BACKGROUND: Air pollution in most countries exceeds the levels recommended by the World Health Organization, causing up to one-third of deaths due to noncommunicable diseases. Fine particulate matter (PM2.5) and black carbon (BC) from mobile sources are the main contaminants. OBJECTIVE: The aim of this study is to assess the relationship of exposure to air pollutants (PM2.5 and BC) in microenvironments according to respiratory health and physical activity in users traveling by different types of transportation in Bogotá, Colombia. METHODS: A mixed methods study based on a convergent parallel design will be performed with workers and students. The sample will include 350 healthy transport users traveling by different urban transportation modes in three main routes in Bogotá. The study is broken down into two components: (1) a descriptive qualitative component focused on assessing the individual perception of air pollution using semistructured interviews; and (2) a cross-sectional study measuring the individual exposure to PM2.5 and BC using portable instruments (DustTrak and microAeth, respectively), pulmonary function by spirometry, and physical activity with accelerometry. The analysis will include concurrent triangulation and logistic regression. RESULTS: The findings will be useful for the conception, design, and decision-making process in the sectors of health and mobility from public, academy, and private perspectives. This study includes personal measurements of PM2.5 and BC during typical trips in the city to assess the exposure to these contaminants in the major roadways in real time. The study further compares the performance of two different lung tests to identify possible short-term respiratory effects. As a limitation, the protocol will include participants from different institutions in the city, which are not necessarily representative of all healthy populations in Bogotá. In this sense, it is not possible to draw causation conclusions. Moreover, a convergent parallel design could be especially problematic concerning integration because such a design often lacks a clear plan for making a connection between the two sets of results, which may not be well connected. Nevertheless, this study adopts a procedure for how to integrate qualitative and quantitative data in the interpretation of the results and a multilevel regression. The time that participants must live in the city will be considered; this will be controlled in the stratified analysis. Another limitation is the wide age range and working status of the participants. Regional pollution levels and episodes (PM2.5) will be handled as confounding variables. The study is currently in the enrollment phase of the participants. Measurements have been made on 300 participants. Pandemic conditions affected the study schedule; however, the results are likely to be obtained by late 2022. CONCLUSIONS: This study investigates the exposure to air pollutants in microenvironments in Bogotá, Colombia. To our knowledge, this is the first mixed methods study focusing on PM2.5, BC, and respiratory health effects in a city over 2 meters above sea level. This study will provide an integration of air pollution exposure variables and respiratory health effects in different microenvironments. It is presented an alternative biological method based on biomineralization mechanisms of Magnetotactic Bacteria (MTB) for the removal in groundwater, of soluble elements such as Fe+2and Mn+2. In first place, it was compared the effectiveness of MTB retention methods for obtention of concentrated volumes in microorganisms, then, it was carried out an inoculation process in groundwater samples and evaluate the removal rate of Fe+2 and Mn+2 in constant conditions of pH and temperature. It was identified electromagnetic method is more efficient in MTB retention, and that the inoculation processes of an enriched solution with MTB in groundwater samples allow to get average removal rates of 47.86% for Fe+2 and 15.26% for Mn+2. In addition, it was evaluated the removal rate of other metals due to magnetic properties of biominerals inside of MTB magnetosome. The highest removal in all cases occurred between the interval of 3 and 5 min of interaction and tended to stabilize in time.

INTRODUCTION: The diagnosis of growth hormone deficiency (GHD) is difficult to determine, and could be associated with severe complications, especially in the neonatal period. The stimulation test of growth hormone (GH) secretion is considered the gold standard for diagnosis, but it has methodological complications and is associated with adverse effects. Neonates present physiological increased secretion of GH, representing a diagnostic window. OBJECTIVE: To evaluate if the dried blood spot on filter paper obtained in the neonatal period, as part of a neonatal screening for con genital hypothyroidism and phenylketonuria, allows differentiating patients with GHD from those who do not have it. PATIENTS AND METHOD: Study of cases and controls by measuring the GH concen tration in dried blood spot on filter paper obtained in the neonatal period, comparing controls with GHD with cases with discarded deficiency. The sample was extracted from the filter paper, obtaining two 0.125 inch discs per each patient from the center of the blood spot on the paper, for a highly sen sitive ELISA assay for human GH based on the use of polyclonal antibodies against 22 kDa recom binant human GH. RESULTS: Seven cases of GHD and ten controls were obtained. The median GH concentration of the dried blood spot in the cases is 2.0 ng/ml (Interquartile range 3.6 ng/ml) and 2.05 ng/ml (Interquartile range 2.0 ng/ml) in the controls, Mann-Whitney U test 30.5 (p = 0.68). The two cases with multiple pituitary-hormone deficiency (MPHD) present concentrations lower than 1 ng/ml. CONCLUSION: The dried blood spot sample did not differentiate GHD patients from control cases, although MPHD cases present much lower concentrations compared to isolated growth hor mone deficiency (IGHD).

