy of the anesthesia was performed by parameters of central hemodynamics and the level of vasopressin in blood plasma at stages of the research. The adequate level of defense of the patient from the operative stress was determined for using this combined method of general anesthesia.

[Benzofurocaine analgesia in acute myocardial infarct]. / Analgeziia benzofurokainom pri ostrom infarkte miokarda.

A native non-narcotic analgetic agent benzofurcaine was used in 51 patients. It was shown that the analgetic activity of benzofurocaine had a dose-dependent character. Most effective was its use in doses of 2.1-2.7 mg/kg of body weight. It was established that the character of hemodynamic reactions due to benzofurocaine depended on the initial state of circulation. Benzofurocaine did not depress breathing. It is concluded that benzofuricaine is an effective means in moderate pain syndrome due to residual pain as well as in pain of pericardial genesis.
