RESUMO

Two-stage revision with 'StageOne™ Select Hip Cement Spacer' is used as treatment of periprosthetic hip joint infection. The aim is to evaluate complications and restoration the joint with the StageOne Select Spacer. From 2013 to 2018 twenty-nine StageOne Select Spacers was inserted. Data was obtained through medical records and radiographs. 3.5% dislocated the spacer, 6.9% sustained a femoral fracture, 6.9% got reinfected and 6.9% failed to control the infection. Leg length discrepancy was 1.5 mm (-4,25-5.25) and offset discrepancy was 4 mm (-0.5-9). The spacer shows promising results with a low reinfection and dislocation rate and allowance of restoration of the joint.

Background and purpose - BoneMaster (BM) is an electrochemically deposited hydroxyapatite (HA) implant-coating, which is evenly distributed, thin, and quickly resorbed. It is designed to stimulate osseointegration and early implant stability and alleviate longer-term HA-induced third-body polyethylene wear. This study evaluates early cup migration and functional outcomes of cementless porous-coated hemispherical cups with or without BM.Patients and methods - In a patient-blinded, randomized, controlled trial 53 patients at mean 64 years (55-75) with coxarthritis were operated with an Exceed cup (Zimmer Biomet) and Bi-Metric stem (Zimmer Biomet) with porous and BM coating (PBM) or with porous coating alone (P). Follow-ups were performed postoperatively and at 3, 6, 12, and 24 months. Effect measures were cup migration measured with RSA and PROMs.Results - At 6-month follow-up, proximal cup migration in the PBM group (0.09 mm, 95% CI 0.02-0.20) was higher than in the P group (0.25 mm, CI 0.15-0.35). At 1- and 2-year follow-up, cup migration in all 6 degrees of freedom was similar between groups (p > 0.2). From before surgery to 2-year follow-up, Oxford Hip Score (OHS) increased by 17 points (CI 14-20). Hip disability and Osteoarthritis Outcome Score (HOOS) increased in all sub-scores, but was more pronounced for PBM cups compared with P cups in the Symptoms sub-score (p = 0.04).Interpretation - Contrary to expectations, PBM cups had higher early migration than P cups. At 2-year follow-up, migration was similar between groups. There seems to be no early benefit of BM coating on acetabular cups.

Background and purpose - Long-term fixation of cemented femoral stems relies on several factors including cement adhesion and fatigue. Hi-Fatigue is a newer third-generation bone cement with low-viscosity properties at room temperature, good mechanical strength, and stable bone-cement interface in a laboratory testing environment. Palacos bone cement has excellent 10-year survival and is considered gold standard. We compared stem subsidence after fixation with Hi-Fatigue and Palacos bone cements using radiostereometry. Patients and methods - In a patient-blinded randomized controlled trial, 52 patients (30 women) at mean age 76 years (71-87) with osteoarthrosis and no osteoporosis received Hi-Fatigue G or Palacos R + G cement fixation of collarless, polished, double-tapered stems (CPT). Tantalum beads were inserted in the periprosthetic bone. Supine stereoradiographs were obtained postoperatively, 3 months, 6 months, 1 year, and 2 years after surgery. Oxford Hip Score (OHS) and VAS pain were recorded preoperatively and 1 and 2 years after surgery. Cement working times and properties were registered. Results - At 2 years, mean stem subsidence of 1.12 mm (95% CI 0.96-1.29) for Hi-Fatigue and 1.19 mm (CI 1.03-1.34) for Palacos was similar. Likewise, stem version was comparable between cement groups. Mean OHS and VAS pain were similar between cement groups. Cement working times were similar between cement groups, but the mean curing time was longer for Hi-Fatigue (13.7 min) than for Palacos (11.6 min). Interpretation - We found similar and generally low migration of CPT femoral stems inserted with Hi-Fatigue and Palacos bone cement until 2 years' follow-up, which indicates a good long-term survival of polished taper femoral stems inserted with both cement types.

RESUMO

This case report describes an 18-year-old woman, who had symptomatic bilateral external and internal snapping hip and completed 28 of 30 training sessions during a 12-week resistance training intervention. The intervention proved to be feasible and improved maximal strength in hip abduction and extension measured with Isokinetic Dynamometry. Improvements were also seen in The Copenhagen Hip and Groin Outcome Score, and The Forgotten Joint Score. Furthermore, visual analog scale pain markedly decreased after the intervention.

RESUMO

Osteopetrosis is a rare hereditary bone disease characterized by increased bone density. Patients with adult osteopetrosis are often diagnosed incidentally by X-ray or on the presence of recurrent fractures. Internal fixations of fractures in patients with osteopetrosis are challenging due to the increased bone density, and special surgical equipment is often necessary. Non-union and malunion occur frequently making close post-operative monitoring important. We present a case of adult osteopetrosis with a femoral neck fracture.

Hip dysplasia and femoroacetabular impingement are frequent causes of hip pain and can often be treated with joint preserving surgery to prevent secondary osteoarthritis (OA). Patient education, progressive resistance training and weight loss at a BMI > 27 kg/m(2) has documented effect on pain and function in primary hip OA. If wound infection occurs after total hip arthroplasty the patient should not be treated with peroral antibiotics but instead be referred to an orthopaedic surgeon. There is no documentation that post-operative movement restrictions prevent dislocation of hip after total hip arthroplasty.

Acute traumatic L5-S1 spondylolisthesis is a rare condition, almost exclusively the result of major trauma, frequently associated with L5 transverse process fracture and neurologic deficit. In recent years, open reduction and internal fixation with posterior stabilization has been the method of treatment most frequently reported. In the current case, the lesion was found in a victim of an automobile accident. Signs of a right L5 root deficit but no sphincter dysfunction were present. A computed tomography scan revealed several fractures in the posterior parts of L5 and anterior displacement of L5 on S1. A magnetic resonance imaging (MRI) scan verified that the lesion was indeed acute by showing the ruptured L5 disc and posterior ligaments, thereby demonstrating the importance of MRI in the planning of the treatment of these lesions. This case was successfully treated with an acute circumferential instrumented L4-S1 spondylodesis.
