BACKGROUND: Multiple studies have been recorded regarding the perception of abortion among students in the health area. OBJECTIVE: To know if medical-scientific training is sufficient to generate a change in the perception of first-year students of the National Autonomous University of Mexico Faculty of Medicine. MATERIAL AND METHODS: A cross-sectional survey was applied in two times, with the level of relationship between the items of each dimension being analyzed using Cramer's V test. The results were compared to analyze if there was a change in the perception of abortion after the human embryology course. RESULTS: With an initial population of 2,150 students, 393 pre-course and 394 post-course surveys were conducted. The results indicated that despite the existence of changes in the perception of Faculty of Medicine students with regard to abortion, they are not significant. CONCLUSIONS: The medical-scientific training provided by the human embryology course is not enough to provide foundations to the perception of students on issues such as abortion or conscientious objection.

OBJETIVO: Conocer si la formación médico-científica puede generar un cambio en la percepción del alumnado que cursa el primer año en la Facultad de Medicina de la Universidad Nacional Autónoma de México. MATERIAL Y MÉTODOS: Se aplicó una encuesta transversal en dos tiempos y se analizó el nivel de relación de los ítems de cada dimensión mediante coeficiente V de Crámer. Los resultados se compararon para identificar si existió un cambio de percepción acerca del aborto después del curso de embriología humana. RESULTADOS: Con una población inicial de 2150 alumnos, se realizaron 393 encuestas previas al curso y 394 encuestas posteriores. Los resultados indicaron que a pesar de existir cambios en la percepción de los alumnos de la Facultad de Medicina sobre el aborto, no fueron significativos. CONCLUSIONES: La formación médico-científica que proporciona el curso de embriología humana no es suficiente para fundamentar la percepción del alumnado en temas como el aborto o la objeción de conciencia.

RESUMO
Metabolic syndrome is a set of risk factors that consist of abdominal obesity, arterial hypertension, alterations in the lipid profile, and hyperglycemia. The current therapeutic strategy includes polypharmacy, using three or more drugs to control each syndrome component. However, this approach has drawbacks that could lead to therapeutic failure. Multitarget drugs are molecules with the ability to act on different targets simultaneously and are an attractive alternative for treating complex diseases such as metabolic syndrome. Previously, we identified a triamide derivative of 5-aminoanthranilic acid that exhibited hypoglycemic, hypolipemic, and antihypertensive activities simultaneously. In the present study, we report the synthesis and in combo evaluation of new derivatives of anthranilic acid, intending to identify the primary structural factors that improve the activity over metabolic syndrome-related parameters. We found that substitution on position 5, incorporation of 3,4-dimethoxyphenyl substituents, and having a free carboxylic acid group lead to the in vitro inhibition of HMG-CoA reductase, and simultaneously the diminution of the serum levels of glucose, triglycerides, and cholesterol in a diet-induced in vivo model.

Abstract Introduction Suicide has shown an international and national trend to increase, mainly in young people, together with seasonal behavior associated with high temperatures. Although Chihuahua saw the highest number of suicides and suicide attempts in 2016, there are no studies documenting their seasonal and trend behavior. Objective This study sought to analyze the trend and seasonality of completed suicides in the state of Chihuahua from 2008 to 2018. Method The number of deaths from intentionally self-inflicted injuries was obtained from INEGI. The absolute suicide rate was estimated, and a time series model applied to identify its trend and seasonality. In addition, environmental temperature was used as a predictor variable for the number of suicides through a Poisson model. Results A trend was found in the completed suicide rate from 2008 to 2018 in men and women, both separately and together (stationary R2 .73, .66 and .69, respectively), together with seasonality in both sexes (p < .002), with the highest figures being recorded in June and July. An increase of 1,028 suicides was found for every 1°C rise in temperature. In the 10-24 and 25-34 age groups, a linear increase in the suicide rate was observed during the period studied (R² > .7, p = .001). Discussion and conclusion Between 2008 and 2018, the suicide rate increased in the state of Chihuahua, mainly among those aged between 10 and 34. Moreover, suicide rates tend to increase during the months of June and July because of temperature.
Resumen Introducción El suicidio ha presentado una tendencia a la alta a nivel internacional y nacional, principalmente en jóvenes, y un comportamiento estacional, asociado a altas temperaturas. Aunque Chihuahua presentó la mayor cantidad de suicidios e intentos de suicidio durante 2016; no hay estudios que evidencien su comportamiento tendencial y estacional. Objetivo El objetivo fue analizar la tendencia y estacionalidad de suicidios consumados en el estado de Chihuahua de 2008 a 2018. Método Se obtuvieron las defunciones por lesiones autoinfligidas intencionalmente del repositorio del INEGI. Se estimó la tasa de suicidio absoluta y se aplicó un modelo de series de tiempo para identificar su tendencia y estacionalidad; además, se utilizó la temperatura ambiental como variable predictora en el número de suicidios por medio de un modelo Poisson. Resultados Se encontró una tendencia en la tasa de suicidios consumados de 2008 a 2018 en hombres, mujeres y en conjunto (R2 estacionaria .73, .66 y .69, respectivamente); y estacionalidad en ambos sexos (p < .002), donde los mayores registros fueron en junio y julio. Se encontró una asociación en un incremento de 1,028 suicidios por cada incremento de 1°C de la temperatura. En los grupos etarios de 10-24 y 25-34 se observó un incremento lineal en la tasa de suicidio durante el periodo estudiado (R2 > .7, p = .001). Discusión y conclusión De 2008 a 2018, la tasa de suicidios se ha incrementado en el estado de Chihuahua, principalmente entre los 10 y 34 años de edad. Además, la tasa de suicidio tiende a incrementarse durante los meses de junio y julio, asociándose a la temperatura.

