RESUMO

BACKGROUND AND OBJECTIVE: Between 5% and 10% of amyotrophic lateral sclerosis (ALS) cases have a family history of the disease, 30% of which do not have an identifiable underlying genetic cause after a comprehensive study of the known ALS-related genes. Based on a significantly increased incidence of ALS in a small geographical region from Spain, the aim of this work was to identify novel ALS-related genes in ALS cases with negative genetic testing. METHODS: We detected an increased incidence of both sporadic and, especially, familial ALS cases in a small region from Spain compared with available demographic and epidemiological data. We performed whole genome sequencing in a group of 12 patients with ALS (5 of them familial) from this unique area. We expanded the study to include affected family members and additional cases from a wider surrounding region. RESULTS: We identified a shared missense mutation (c.1586C>T; p.Pro529Leu) in the cyclic AMP regulated phosphoprotein 21 (ARPP21) gene that encodes an RNA-binding protein, in a total of 10 patients with ALS from 7 unrelated families. No mutations were found in other ALS-causing genes. CONCLUSIONS: While previous studies have dismissed a causal role of ARPP21 in ALS, our results strongly support ARPP21 as a novel ALS-causing gene.

RESUMO

The long-term variability of a 22-year dataset of 7Be, 210Pb and gross alpha concentrations in surface aerosols collected in the Canary Islands has been analysed in this study. These "time series" were collected on a weekly basis. Various analytical techniques, including Principal Component Analysis (PCA), K-means clustering, correlation analyses, and back-trajectory were used to determine the variability of the data and assess the statistical importance of the source of the air masses reaching the study area. Monthly and annual variations for the time series were also studied. As expected, 7Be, 210Pb time series showed common variability, while gross alpha concentrations were strongly correlated with average PM10 concentration in air. The fractal properties of the time series were studied to gain a deeper understanding of the underlying structure and dynamics of the data. Multifractal Detrended Fluctuation Analysis (MF-DFA) and Multifractal Detrended Cross-Correlation Analysis (MF-DCCA) techniques were also used to detect and analyse the multifractal characteristics of the 7Be, 210Pb, and gross alpha time series. Multifractality was observed, with values of 0.28, 0.67, and 0.61 for 7Be, 210Pb, and gross alpha, respectively. Long-range correlation was found to be the source of the observed multifractality in the three parameters. Multifractal detrended cross-correlation analysis supports the correlation between 7Be - Alpha, 210Pb - Alpha, and 7Be - 210Pb pairs. The results from this study will help model the transport and destiny of natural radionuclides in the atmosphere at this site. The evolution and interactions between 7Be, 210Pb, and gross alpha, reported herein occurred not just locally but also across extensive temporal domains, leading to the emergence of multifractal behaviour in their concentrations. These long-range behaviours/correlations might result from various factors such as atmospheric circulation patterns, global transport mechanisms, or large-scale environmental processes.

RESUMO

PURPOSE: The aim of this study was to describe our technique for performing femtosecond laser (FSL)-assisted mushroom configuration in deep anterior lamellar keratoplasty (DALK). METHODS: We describe our surgical technique for a mushroom-configuration DALK using a femtosecond laser (FSL) both to prepare the graft and to perform a precut of the recipient cornea, as well as the steps for the dissection of the recipient cornea and for donor cornea implantation. Moreover, we show the parameters of energy and spot separation and the external and internal diameters as well as the thickness of the external and internal keratotomy. RESULTS: We performed a retrospective case series study of 20 patients with a mean follow-up of 4.36 ± 2.54 years. The indication for surgery was leukoma in 15 cases (75%), keratoconus in 4 cases (20%), and stromal corneal dystrophy in 1 case (5%). Four cases had to be converted to penetrating keratoplasty. The overall results were as follows: The mean preoperative corrected distance visual acuity increased from 0.11 ± 0.09 (0.01-0.30) to 0.78 ± 0.22 (0.30-1.0) with spectacles and to 0.92 ± 0.13 (0.5-1.0) with a gas permeable contact lens. The mean final cylinder was 3.90 ± 1.86 (1.25-7.0). The mean endothelial cell count at 6 months was 2033.83 ± 570.53 cells/mm2 (930-3207), and the mean final spherical equivalent was -4.67 ± 2.91 (-0.25 to -9.00). CONCLUSIONS: FSL-assisted technology is useful to achieve a predictable and safe procedure when using mushroom configuration to perform DALK. Our conversion rate from DALK to penetrating keratoplasty was similar to or even lower than that reported in the literature. In the successful DALK cases, the visual and refractive results were similar to those reported in other studies using FSL-assisted DALK (with a standard or mushroom configuration).

