INTRODUCTION: For many years the specialists have been interested in gastroesophageal reflux. The aim of the work is to present our experience in the diagnosis and the treatment of laryngopharyngeal reflux. MATERIAL AND METHODS: We studied 60 patients (35 women and 25 men) aged 25 to 60 (average age - 43), with symptoms of reflux, hospitalized in ENT Department in the years 2010-2011. In all patients in ENT examination laryngitis were stated. Each patient was treated with 24-hour esophageal pH-impedance during gastrological consultation. In case of confirmed reflux disease proper diet and medication blocking the proton pomp were prescribed for a period of 3 months. The check-up included: patient interview, filling the questionnaire, ENT examination and gastrological consultation. The treatment was continued for the next 3 months with patients with sustaining problems of upper airway. RESULTS: Laryngopharyngeal reflux disease was confirmed in all examined patients. After 3 months over 80% patients finished the treatment, the other (13) patients finished their treatment after 6 months. CONCLUSIONS: ENT and gastrological consultations with 24-hour esophageal pH-impedance help with successful diagnosis and curing the reflux disease.

INTRODUCTION: Tumors of parotid gland represent a diverse group of neoplasms with varied clinical behaviors. MATERIAL AND METHODS: The diagnostic methods and treatment of salivary gland tumors are presented in this paper. The authors describe the group of 44 patients with salivary gland tumors treated surgically in ENT Departament Laryngology of Medical University in Lódz. Enucleation, partial and total parotidectomy were performed. Neurosign 100 unit was used during parotidectomy to monitoring course of facial nerve. RESULTS: Tumor mixtus was a dominate type of tumors. Only three patients with malignant tumor had facial nerve paralysis after surgery. CONCLUSIONS: (1) The extent of the surgical procedure depends on histopathologic diagnosis. (2) Using of facial nerve monitoring during parotid surgery improves its outcomes.

RESUMO

Ameloblastoma (adamantinoma) is a benign neoplasm deriving from the enamel organ. Its etiology has not been ultimately determined. It constitutes about 1% of all head and neck tumours, and about 11% of teeth-originating tumours. Usually it occurs in the mandible near premolar and molar teeth, more rarely in its anterior part. About 20% of described cases of ameloblastoma relate to the jaw, its very rare location is gingiva or cheak tunica mucosa. The essay presents a case of ameloblastoma of a maxillary sinus in a 65-year-old man. The diagnostic and treatment algorithm in ameloblastoma is discussed.

UNLABELLED: Wegener's granulomatosis there is a small--and middle--vessels vasculitis. The pathomorphological diagnostic criteria is known as Wegener's triad: 1) necrotizing granlomatous inflammation of upper and/or lower respiratory tract, 2) systemic or focal necrotizing vasculitis involving arteries and vein, and 3) focal segmental necrotizing crescentic gromerulonephritis. According to the current theory of pathogenesis of Wegener's granulomatosis, Staphylococcus aureus is involved. The rise in ANCA level during vascular inflammation is very important for monitoring disease. The main localization of Wagener's granulomatosis is in lung, nasal sinusites, nose, kidneys and nasal pharynx. MATERIAL AND METHODS: The history of 60-years old female patient who was being treated in Department of Otolaryngology for chronic bilateral otitis media for many years is presented. During hospitalization the sinusites, rhinitis and renal failure additionally were found. The CT sinuses and head, histopatological and ANCA exam in patient were performed. RESULTS: Based on clinical, physical, additional exam Wegener's granulomatosis was diagnosed. CONCLUSIONS: 1) Surgical procedure is not indicated method of treatment of Wegener's granulomatosis, because it can increase the pathologic process. 2) The prognosis of Wegener's granulomatosis depends mainly on the ability to diagnose the early disease and the aplication of adequate treatment.

OBJECTIVE: Sleep apnea is one of the most deleterious disorders in children with adenotonsillar hypertrophy because it can induce hypoxemia of brain. Sleep apnea may lead to failure to thrive or to physical and mental delay in development, including cognitive disturbances. The aim of this study was to analyze the influence of adenotonsillar hypertrophy, causing obstructive sleep apnea on neurocognitive abnormalities. We were interested in sensorimotor coordination, perception, memory, learning ability, concentration, focused attention and language reception. MATERIALS AND METHODS: We examined 221 children. One-hundred and seventeen children had sleep apnea caused by adenotonsillar hypertrophy: 87 children aged 6-9 years and 34 children aged 10-13 years. The control group, without adenotonsillar hypertrophy, consisted of 104 healthy children. Both groups of children with and without apnea were examined psychologically to determine abilities and minor neurocognitive deficit. The token test (TT), diagnosis test of brain dysfunction (DCS-test), Luria auditory verbal learning test (LAVLT) and Rey complex figure test (RCFT) were applied to both groups. The tests: TT, DCS-test, RCFT were used to investigate the level of sensorimotor integration and perception processes. Memory and learning abilities were measured using LAVLT. The TT assessed language dysfunction in children. RESULTS: This study shows that adenotonsillar hypertrophy in children aged 6-9 years is associated with neurocognitive abnormalities such as: memory problems, concentration of attention deficits, learning disability, language dysfunction, lower sensorimotor integration and perception. The older children (aged 10-13 years) with adenotonsillar hypertrophy had memory problems and learning disabilities. They are likely to be caused by of concentration of attention deficits. The older children were found to have more severe language dysfunction. CONCLUSIONS: Our study shows that sleep apnea may lead to neurocognitive deficits. The results may be helpful in the process of making decision for or against surgery in doubtful cases. It is important to know the problems connected with apnea in order to recognize them and help the child develop by providing adequate treatment and cognitive stimulation.

RESUMO

The most common sleep disorder in children is obstructive sleep apnea syndrome (OSAS). The majority of children with OSAS improve following tonsillectomy and adenoidectomy. Diagnosis of OSAS in children and adults has difference in criteria if disease and clinical picture.

RESUMO

The authors present results of retrospective clinical analysis of usefulness the cefuroxime therapy of acute ENT diseases in children. The study group consist of 886 patients, aged 4 m. to 17 year, hospitalized at the Department of Paediatric Otolaryngology between 1997-2002. The efficacy of therapy was estimated on the ground of 4 degree scale. Particular attention was paid on measuring an average time of intravenous and oral administration of drug and on side effects of treatment. The results of the study shown that cefuroxime therapy is safe and effective. Beneficial therapeutic effect was obtained in 98.9% of patients.