OBJECTIVE: to investigate the influence of the application of new methodologies on learning and the motivation of students of the Anatomy discipline. METHOD: randomized, longitudinal, prospective, intervention study. Sixty-two students were recruited to assess the impact of different methodologies. The sample was randomized to compare the results of teaching with a 3D atlas, ultrasound and the traditional method. The parameters were assessed through a satisfaction evaluation questionnaire and anatomical charts. Repeated measures ANOVA was used to determine statistical significance. RESULTS: in terms of the usefulness of the seminars, 98.1% of the students considered them to be very positive or positive, stating that they had stimulated their interest in anatomy. The students who learned with the 3D atlas improved their understanding of anatomy (p=0.040). In general, the students improved their grades by around 20%. CONCLUSION: the traditional method combined with new technologies increases the interest of students in human anatomy and enables them to acquire skills and competencies during the learning process.

Objetivo: investigar a influência da aplicação de novas metodologias na aprendizagem e a motivação de alunos da disciplina de Anatomia. Método: estudo de intervenção, prospectivo, longitudinal e randomizado. Um total de 62 alunos foram recrutados para se avaliar o impacto de diferentes metodologias. Eles foram distribuídos de forma aleatória para comparar os resultados de ensino usando o atlas 3D, o ultrassom e o método tradicional. Os parâmetros foram medidos por meio de um questionário de avaliação de satisfação do aluno e uma prova usando lâminas anatômicas. O teste ANOVA de medidas repetidas foi usado para determinar significância estatística. Resultados: relativo à satisfação dos alunos, 98,1% consideraram muito positivo ou positivo o uso dos seminários, além de afirmarem que o seminário havia estimulado seu interesse pela anatomia. Os alunos submetidos ao método com o atlas 3D apresentaram uma melhora em sua compreensão de anatomia (p=0,040). De forma geral, os alunos melhoraram suas pontuações em aproximadamente 20%. Conclusão: o método tradicional, junto com as novas tecnologias, contribui para aumentar o interesse dos alunos pela Anatomia Humana assim como na aquisição de habilidades e competências em seu processo de aprendizagem.
Objetivo: investigar la influencia de la aplicación de nuevas metodologías en el aprendizaje y en la motivación de los alumnos en la asignatura Anatomia. Método: estudio de intervención, prospectivo, longitudinal y aleatorizado. Fueron convocados 62 alumnos para evaluar el impacto de diferentes metodologías. Se los dividió aleatoriamente para comparar resultados entre enseñanza con atlas 3D, ultrasonido y metodología tradicional. Los parámetros fueron analizados mediante un cuestionario de evaluación de satisfacción y láminas anatómicas. Se usó ANOVA de medidas repetidas para determinar la significancia estadística. Resultados: el 98,1% de los alumnos consideraron muy positiva o positiva la utilización de los seminarios, afirmaron asimismo que el seminario había estimulado su interés por la anatomía. Los alumnos que realizaron el aprendizaje con atlas 3D mejoraron su compresión de la anatomía (p=0,040). En general, los alumnos mejoraron sus puntajes en alrededor del 20%. Conclusión: el método tradicional con el agregado de las nuevas tecnologías permite a los alumnos incrementar su interés por la Anatomía Humana, así como adquirir habilidades y competencias en su proceso de aprendizaje.