RESUMO

The wine industry is a sector of great importance in the Spanish economy, contributing substantial annual revenues. However, one challenge facing the industry is the amount of waste generated, reaching millions of tons annually. These residues consist of organic matter of industrial interest, such as polyphenols. These substances are characterised by their excellent antioxidant properties, making them ideal for use in the food, cosmetic, and pharmaceutical industries. Modern techniques, such as membrane technology, are explored for their extraction based on separating compounds according to size. This work studies a sequential filtration process using ultrafiltration (UF) and nanofiltration (NF) membranes at different operating conditions (2 bar and 9.5 bar for UF and NF, respectively, at 20 °C) to extract polyphenols from wine lees. The results show a total polyphenols rejection rate for each process of 54% for UF and 90% for NF. Pore blocking models have been studied for the UF process and an intermediate pore blocking of the membrane upon wine lees filtration has been identified. A mathematical model that justifies the behavior of a polymeric NF membrane with the filtration of pre-treated vinasse residues has been validated. This study shows a viable process for extracting polyphenols from wine lees with sequential membrane technology.

RESUMO

OBJECTIVE: Breast units led by nurse case managers are being implemented to provide comprehensive care in the detection and treatment of breast cancer. However, their implementation is heterogeneous and the results of the care process with this professional have not been studied. The aim of the study is to describe the management in time and the approach of the process by a nurse case manager in the breast unit of women with suspected breast cancer pathology, derived from the breast cancer screening program. METHODS: Descriptive, cross-sectional, retrospective study carried out in 2021. Women treated in a breast unit managed by a nurse case manager in a hospital in southern Spain were included. Sociodemographic, clinical and care process characteristics were analysed RESULTS: A total of 118 women of Spanish nationality (92%) participated, with a mean age of 59 years. The diagnosis of malignancy was made in 74.6% of them. Seventy-nine percent of the women had their first visit within 3 days. The mean time to diagnosis was 3.98 days (SD: ±3.93), 4.2 weeks (SD: ±1.84) to initiate treatment and a total in-hospital time of 33 days (SD: ±13.45). CONCLUSIONS: The management of nurse case managers in breast units contributes to improving or speeding up times, in accordance with international guidelines, helping this approach in the continuity of the care process for women referred after screening for breast cancer detection.

RESUMO

The present study focuses on the two consecutive and markedly intense Saharan dust intrusion episodes that greatly affected southern Spain (Málaga) and, to a lesser extent, the Canary Islands (Tenerife), in March 2022. These two episodes were the result of atypical meteorological conditions in the region and resulted in record levels of aerosols in the air at the Málaga location. The activity levels of various natural and artificial radionuclides (7Be, 210Pb, 40K, 137Cs, 239Pu, 240Pu, 239+240Pu) and radioactive indicators (gross alpha and gross beta) were impacted by these events and the results are described herein. These episodes caused, for example, the activities of 137Cs in aerosol samples at the Málaga monitoring station to reach the highest concentrations ever recorded since high-volume aerosol monitoring started at this site in 2009. A link between the activity levels of 137Cs, 40K and gross alpha in the atmospheric aerosols and daily PM10 concentrations during the episodes is also reported. In addition, isotopic ratios are discussed in the context of the source and destination of the various anthropogenic radionuclides measured. The atmospheric residence time of aerosols during these episodes is also evaluated because it concerns how intrusions to the Canary Islands should be analysed. Finally, for the first time, the concentrations of 137Cs deposition by rainwater during a Saharan dust intrusion are reported and the deposition rate of these radionuclides during these episodes is discussed.