INTRODUCCIÓN: El diagnóstico de deficiencia de hormona de crecimiento (DHC) es difícil de establecer, y se puede asociar a serias complicaciones, especialmente en el período neonatal. La prueba de estímulo de secreción de hormona de crecimiento (HC) se considera de elección para el diagnóstico, pero presenta complicaciones metodológicas y se asocia a efectos adversos. Los neonatos presentan aumento de la secreción de HC de forma fisiológica, siendo una ventana diagnóstica. OBJETIVO: Evaluar si la muestra de sangre en papel filtro tomada en el período neonatal, en contexto del tamizaje neonatal de hipotiroidismo congénito y fenilcetonuria, permite diferenciar pacientes con DHC, de los que no la presentan. PACIENTES Y MÉTODO: Estudio de casos y controles mediante determinación de concentración de HC en sangre de papel filtro extraída en período neonatal, comparando controles con DHC con casos con deficiencia descartada. Se realizó extracción de la muestra del papel filtro, obteniendo dos discos de 0,125 pulgada por cada uno de los pacientes desde el centro de la mancha de sangre del papel, para un ELISA de HC humana altamente sensible basado en el uso de anticuerpos policlonales dirigidos contra la HC humana recombinante de 22kDa de peso molecular. RESULTADOS: Se obtuvo un total de 7 casos de DHC y 10 controles. La mediana de concentración de HC de papel filtro en los casos es 2,0 ng/ml (Rango intercuartil 3,6 ng/ml) y controles 2,05 ng/mL (RIC 2,0 ng/ml), U de Mann-Withney 30,5 (p = 0,68). Los dos casos con deficiencia de hormonas hipofisarias múltiples (DHHM) presentan concentraciones menores a 1 ng/ml. CONCLUSIÓN: La muestra de papel filtro no permitió diferenciar a los pacientes con DHC de los casos controles, aunque los casos con DHHM presentaron concentraciones mucho menores, en comparación a la deficiencia de hormona de crecimiento aislada (DHCA).

Inhaled radon and its progenies induce health concern due to high activity-concentration in selected thermal spas of Boyacá region. Hydrogeothermal water sources in a high risk seismic area, are studied to determine by water bubbling method radon concentration values; their occurrence is between few hundreds and 2000 Bq dm-3. Deposits, existing in this area, reach at the surface soil gas radon concentration up to 210 kBq m-3. Maintenance workers, health tourists and visitor's possible detrimental health effects, are discussed in relation to radon balneotherapy beneficial aspects.

The Surface Immunogenic Protein (SIP) of Group B Streptococcus (GBS) had been described as a good target for vaccine development. To date, SIP has been reported as a highly conserved protein, and in a mouse model it induces protection against lethal GBS challenge. Also, similar effects have been described by intranasal immunization with a SIP-based vaccine. In this study, we show the immune response induced by an oral SIP-based vaccine formulated on alum in a mouse model. Our vaccine can reduce vaginal GBS colonization and induce specific SIP-antibodies with opsonophagocytosis activities against GBS. Moreover, we observed the activation of T-cells producing IFN-γ, TNF-α, IL-10, IL-2, and increased expression of the transcription factor T-bet, suggesting a Th1-type humoral response. The oral SIP-based vaccine is a novel alternative in the development of a vaccine against GBS.

RESUMO

Radioactive related pollution due to suspended particulate matter dispersion is an important workplace and health care issue. Recycling oil production ducts and contaminated production equipment, represent a health hazard to workers and public alike. Radioactive plate-out NORM scales with crystal deposit is analyzed by different techniques; results provide proper information on physico-chemical features and emitted alpha particles. Recommendations for handling and recycling procedures are included in relation to health risk and radiological hazard.

RESUMO

Different types the spectrum of photons were studied; they were emitted from the flattening filter of a LINAC Varian 2100 C/D that operates at 15 MV. The simplified geometry of the LINAC head was calculated using the MCNPX code based on the studies of the materials of the flattening filter, namely, SST, W, Pb, Fe, Ta, Al, and Cu. These materials were replaced in the flattening filter to calculate the photon spectra at the output of this device to obtain the spectrum that makes an impact with the patient. The different spectra obtained were analyzed and compared to the emission from the original spectra configuration of the LINAC, which uses material W. In the study, different combinations of materials were considered in order to establish differences between the use of different materials and the original material, with the objective of establishing advantages and disadvantages from a clinical standpoint.

