RESUMO

The steady growth of antibiotic resistance of bacteria and, as a result, difficulties in selecting effective drugs determine the search for an alternative strategy for the use of antimicrobials, and therapy based on the use of bacteriophages is such. Phage therapy in otorhinolaryngology is actively used for the treatment of rhinosinusitis, diseases of the pharynx and larynx. However, it should be noted that the use of this group of drugs in the treatment of ear diseases is not sufficiently covered. Currently, institutes and clinics around the world are undergoing many large multicenter studies aimed not only at finding effective bacteriophages against antibiotic-resistant bacteria, but also at isolating their enzymes (lysines), expression of transport proteins, genetic modifications that lead to the restoration of the sensitivity of bacteria to antimicrobial drugs. In this review, we sought to highlight modern works concerning the treatment of inflammatory ear diseases with bacteriophage preparations, the results of microbiological studies conducted in vitro, the main purpose of which was to combat multi-resistant strains of microorganisms - pathogens of purulent-inflammatory diseases of the outer and middle ear.

RESUMO

He article describes an observational (non-intervening) research 'Analysis of the therapeutic efficacy of the Polyvalent Piobacteriophage (secstafag) in the treatment of acute sinusitis'. The obtained results prove the effectiveness of the drug in the treatment of this ENT pathology.

RESUMO

Bacterial infections of the upper respiratory tract in the structure of the incidence of the population ranked second. Recently in practice there are more and more cases of a protracted course of the disease, chronic inflammation, as well as various kinds of complications. The abuse and misuse of antibiotics has contributed to the growth of antibiotic-resistant bacterial strains. This problem is named one of the most pressing for public health in the world. In this regard, the topical issue is the search for alternative drugs foe treating infectious diseases. Bacteriophage preparations that have a 100-year history and have gone a long way in development have shown their effectiveness as an alternative to antibiotics, and as an additional therapy in the treatment of certain bacterial infections. This review includes data from various studies available in English, as well as the results of domestic reports published in Russian scientific literature.

The objective of the present study was the analysis of the medical assistance seeking behaviour of the patients presenting with suppurative chronic otitis media (CSOM) and their treatment in Moscow during the period from 2009 till 2016. The incidence of suppurative chronic otitis media in the city of Moscow in 2016 was estimated to be 0.14 per 1000 population, its prevalence 1.23 per 1000 population. The materials of the statistical reports of Moscow ENT clinics give evidence of the considerable growth in the number of the admitted patients suffering from ear diseases including suppurative chronic otitis media (mean 5.5). The surgical activity of the ENT clinics with respect to CSOM amounted to 46%. The overall increase in the number of surgical interventions is attributable to a rise in the frequency of reconstructive operations. These data give evidence of excellent continuity in the work of outpatient facilities and ENT clinics, intensification of their activities, and a change of the people's attitude toward the care for their health. Suppurative chronic otitis media is the commonest pathology encountered in the specialized ENT hospitals where it accounts for 59.8% of the total number of ENT diseases. The frequency of the surgical interventions performed for the treatment of CSOM amounts to 98.9%. The enhanced attention given to the patients with this pathology resulted in the two-fold increase of the specialized medical assistance provided to the patients presenting with suppurative chronic otitis media. The most frequently practiced form of the surgical treatment employed for the management of CSOM is tympanoplasty (in 72% of the patients on the average). The 'closed' variants of sanitizing operations are performed in 59.8% of the patients presenting with chronic suppurative otitis media and concomitant cholesteatoma whereas the 'open' interventions are used to treat 45.1% of the patients in need of tympano- and mastoidoplasty. On the whole, the priority surgical treatment of the patients with chronic suppurative otitis media in specialized medical facilities is reconstructive operations intended to simultaneously reach the beneficial sanative and functional effects without deterioration of the quality of life of the patients suffering from chronic suppurative otitis media.

RESUMO

This article was designed to report a clinical case of an acute acoustic injury inflicted by a discharge of the lightning ball during the early postoperative period in a female patient following surgical stapedoplasty. The acoustic impact resulted in a marked deterioration of hearing in the operated ear with simultaneous elevation of the threshold of bone sound conduction to above the preoperative value. It is concluded that the patients who had undergone the stapedoplastic surgical intervention should be advised to avoid strong acoustic impacts during the postoperative period.

The objective of the present work was to study syntopy of the distal end of the nasolacrimal duct based on the results of the vector analysis of the CT-identified anatomical features of the nasal cavity and the paranasal sinuses in 102 patients (204 nasolacrimal channels). The main criterion for inclusion in the study was the absence of traumatic injuries and inflammatory changes in the nasal cavity and the paranasal sinuses. X-ray studies of the distal end of the nasolacrimal duct (NLD), turbinate crest (crista conchalis), the edge of the nasal aperture, and the floor of the nasal cavity were carried out. The vector and angle measurements were made with respect to the bottom of the nasal cavity in the perpendicular and parallel planes. The study has demonstrated that the distal end of the nasolacrimal duct was located 3.49 mm above the level of crista conchalis of the maxillary bone 19.77 mm and 22.04 mm apart from the edge of the aperture of the nose in the individuals younger than 20 years and in the older subjects respectively. Syntopy of the distal end of NLD relative to the bottom of the nasal cavity did not depend on the sex and age of the patients and was localized at a depth of 13.93 mm from the edge of the aperture of the bone and 11.86 mm above the bottom of the nose.