RESUMO

BACKGROUND: Ex vivo lung perfusion (EVLP) constitutes a tool with great research potential due to its advantages over in vivo and in vitro models. Despite its important contribution to lung reconditioning, this technique has the disadvantage of incurring high costs and can induce pulmonary endothelial injury through perfusion and ventilation. The pulmonary endothelium is made up of endothelial glycocalyx (EG), a coating of proteoglycans (PG) on the luminal surface. PGs are glycoproteins linked to terminal sialic acids (Sia) that can affect homeostasis with responses leading to edema formation. This study evaluated the effect of two ex vivo perfusion solutions on lung function and endothelial injury. METHODS: We divided ten landrace swine into two groups and subjected them to EVLP for 120 min: Group I (n = 5) was perfused with Steen® solution, and Group II (n = 5) was perfused with low-potassium dextran-albumin solution. Ventilatory mechanics, histology, gravimetry, and sialic acid concentrations were evaluated. RESULTS: Both groups showed changes in pulmonary vascular resistance and ventilatory mechanics (p < 0.05, Student's t-test). In addition, the lung injury severity score was better in Group I than in Group II (p < 0.05, Mann-Whitney U); and both groups exhibited a significant increase in Sia concentrations in the perfusate (p < 0.05 t-Student) and Sia immunohistochemical expression. CONCLUSIONS: Sia, as a product of EG disruption during EVLP, was found in all samples obtained in the system; however, the changes in its concentration showed no apparent correlation with lung function.

RESUMO

BACKGROUND: Burnout and engagement in health students surpass work life. Although levels of burnout in Nursing students are high, academic engagement is an understudied topic in Nursing, which has shown benefits. The aims were to know the level of engagement of Nursing students and to identify factors that prompt higher levels of engagement. METHODS: An exploratory study was conducted during 2019 in three Universities in Spain. The UWES-S-17 questionnaire was used. RESULTS: The sample consisted of 808 Nursing students. An average level of engagement was found. Engagement differed significantly by gender and age, with females and older students showing greater attitudes. Participants in the second year showed lower scores in the willingness to dedicate effort and persistence than those in the first and third year (p < 0.05). CONCLUSIONS: The identification of the level of engagement and factors involved are an opportunity to probe into this approach by reinforcing positive attitudes in Nursing students. These findings show the need to seek strategies through specific educational interventions and policies. Engagement should be upheld throughout the degree and once they enter the job market to ensure the well-being during academic and future work life, a high-quality nursing care and patient safety.

RESUMO

PURPOSE: To demonstrate that intense pulsed light therapy (IPL) of the upper and lower eyelids with meibomian gland expression (MGX) is effective in improving dry eye disease due to meibomian gland dysfunction (MGD). METHODS: Patients with ocular discomfort (Ocular Surface Disease Index -OSDI- above 13) and signs of MGD were recruited. All patients underwent OSDI, visual acuity (VA), intraocular pressure, Schirmer test, meibography, non-invasive tear breakup time (NITBUT), slit-lamp examination (corneal and conjunctival staining, hyperemia, gland expressibility, and meibum quality), tear osmolarity and lipid layer thickness. IPL was performed with Optima IPL (Lumenis Ltd.) following a standardized protocol on upper and lower eyelids of both eyes, with inferior eyelid MGX. Patients received four sessions separated by two weeks each. Four weeks after, examinations were repeated. RESULTS: 160 patients (320 eyes) were included, of which 108 (67.5%) were women and mean age was 59.2 ± 15.08 (range 20-89). After four sessions, VA, OSDI, tear osmolarity, lipid layer thickness, NITBUT, hyperemia, corneal and conjunctival staining, gland expressibility, meibum quality, inferior eyelid Meiboscore and Schirmer test improved (all, p < 0.027). Changes in OSDI, initial and average NITBUT increased with dry eye disease severity (according to OSDI). Increased pre-treatment OSDI, hyperemia, corneal and conjunctival staining and Schirmer test were associated with an improvement in OSDI (all, p < 0.040). No adverse events were noted. CONCLUSIONS: The combination of IPL on upper and lower eyelids with MGX is safe and effective for the treatment of MGD. Patients with severe dry eye disease present greater improvements.

