RESUMO

Nursing faculty are challenged to integrate immunization content in prelicensure nursing curricula. Historically, most immunization content has been delivered in pediatrics courses, with less emphasis on other populations across the lifespan. Skills related to vaccine administration may be prioritized over the most current immunization science, such as pathophysiology, immunology, and epidemiology. As the most trusted profession rated by the public (Saad, 2020), nurses are ideally suited to address vaccine hesitancy and promote vaccination in the communities they serve. Nurses apply active listening, problem solving, and communication skills with patients and their families, contributing to a person's confidence in their decision to be vaccinated. The Centers for Disease Control and Prevention and the Association for Prevention Teaching and Research collaborated to develop a framework for immunization content and teaching resources, Immunization Resources for Undergraduate Nursing (IRUN), for faculty to use in designing the nursing curricula. Content includes a curriculum framework, curriculum mapping tool, multiple teaching resources, and a dedicated website (IRUNursing.org). The framework provides guidance for faculty on integrating immunization content into a curriculum. Teaching resources include case studies, simulation scenarios, and PowerPoint slide decks. Although primarily focused on prelicensure nursing education, resources are also relevant to advanced professional nursing education.

RESUMO

Obesity has a strong impact on the pathogenesis of cardiovascular disease, which raises enthusiasm to understand how excess adiposity causes vascular injury. Adipose tissue is an essential regulator of cardiovascular system through its endocrine and paracrine bioactive products. Obesity induces endothelial dysfunction, which often precedes and leads to the development of cardiovascular diseases. Connecting adipose tissue-endothelial cell interplay to endothelial dysfunction may help us to better understand obesity-induced cardiovascular disease. This Mini Review discussed (1) the general interactions and obesity-induced endothelial dysfunction, (2) potential targets, and (3) the outstanding questions for future research.

Alcohol consumption during pregnancy can produce adverse outcomes; maternal smoking compounds this risk. We examined prevalence of smoking and associations between smoking and alcohol use in Russian women of childbearing age (N = 648). Smoking was reported by 35% of nonpregnant and 14% of pregnant women. Smoking prevalence was higher (45%) among at-risk drinkers and those at risk for an alcohol-exposed pregnancy (AEP). In a multivariate model, smoking status and city of residence significantly predicted AEP risk. Pregnant women in urban locations were more likely to smoke. Smoking and alcohol misuse often co-occur among Russian women, presenting risk for dual prenatal exposure.

Atherosclerosis is a chronic progressive inflammatory process that can eventually lead to cardiovascular disease (CVD). Despite available treatment, the prevalence of atherosclerotic CVD, which has become the leading cause of death worldwide, persists. Identification of new mechanisms of atherogenesis are highly needed in order to develop an effective therapeutic treatment. The blood vessels contain two primary major cell types: endothelial cells (EC) and vascular smooth muscle cells (VSMC). Each of these performs an essential function in sustaining vascular homeostasis. EC-VSMC communication is essential not only to development, but also to the homeostasis of mature blood vessels. Aberrant EC-VSMC interaction could promote atherogenesis. Identification of the mode of EC-VSMC crosstalk that regulates vascular functionality and sustains homeostasis may offer strategic insights for prevention and treatment of atherosclerotic CVD. Here we will review the molecular mechanisms underlying the interplay between EC and VSMC that could contribute to atherosclerosis. We also highlight open questions for future research directions.

PURPOSE: The purpose of this research was to examine the construct validity of scores from the Postpartum Depression Screening Scale administered to mothers of infants in a neonatal intensive care unit. SUBJECTS: Two samples (n = 385 and n = 110) of mothers with infants in a neonatal intensive care unit in the south-central region of the United States completed the Postpartum Depression Screening Scale 2 weeks postpartum. Both samples were similar in race and education level but differed according to marital status. DESIGN: Study 1 was retrospective and descriptive, whereas study 2 was prospective and descriptive. METHODS: In study 1, confirmatory factor analysis was used to evaluate the originally proposed 7-factor structure and a 1-factor model. Because of overly high correlations between 3 of the factors, a revised 5-factor model was also tested. In study 2, confirmatory factor analysis was again used to test the construct validity or goodness of fit of the 7-factor and 5-factor models. MAIN OUTCOME MEASURES: Goodness-of-fit indices and factor pattern coefficients. PRINCIPAL RESULTS: In study 1, high correlations between 3 of the factors in the 7-factor model did not converge. Thus, a 5-factor model was also tested. This model had reasonable fit: χ= 1339.70 (550); P < 0.01; comparative fit index = 0.85; root mean square error of approximation = 0.06; and 90% confidence interval for root mean square error of approximation = 0.058 to 0.067. In study 2, confirmatory factor analysis was again used to test the construct validity or goodness of fit of the 7-factor and 5-factor models. Results for the 7-factor model indicated unacceptable fit: χ (539) = 959.10; P < 0.01; comparative fit index = 0.76; and root mean square error of approximation = 0.09. The 5-factor model was also poor: χ (550) = 992.95; P < 0.01; comparative fit index = 0.75; and root mean square error of approximation = 0.09. CONCLUSIONS: Although the construct validity of the 7-factor model of the PDSS was not supported for this sample of mothers, additional factor analytic work was used to develop and provide initial validation of a 5-factor model. Future research should continue to explore the unique experiences of mothers in the NICU who experience postpartum depression.