RESUMO

The detection of an electromagnetic counterpart (GRB 170817A) to the gravitational-wave signal (GW170817) from the merger of two neutron stars opens a completely new arena for testing theories of gravity. We show that this measurement allows us to place stringent constraints on general scalar-tensor and vector-tensor theories, while allowing us to place an independent bound on the graviton mass in bimetric theories of gravity. These constraints severely reduce the viable range of cosmological models that have been proposed as alternatives to general relativistic cosmology.

Most patients who require a sibling stem cell transplantation do not have a matched donor. In our experience, only 1/3 patients have a matched unrelated donor (MUD); therefore, the majority of the patients will require umbilical cord blood (UCB). Patients treated for hematologic diseases with UCB transplants were included. UCB selection and conditioning regimens were performed according to the Minnesota group. Graft-versus-host disease (GVHD) prophylaxis, infection prevention, and patient care were performed according to institutional guidelines. We analyzed patients and graft demography, neutrophil and platelet recovery, chimerism kinetics, GVHD incidence, overall (OS), progression-free survival (PFS) and transplant-related mortality (TRM). We included 29 patients with a median age of 34.8 years (range 15-55). Eighteen were male and the median weight was 72.6 kg (range 54-100). Nineteen patients had acute leukemia. Myeloablative (MA) conditioning was used in 27 patients. Seventeen received double UCB (DUCB) grafts. Median total nucleated cell (10(7)/kg) was 4.2 (range 3.9-4.9) and 4.4 (range 2.8-6.3) for single UCB (SUCB) and DUCB transplants, respectively. Median time for neutrophil engraftment was 24.7 (range 14-43) and 25.8 days (range 14-52) after SUCB and DUCB transplants, respectively. Median time for platelet engraftment was 147 (range 30-516) and 81 days (range 37-200) after SUCB and DUCB transplants, respectively. All the patients receiving MA conditioning had >95% chimerism shortly after transplant. Cumulative incidence of grades II-IV and III-IV acute GVHD was 41% and 20%, respectively. Localized chronic GVHD was seen in 14% of the patients. Median follow-up was 16.7 months (range 1-63). Five-year OS and PFS were 38% and 39%, respectively. One-year TRM was 42%. UCB transplantation is associated with potential cure of hematologic malignancies and our results are similar to other series. Studies are needed to decrease mortality and improve immune reconstitution.

BACKGROUND: Glycoprotein VI (GPVI), 60-65 kDa, is a major collagen receptor on platelet membranes involved in adhesive and signaling responses. Mice lacking GPVI have impaired platelet response to collagen and defective primary adhesion and subsequent thrombus formation. Complete or partial deficiency of GPVI in humans is a rare condition presenting as a mild bleeding disorder. The defect in most of the reported patients is acquired and associated with other diseases. To date, only two patients have been characterized at the molecular level who carry different compound heterozygous mutations in the GP6 gene. OBJECTIVE: To report four unrelated patients from non-consanguineous families who presented with mucocutaneous bleeding. They had absent platelet aggregation and (14) C-5-HT secretion with collagen, convulxin and collagen-related peptide. RESULTS: Flow cytometry and immunofluorescence-confocal microscopy showed an absence of GPVI in non-permeabilized platelets. All the patients had an adenine insertion in exon 6 (c.711_712insA), changing the reading frame and generating a premature 'stop codon' in site 242 of the protein. The mutation predicts the synthesis of the truncated protein before the trans-membrane domain, corresponding to a band of ≈49 kDa observed in western blots and in permeabilized platelets by immunofluorescence. Platelet mRNA from all the patients was sequenced and contained the corresponding adenine insertion. Heterozygous relatives had no pathological bleeding, normal response to collagen and convulxin and intermediate membrane expression of GPVI. CONCLUSIONS: The identification of four unrelated homozygous patients with an identical defect suggests that inherited GPVI deficiency is more frequent than previously suspected, at least in Chile.