RESUMO

INTRODUCTION: The population of older adults is rapidly increasing worldwide, presenting both prospects and complexities for society and healthcare professionals to maximize the functional capacity of this age group. Social isolation and loneliness significantly affect this population. The objective was to determine the effectiveness, satisfaction, and perceptions of the simulation-based education practices of a training program for nursing students, which was created to palliate the social isolation and loneliness of older adults. METHOD: A quasi-experimental study was conducted with nursing students who participated in an online training program using teleservice based on high-fidelity clinical simulation. The program included asynchronous theoretical training and synchronous practical training using an online platform. Five scenarios were designed using simulated phone calls to address the social isolation and loneliness of older adults. RESULTS: Twenty-five nursing students participated in the program, and they had a mean age of 27.44, with 76% of them being women. After the training program, the participants showed statistically significant improvements (p < 0.05) with respect to their knowledge and attitudes towards older adults, and the program was adapted to the best educational practices in simulations. CONCLUSIONS: Simulation-based online training efficiently improved the knowledge and attitudes of nursing students towards older adults, improving their ability to address social isolation and loneliness. The high satisfaction and adhesion to the best educational practices underline the usefulness of high-fidelity online simulations, especially in situations in which face-to-face training is not feasible, and accessibility and equilibrium could be guaranteed between work and personal life.

RESUMO

La Cueva del Viento is a volcanic lava tube located in Tenerife Island (Canary Islands, Spain). Its touristic section, 180 m long, receives more than 28,200 visitants each year. According to the European and Spanish legislation, a radon monitoring program is required to minimize the radon exposition of workers, tourists, and cavers. In this work, we studied the radon concentration dynamics in the touristic section of the cave for ca. 1 year, using both passive and active radon detectors. Pluviometry and external air temperature played an important role in the seasonal and daily variations of indoor radon concentrations. Daily fluctuations during the dry season were analyzed using time series (Box-Jenkins methodology) and frequency analysis (Fourier and Wavelet transforms) methods. The experimental radon time-series was well-fitted using a seasonal autoregressive integrated moving average model: Seasonal Auto-Regressive Integrated Moving Average (2,0,1) (2,1,0)24, and its value, in a short-time window (ca. 1 week) was conveniently forecasted. Finally, this work revealed that the annual effective doses received, during the observation period (1 year), by the touristic guides and visitors was ca. 2 mSv/yr and 4 µSv/hr, respectively. We concluded that the touristic exploitation of La Cueva del Viento is safe for both tourists and guides. However, based on our results, La Cueva del Viento had to be classified as a "Monitoring zone" and a regular monitoring program should be implemented.

RESUMO

Women frequently suffer from urinary incontinence due to atrophic changes in the urogenital tract. Recommended conservative treatment includes evaluation of pelvic-floor strength and the functional use of pelvic-floor-muscle (PFM) training. Following the PRISMA 2020 guidelines, a search was conducted in the electronic databases PubMed, Web of Science, and Scopus for articles with at least one group performing PFM exercises in post-menopausal women with urinary incontinence. Eight articles were included, and each study had at least one group of PFM exercise-based intervention alone or combined. The volume or duration, frequency, and number of sessions were heterogeneous. All the studies reported significant differences in favor of PFM exercise in strength, quality of life, and/or severity of urinary incontinence. PFM exercise is a highly recommended intervention to treat urinary incontinence in postmenopausal women. However, more research is needed to establish specific factors such as dose-response relationships and to standardize methods for measuring effects.

RESUMO

In recent years, smart city projects and initiatives have surged around the globe. Yet, a wide range of factors determine the success or failure of such initiatives and there is still a long road ahead in terms of effective governance and innovation management. In such a context, this study explores the specific case of PCT Cartuja (science and technology park in Seville, Spain)-analyzing several smart-mobility and smart-climate solutions from a Triple Helix Model standpoint. The authors tap into multiple information sources to describe the case and key implications of smart initiatives for both theory and management are discussed. This paper shows the current progress as well as the remaining challenges to illustrate how public-private partnerships and conflict can be effectively managed.

