RESUMO

<p><b>AIM</b>To investigate whether estrogen was involved in relaxation of rabbit cavernous smooth muscle.</p><p><b>METHODS</b>Relaxation response of the rabbit cavernous smooth muscles to 17beta-estradiol (0.3, 3, 30 and 300 nmol/L) were observed in vitro. The response of the muscle strips to estrogen after incubation with either actinomycin D (10 micromol/L) or L-NAME (10 micromol/L) were also evaluated. Inside-out mode of patch clamp in a single smooth muscle cell was applied to investigate the Maxi-K channel activities.</p><p><b>RESULTS</b>Estrogen caused a dose-dependent relaxation of the strips precontracted with norepinephrine. The maximal response was noted about 10 minutes after treatment. The estrogen-induced relaxation was prevented by neither actinomycin D nor L-NAME, suggesting that the response was not mediated by gene transcription or nitric oxide (NO). Application of 17beta-estradiol increased the Maxi-K channel activities.</p><p><b>CONCLUSION</b>17beta-estradiol may be involved in relaxation of rabbit cavernous smooth muscles via a non-genomic and NO independent mechanism. 17beta-estradiol stimulates Maxi-K channel of the rabbit cavernous myocyte.</p>

RESUMO

PURPOSE: To evaluate the efficacy and safety of an intracavernous injection of lyophilized papaverine/phentolamine/alprostadil (Standro(R)) for the treatment of erectile dysfunction (ED) in Koreans. MATERIALS AND METHODS: 249 men (>20 years old), with ED (>6 month duration), were enrolled from 14 clinical centers. The intracavernous 'TMs' were titrated in a stepwise fashion at the clinic, from 0.05-0.25ml (17.64mg papaverine, 0.6mg phentolamine, and 6mug alprostadil per ml), with increment of 0.02-0.05ml, according to the etiology and severity of the ED and the patients' ages. RESULTS: Of the 249 men, 238 completed the dose titration, and progressed to home treatment of 3 months duration. Of these 238, 193 (psychogenic 13.0%, organic 75.5%, mixed 11.5%) completed the home treatment (4 or more self-injections), with the other 45 dropping out (lost to follow-up in 24, patient refusal in 9, no chance to have intercourse in 7 and omitted recording of patient diary in 2). The success rate per trial (a total number of sufficient erection for vaginal intromission/a total number of injections) and per patient (number of patients who had one or more sufficient erections for vaginal intromission/the enrolled patients at beginning or 193 patients), and the satisfaction rate per patient (number of patients who had both patient and partner satisfaction with erection/193 patients) were 74.1, or 91.2 and 75.1%, respectively. The adverse reactions were prolonged erections in 3, urethral pain in 1 and penile skin edema in 2. Three patients complained of penile pain during an erection, but there was no dropout due to the pain. No significant changes in laboratory tests were found after the home treatment. CONCLUSIONS: A 'TM' seems to be effective and safe for an intracavernous injection for the treatment of men with erectile dysfunction.

RESUMO

PURPOSE: To assess the erectile response to intracavernous injection of prostaglandin E1 (PGE 1) or papaverine in the impotence patients with diabetes mellitus (DM), and to determine the factors which would predict good response to either drug. MATERIALS AND METHODS: We injected papaverine (10~60 mg) or PGE1 (10~20 mg) into cavernosal tissues of 305 impotence patients with DM (mean age; 56.1 years). Erectile response was classified into poor, moderate, or good, and was comparatively analyzed according to kinds of vasoactive agents, patients` age, duration of DM and impotence, and treatment modality of DM. RESULTS: The frequency of good response to PGE1 was significantly higher than to papaverine (61.5% vs. 42.8%; p<0.05). However, no association between erectile response and patients` age, duration of impotence and DM, and treatment modality of DM was noted. CONCLUSIONS: Prostaglandin E1 seems to be more effective substance than papaverine for intracavernous injection therapy in impotence patients with DM.
