RESUMO

A 2.9-year follow up of 50 primary Bi-Metric System cementless, porous-coated hip arthroplasties (Biomet) is presented. Four patients were operated bilaterally. In two patients, revision arthroplasty was carried out, in one because of infection, in the other because of loosening of the stem. The average Harris hip score was 96 points. The pain level according to d'Aubigné was 5.87. All patients belonged to Jensen's social function group 1. According to Engh's x-ray score, the femoral component achieved an average total of 19 points. Twenty-three patients with heterotopic ossification were graded according to Brooker et al; 18 patients belonged to group I-II, and 5 patients to group III. Discrepancy of leg length was measured in 20 patients, and averaged 0.52 cm (range: 0 cm to 2.5 cm). Four patients complained of thigh pain, but they had no radiographic evidence of loosening of components. The mean age was 63.4 years for women and 58.1 for men. Acceptably high Harris Hip Scores were obtained for the Bi-Metric hip prosthesis at 2-year follow up.

RESUMO

The relationship between methods of treatment, quality of reduction and the occurrence of late disability and osteo-arthritic changes was studied in 17 Rolando's fractures. Three were undisplaced, 14 were displaced: of these excellent reduction was achieved in five of 11 treated by open reduction and in none of the three treated by percutaneous K wire fixation. After a median interval of 5.8 years, six of 16 reviewed patients had minor symptoms, while nine had no symptoms. Radiographic examination of 11 patients showed signs of osteoarthritis in six. We did not establish a relationship between the quality of reduction and the occurrence of late symptoms and osteoarthritis.

In a case-referent study on the possible role of selenium in human mammary carcinogenesis, serum selenium was found to be 79 +/- 12 micrograms/l in 66 cases and 81 +/- 12 micrograms/l in 93 referents. An internal trend in serum selenium was observed among cases (TNM stage I 81 +/- 11 micrograms/l and TNM stage II 76 +/- 13 micrograms selenium/l), indicating disease-mediated changes. The evaluation of selenium as a risk indicator in human breast cancer was therefore restricted to TNM stage I patients (n = 36). Multiple logistic regression analyses including variables associated with selenium levels revealed no association between selenium levels and breast cancer risk.

The relationship between the quality of reduction and the occurrence of arthritic changes, symptoms and disability was studied in 41 Bennett's fractures. Excellent position was obtained in five of nine fractures treated by closed reduction and plaster immobilisation, in four of six fractures treated by percutaneous K wire fixation and in 18 of 26 fractures treated by open reduction. After a median interval of 7.3 years, 15 of 18 reviewed patients with fractures healed in excellent position were free of symptoms, but this was so in only six of 13 fractures with residual displacement. The remainder had intermittent slight pain. Radiographic signs of arthritis was found in ten of 24 patients: three of 14 patients with excellent reduction and seven of ten patients with residual displacement.

In order to determine the relationship between delay before immobilization and the development of non-union or delayed union, 285 fractures of the scaphoid have been analysed from records and radiographs. 16 non-unions and 25 of 27 fractures with delayed union were found in proximal pole or waist fractures in adults, which were analysed separately. No increment in the time to bony union or in the incidence of non-union was found following a delay of immobilization of less than four weeks, but when the delay exceeded four weeks, most fractures had healing complications.

During a ten-year period scaphoid fracture was suggested in 541 wrists, but in 283 cases could not be confirmed at the initial radiographic examination. These cases were immobilized in plaster of Paris and examined clinically and radiologically 10 to 14 days later. A total of 274 additional radiographic examinations revealed 32 scaphoid fractures not initially recognized. Two fractures were at the retrospective analysis found to have been neglected. In the 249 cases where a fracture was never demonstrated, the immobilization and resulting sick leave were in retrospect considered not to have been indicated. In a retrospective analysis of case records and radiographic material including the configuration of the scaphoid fat stripe (SFS) at the initial radiographic examination the SFS was found to be useful in diagnosing scaphoid fractures. The use of the SFS in differential diagnosis would result in reduction of costs and inconvenience for patients without scaphoid fracture.