Abstract Introduction: The pectoralis major flap is a reconstructive option to consider in the treatment of pharyngocutaneous fistula after a total laryngectomy. There are not large studies assessing variables related to pharyngocutaneous fistula recurrence after removal of the larynx. Our objectives were to review the results obtained with this type of treatment when pharyngocutaneous fistula appears in laryngectomized patients, and to evaluate variables related to the results. Methods: We retrospectively reviewed our results using either a myocutaneous or fasciomuscular pectoralis major flap to repair pharyngocutaneous fistula in 50 patients. Results: There were no cases of flap necrosis. Oral intake after fistula repair with a pectoralis major flap was restored in 94% of cases. Fistula recurrence occurred in 22 cases (44%), and it was associated with a lengthening of the hospital stay. Performing the flap as an emergency procedure was associated with a significantly higher risk of fistula recurrence. Hospital stay was significantly shorter when a salivary tube was placed. Conclusions: The pectoralis major flap is a useful approach to repair pharyngocutaneous fistula. Placing salivary tubes during fistula repair significantly reduces hospital stay and complication severity in case of pharyngocutaneous fistula recurrence.
Resumo Introdução: O retalho do músculo peitoral maior é uma opção a ser considerada no fechamento de fístula faringocutânea pós-laringectomia total. Não há grandes estudos que avaliem as variáveis relacionadas à recorrência da fístula faringocutânea após esse procedimento. Nossos objetivos foram avaliar os resultados obtidos com esse tipo de tratamento em pacientes laringectomizados com fístula faringocutânea e as variáveis relacionadas aos resultados. Método: Revisamos retrospectivamente os nossos resultados em 50 pacientes nos quais um retalho miocutâneo ou fasciomuscular do músculo peitoral maior foram utilizados para reparar a fístula faringocutânea. Resultados: Não houve casos de necrose de retalho. Após o reparo da fístula com um retalho do músculo peitoral maior, a ingestão oral foi restaurada em 94% dos casos. Houve recorrência da fístula em 22 casos (44%), a qual foi associada à duração da hospitalização. O uso do retalho como procedimento de emergência foi associado a um risco significativamente maior de recorrência da fístula. A permanência hospitalar foi significativamente menor quando utilizado um tubo de derivação salivar. Conclusões: O uso do retalho do músculo peitoral maior é uma abordagem útil para reparar a fístula faringocutânea. A colocação de tubos de derivação salivar durante o reparo da fístula reduz significativamente o tempo de hospitalização e a gravidade das complicações em caso de recorrência da fístula faringocutânea.

In Central America, few cases of leprosy have been reported, but the disease may be unrecognized. Diagnosis is based on clinical criteria and histology. Preliminary field work in Nicaragua and Honduras found patients, including many children, with skin lesions clinically suggestive of atypical cutaneous leishmaniasis or indeterminate leprosy. Histology could not distinguish these diseases although acid-fast organisms were visible in a few biopsies. Lesions healed after standard antimicrobial therapy for leprosy. In the present study, patients, family members, and other community members were skin-tested and provided nasal swabs and blood samples. Biopsies were taken from a subgroup of patients with clinical signs of infection. Two laboratories analyzed samples, using local in-house techniques. Mycobacterium leprae, Leishmania spp. and Leishmania infantum were detected using polymerase chain reactions. Mycobacterium leprae DNA was detected in blood samples and nasal swabs, including some cases where leprosy was not clinically suspected. Leishmania spp. were also detected in blood and nasal swabs. Most biopsies contained Leishmania DNA and coinfection of Leishmania spp. with M. leprae occurred in 33% of cases. Mycobacterium leprae DNA was also detected and sequenced from Nicaraguan and Honduran environmental samples. In conclusion, leprosy and leishmaniasis are present in both regions, and leprosy appears to be widespread. The nature of any relationship between these two pathogens and the epidemiology of these infections need to be elucidated.

Introduction Parotid gland incidentalomas (PGIs) are unexpected hypermetabolic foci in the parotid region that can be found when scanning with whole-body positron emission/computed tomography (PET/CT). These deposits are most commonly due to benign lesions such as Warthin tumor. Objective The aim of this study was to determine the prevalence of PGIs identified in PET/CT scans and to assess the role of smoking in their etiology. Methods We retrospectively reviewed all PET/CT scans performed at our center in search of PGIs and identified smoking status and standardized uptake value (SUVmax) in each case. We also analyzed the database of parotidectomies performed in our department in the previous 10 years and focused on the pathologic diagnosis and the presence or absence of smoking in each case. Results Sixteen cases of PGIs were found in 4,250 PET/CT scans, accounting for 0.4% . The average SUVmax was 6.5 (range 2.8 to 16). Cytology was performed in five patients; it was benign in four cases and inconclusive in one case. Thirteen patients had a history of smoking. Of the parotidectomies performed in our center with a diagnosis of Warthin tumor, we identified a history of smoking in 93.8% of those patients. Conclusions The prevalence of PGIs on PET/CT was similar to that reported by other authors. Warthin tumor is frequently diagnosed among PGIs on PET/CT, and it has a strong relationship with smoking. We suggest that a diagnosis other than Warthin tumor should be considered for PGIs in nonsmokers. .