RESUMO

In the present study, nine fabrics have been tested for brackish water treatment with the aim of industrial application under the concept of zero liquid discharge (ZLD). Moisture content was determined, where it was observed that the lignocellulosic fabrics had a moisture content ranging from 2.5 to 8.5%. The wetting contact angle showed that the flax with polylactic acid (LPLA) was the most hydrophobic. The determination of the liquid absorption capacity showed that, of the synthetic fabrics, the one with the highest absorption, both in distilled water and in brackish water, was the polyester (PES) fabric with an absorption of 816% compared to its initial weight. In the natural fabrics, the highest absorption capacity was shown by the wet-laid without treatment (WL-WT) fabric for both distilled water and brackish water, although it required several cycles of operation to maintain this stable absorption. Exposure to brackish water improved the absorption capacity of all samples. Mechanical and thermal characterization showed that the synthetic fabrics were more resistant than the natural fabrics, although they may compete in terms of applicability. The capillarity study showed that the most hydrophilic fabrics completed the test the fastest. Finally, the composting degradation test showed that those fabrics with polylactic acid (PLA) content degraded faster in the first 14 days and thereafter the degradation of the lignocellulosic content showed a slower degradation until 112 days. The Bam fabric did not degrade during the course of the experiment.

RESUMO

PURPOSE: To report our first clinical and patient-reported outcomes in the early postoperative period with a new model of trifocal diffractive intraocular lens (IOL). METHODS: Pilot prospective analysis of the results of 26 eyes of 13 patients (age, 44-79 years) undergoing cataract surgery with implantation of the trifocal diffractive IOL Liberty 677MY (Medicontur Medical Engineering Ltd., Zsámbék, Hungary). The following clinical outcomes were evaluated during the first postoperative month: measurement of distance, intermediate and near visual acuity, binocular defocus curve, and level of spectacle independence, patient satisfaction, perception of photic phenomena, and difficulty in performing several vision-related daily tasks by means of a questionnaire. RESULTS: Mean binocular LogMAR uncorrected distance, intermediate and near visual acuities were -0.03 ± 0.13, 0.21 ± 0.16, and 0.16 ± 0.09, respectively. Furthermore, 100.0%, 84.6% and 92.3% of patients achieved a binocular corrected distance, distance-corrected intermediate and distance-corrected near visual acuity of 20/32 or better, respectively. In the defocus curve, visual acuities were better than 0.2 logMAR for defocus levels between +1.00 and -3.00 D. Spectacle independence was referred by all patients, with any of them reporting dissatisfaction with the outcome of the surgery. The postoperative vision did not lead to difficulties in the daily life in 92.3% of patients. Bothersome halos, glare or starbursts were only reported by only 7.7% of patients. CONCLUSIONS: The trifocal IOL evaluated provides a successful visual rehabilitation with minimal photic phenomena associated, leading to high levels of spectacle independence and patient satisfaction.

RESUMO

Different studies have observed that respiratory muscle training (RMT) improve the endurance and strength of the respiratory muscles, having a positive impact on performance of endurance sports. Nevertheless, it remains to be clarified how to improve the efficiency of such training. The objective of this systematic review was to evaluate the acute physiological responses produced by training the respiratory muscles during exercise with flow resistive devices because such information may support us improve the efficiency of this type of training. A search in the Medline, Science Direct, Web of Science and Scopus databases was conducted, following the PRISMA guidelines. The methodological quality of the articles was assessed using the PEDro scale. Nineteen studies met the inclusion criteria and a total of 212 subjects were included in the studies. The RMT method used in all studies was flow resistive loading, whereas the constant load exercise was the most common type of exercise among the studies. The results obtained seem to indicate that the use of this type of training during exercise reduces the performance, the lactate (La-) values and the ventilation, whereas the end - tidal partial pressure of carbon dioxide (PCO2) is increased.