Objetivo. El objetivo del trabajo fue analizar, a partir de series temporales, las diferencias en la variabilidad de la marcha en dos situaciones distintas (marcha espontánea frente a marcha en tapiz rodante) en sujetos jóvenes, sanos y activos. Método. Las series temporales de diez hombres sanos y físicamente activos fueron evaluadas en dos situaciones experimentales: a) 25 minutos caminando de forma espontánea por un circuito circular abierto; y b) 25 minutos caminando en un tapiz rodante a la misma velocidad que la situación a. Un dispositivo de captura fue diseñado expresamente para la obtención directa y almacenamiento de series de tiempo durante la marcha en ambas situaciones, a partir de un simple pulsador electrónico colocado en la zona del calcáneo de la palmilla del pie dominante introducida en la zapatilla deportiva. Se calcularon la media y desviación estándar (DE), la entropía aproximada (ApEn), la entropía muestral (SampEn) y la entropía multiescala (MSE). Resultados. La DE, la ApEn y la SampEn no presentaron cambios significativos al comparar ambas situaciones experimentales. Todas las MSE disminuyen significativamente al comparar la marcha espontánea con la situación en tapiz. Conclusiones. Existe una pérdida de complejidad de la señal cuando la marcha se realiza en un tapiz rodante y esto debe ser tenido en cuenta para futuras estudios en esta situación(AU)


Objective. The aim of study was to analyze the differences in gait variability (stride interval time series) in two different conditions (spontaneous walking vs. treadmill walking) in healthy and active young subjects. Method. The stride interval time series derived from ten healthy and active males were studied in three experimental conditions: a) walking on level ground at their self-determined usual paces around open circle circuit for 25 minutes; and b) walking on a treadmill for 25 minutes at the same pace as in Situation a. A capture device enabled stride interval time series to be collected directly and stored during walking in both conditions, using a simple electronic push-button mounted in the heel of the dominant foot support of an insole placed in the running shoe. The mean and standard deviation (SD), Approximate Entropy (ApEn), Sample Entropy (SampEn) and Multiscale Entropy (MSE) were calculated. Results. DE, ApEn and SampEn showed no significant changes between both experimental conditions. All MSE significantly decreased from the spontaneous walking to treadmill walking. Conclusions. There was a loss of complexity signal when subjects walked on a treadmill. This should be taken into account for future studies under these conditions(AU)

BACKGROUND: Mitochondrial diseases are a group of disorders caused by mutations in nuclear DNA or mitochondrial DNA, usually involving multiple organ systems. Primary adrenal insufficiency due to mitochondrial disease is extremely infrequent and has been reported in association with mitochondrial DNA deletion syndromes such as Kearns-Sayre syndrome. AIM: To report a 3-year-old boy with Addison disease, congenital glaucoma, chronic pancreatitis, and mitochondrial myopathy due to large mitochondrial DNA deletion. METHOD: Molecular analysis of mitochondrial DNA samples obtained from peripheral blood, oral mucosa, and muscle tissue. RESULTS: A novel large mitochondrial DNA deletion of 7,372bp was identified involving almost all genes on the big arch of mtDNA. CONCLUSIONS: This case reaffirms the association of adrenal insufficiency and mitochondrial DNA deletions and presents new evidence that glaucoma is another manifestation of mitochondrial diseases. Due to the genetic and clinical heterogeneity of mitochondrial disorders, molecular analysis is crucial to confirm diagnosis and to allow accurate genetic counseling.

RESUMO

Face centered cubic metals deform mainly by propagating partial dislocations generating planar fault ribbons. How do metals deform if the size is smaller than the fault ribbons? We studied the elongation of Au and Pt nanorods by in situ electron microscopy and ab initio calculations. Planar fault activation barriers are so low that, for each temperature, a minimal rod size is required to become active for releasing elastic energy. Surface effects dominate deformation energetics; system size and shape determine the preferred fault gliding directions which induce different tensile and compressive behavior.

RESUMO

Recently, Lagos et al. [Nature Nanotech. 4, 149 (2009)] reported the discovery of the smallest possible Ag nanotube with a square cross section. Ab initio density functional theory calculations strongly support that the stability of these hollow structures is structurally intrinsic and not the result of contamination by light atoms. We also report the first experimental observation of the theoretically predicted corrugation of the hollow structure. Quantum conductance calculations predict a unique signature of 3.6 G0 for this new family of nanotubes.