RESUMO

A radon survey was carried out in 18 high schools located in Tenerife Island when anti-pandemic strategies were used to reduce COVID-19 dissemination during 2021. High schools were located in radon-prone areas previously identified by the Spanish Nuclear Safety Council. Our results showed that 12 high schools presented radon activities lower than 100 Bq/m3, 5 high schools presented values in the range 100-200 Bq/m3, and only 1 high school presented radon activity concentration higher than 200 Bq/m3. Such values are below the reference level (300 Bq/m3) recommended by the Spanish legislation in the Basics Document of Health Standards (section HS6) of the Technical Building Code and the European Union directive (2013/59/EURATOM). Assuming an indoor occupancy time of about 1620 h per year, the annual dose contribution due to indoor radon exposure ranged from 0.07 to 1.18 mSv/year. Comparing such result against previous values reported in the literature on the island of Tenerife, we conclude that during the pandemic situation the indoor radon concentration (median valued) was reduced from 130.9 (2007) to 73.5 (2021) Bq/m3. Finally, continuous indoor radon concentration measurements were obtained to study short-time fluctuations (intra-day changes) under different ventilation conditions.

RESUMO

The winemaking process in Spain generates a significant amount of wastes such as wine lees. Currently, the nanofiltration process is a viable technique for the revalorization of compounds from wastes. In this aspect, this technique can be used for the recovery of compounds, such as polyphenols, as well as active principles widely used in industries, such as pharmaceuticals or cosmetics. Polyphenols are found in acceptable amounts in wine lees wastes and it is interesting to study the nanofiltration process viability to recover them. In order to study this possibility, it is necessary to determine the choice of the best membrane to use and the effect of operational parameters such as pressure, temperature, cross-flow rates, and concentration. In addition, it is important to be able to develop a mathematical model that can help in the future design of lees treatment plants. The treatment of red wine lees to concentrate polyphenols has been studied in a laboratory plant using different membranes (RO and NF) at different pressures (4.5, 9.5, and 14.5 bar), different temperatures (293, 303, and 308 K), and two concentrations (2100 and 1100 mg tyrosol eq·L-1). The results have been encouraging to consider nanofiltration as a viable technique for the treatment and revalorization of this waste. The most suitable membrane has been the NF270, in which 96% rejection rates have been obtained, with a flux of 30 L·h-1·m-2. Moreover, in this study, the Spiegler-Kedem model (SKM) was used to calculate mass transfer constants and permeabilities. Suitable adjustments of these parameters were obtained to validate this mathematical model. For this reason, the SKM might be used in future studies to continue in the research work of the treatment of wine lees wastes.

RESUMO

ABSTRACT: López-Pérez, ME, Romero-Arenas, S, Colomer-Poveda, D, Keller, M, and Márquez, G. Psychophysiological responses during a cycling test to exhaustion while wearing the elevation training mask. J Strength Cond Res 36(5): 1282-1289, 2022-The aim of this study was to investigate the psychophysiological effects of wearing the elevation training mask (ETM). Twelve men performed time-to-exhaustion (TTE) tests at 75% of peak power output with and without wearing the ETM. Heart rate (HR), rating of perceived exertion (RPE), breathing discomfort (BD), and oxygen saturation (SpO2) were measured during the TTE. Prefrontal cortex (PFC) and vastus lateralis oxygenated, deoxygenated, and total hemoglobin were monitored using near-infrared spectroscopy. At the end of each test, blood lactate values (La-) were collected, and subjects completed the Beck Anxiety Inventory (BAI). The mask caused a reduction in the TTE (-37.7%; p < 0.001) and in the SpO2 (-2%; p < 0.001). Beck Anxiety Inventory scores were negatively correlated with the changes observed in the TTE (r = -0.77; p < 0.01). La-, HR, and muscle oxygenation displayed similar results across conditions. In conjunction with an increased hemodynamic response in the PFC, subjects reported higher RPE and BD values in the ETM condition (p < 0.01). Finally, BAI scores were negatively correlated with the changes observed in the TTE (r = -0.77; p < 0.01). This study suggests that wearing the ETM induces psychophysiological alterations affecting the exercise tolerance and limiting the performance.