RESUMO

Experts from six Latin American countries met to discuss critical issues and needs in the diagnosis and management of primary immunodeficiency diseases (PIDD). The diagnosis of PIDD is generally made following referral to an immunology centre located in a major city, but many paediatricians and general practitioners are not sufficiently trained to suspect PIDD in the first place. Access to laboratory testing is generally limited, and only some screening tests are typically covered by government health programmes. Specialised diagnostic tests are generally not reimbursed. Access to treatment varies by country reflecting differences in healthcare systems and reimbursement policies. An online PIDD Registry Programme for Latin America has been available since 2009, which will provide information about PIDD epidemiology in the region. Additional collaboration across countries appears feasible in at least two areas: a laboratory network to facilitate the diagnosis of PIDD, and educational programmes to improve PIDD awareness. In total, these collaborations should make it possible to advance the diagnosis and management of PIDD in Latin Americ(AU)

RESUMO

The origin of long interatomic distances in suspended gold atomic chains formed from stretched nanowires remains the object of debate despite the large amount of theoretical and experimental work. Here, we report new atomic resolution electron microscopy observations acquired at room and liquid-nitrogen temperatures and theoretical results from ab initio quantum molecular dynamics on chain formation and stability. These new data are suggestive that the long distances are due to contamination by carbon atoms originating from the decomposition of adsorbed hydrocarbon molecules.

Introducción: Un porcentaje significativo de personas que adquieren una lesión cerebral necesita programas de rehabilitación ambulatoria. En Chile predomina aún un modelo biomédico de rehabilitación, no obstante, las nuevas guías clínicas recomiendan un abordaje interdisciplinario que considere las necesidades del paciente y su familia. Actualmente existe un vacío respecto a cómo operacionalizar intervenciones eficaces que respondan a este paradigma. Objetivo: Encontrar evidencia que permita el desarrollo y evaluación de enfoques en neurorehabilitación ambulatoria de personas con lesión cerebral adquirida y las posibles metodologías de operacionalización de estos. Metodología: Revisión inductiva y conceptual de la literatura. Resultados: En la literatura chilena no existe evidencia empírica ni desarrollos sistemáticos sobre los fundamentos y la efectividad de los programas de rehabilitación ambulatoria para estas personas. En la literatura internacional se destacan tres enfoques: el enfoque holista, el centrado en el cliente y el basado en la planificación de metas. Si bien tales enfoques permiten guiar las acciones de los equipos, la evidencia de su efectividad, en base al criterio de expertos y pequeños estudios longitudinales y de caso único, es aun limitada. Conclusiones: Esencial a la construcción de identidad de un equipo de rehabilitación es determinar los principios y enfoques que sustente su labor. La complejidad y diversidad de los pacientes en la fase ambulatoria requiere que los equipos posean claros fundamentos para guiar la constante toma de decisiones. Es necesaria la evaluación de la eficacia de los programas de rehabilitación ambulatoria en Chile, desarrollando metodologías acordes a la naturaleza multifactorial del fenómeno.


Introduction: A significant percentage of people who acquire a Brain Injury need outpatient rehabilitation programs. In Chile the rehabilitation field is in a transitional stage. Even though the biomedical model is predominant in rehabilitation, the clinical guidelines have begun to recommend an interdisciplinary approach that considers patients needs and their families. Now-a-days, there is a gap on how to operationalize effective interventions responding to this paradigm. Objectives: To find evidence that sustains the development and assessment of outpatient neurorehabilitation approaches of people with brain injury and, to describe the possible operationalization methodologies of these. Methodology: Inductive and conceptual review of literature. Results: In Chilean Literature does not exist empirical evidence nor systematic developments on the foundations and effectiveness of outpatient rehabilitation programs for people with brain injury. International Literature describes three approaches: holistic, client centered, and goal-based. While such approaches can guide the team actions, the evidence of their effectiveness, based on expert criteria and small longitudinal studies and single case studies is still limited. Conclusions: For the development of a rehabilitation team identity, it is essential to determine the principles and approaches that support their work. Due to complexity and diversity of patients in the ambulatory phase, it is crucial that teams have clear foundations to guide the constant decision making. The evaluation of the effectiveness of ambulatory programs in Chile is a necessity; also it is to develop methodologies according to the nature of the phenomenon.

We have studied the changes induced by thermal effects in the structural and transport response of Au nanowires generated by mechanical elongation. We have used time-resolved atomic resolution transmission electron microscopy imaging and quantum conductance measurement using a mechanically controllable break junction. Our results showed remarkable differences in the NW evolution for experiments realized at 150 and 300 K, which modifies drastically the conductance response during elongation. Molecular dynamics and electronic transport calculations were used to consistently correlate the observed structural and conductance behavior. These results emphasize that it is essential to take into account the precise atomic arrangement of nanocontacts generated by mechanical stretching to understand electrical transport properties. Also, our study shows that much care must be taken when comparing results obtained in different experimental conditions, mainly different temperatures.
