RESUMO

A new antiglaucoma operation has been developed: limbosclerectomy with valvular drainage of the supraciliary space. The operation was performed on 82 eyes with artiphakia, pseudophakia, and previously operated primary open-angle glaucoma. The patients were observed for 1.3 +/- 0.6 years after the intervention. Compensation of intraocular pressure and a significant increase in the coefficient of liquid discharge facility were observed in remote periods in all patients: in 95.1% patients without hypotensive therapy and in 4.9% cases instillations of 0.5% thimolol maleate twice a day were needed. Visual acuity and visual field borders did not appreciably change after the intervention. The operation is recommended for practice.

RESUMO

A clinically significant relationship between the myopic eye astigmatism and manifestation of peripheral chorioretinal dystrophies in them in young patients is described. A simple and effective method for predicting peripheral vascular chorioretinal dystrophies in myopic patients is proposed, based on the presence and severity of astigmatism. The incidence of various dystrophies in spherical and aspherical myopia is 32 and 78%, respectively. Dystrophies fraught with retinal detachment occur three times more often in astigmatic eyes, which are characterized by a more flat lens.

RESUMO

The clinical picture and cytostatic treatment of solitary subconjunctival cysts of filtration pad after fistula-forming antiglaucoma surgery are described. Twenty-seven eyes with hypertension which developed as a result of subconjunctival cysts formed in the early postoperative period in 7% of patients operated on for open-angle glaucoma. Probable mechanisms of the pathogenesis of the formation of solitary cysts are discussed and 3-staged therapy proposed, including beta-blockers, perforation of the cyst wall by YAG laser, and, in 10% of cases with rigid cysts, microsurgical revision of filtration pad.

RESUMO

A service program for selecting magnifying means has been developed and tried in 59 patients (110 eyes) with senile maculodystrophy and cataracts. Attempts at choosing traditional magnifiers (magnifying glasses, hyperoculars, telescopic eyeglasses, and contact lens-eyeglasses system) were made with due consideration for the focus of lesions and the anatomooptic parameters of the eye (corneal refraction, size of the anteroposterior axis, and clinical refraction). The results indicate that the proposed program helped effectively choose traditional magnifiers for the majority (81.35%) of the above patient population.

RESUMO

The authors present the fundamentals of making outpatient computer case histories for prophylactic check-ups of chronic outpatients with ocular diseases. They validate the necessity of creating mononosological versions of computer histories which should not only describe the underlying diseases in detail, but reflect any concomitant ocular disease as well. The basic criteria formulated by the authors helped create a computer history of a modulus type, which may be easily mastered by physicians within just 2-3 hours and be adapted to research of this or that patient population. The authors emphasize the importance of creating and wide use of software in everyday practical medicine.

The author summarizes the results of many-year profound statistical analysis of ophthalmic tone in myopic patients. Intraocular pressure (IOP) of myopic eyes in patients aged 10 to 62 years and refraction from 0.5 to -24.0 diopters was measured in 1220 eyes by the same type of Goldman's tonometer. Detailed distribution of IOP for age and refraction factor is presented. IOP has been studied in emmetropia, stable and progressive myopia, and in paired anisometropic eyes. The author emphasizes that myopia progresses at various levels of ophthalmic tone, whose increase is mainly functional and reversible, and is caused by organic changes in the anterior chamber angle in only 7% of cases. The author claims that IOP is an important characteristic of a myopic eye and is to be taken into consideration when prescribing pathogenetic therapy for progressive myopia.

RESUMO

A modified scleroplastic operation is suggested, trophic collagenoplasty in which sclera-fortifying collagenoplasty is combined with subtenon implantation of a collagen infusion system. Using this system drugs stimulating trophic processes in the tissues of the posterior segment of the eyeball are infused within 7 to 9 days after surgery. Such operations were carried out in 42 eyes of 22 patients with high progressive myopia (8.0 to 27.0 diopters). The patients' ages varied from 30 to 57. Three months after surgery a manifest improvement of hydro- and hemodynamic parameters of the eyes, as well as of visual acuity with correction, of visual field, and of retinal light discriminating sensitivity were observed in the majority of patients (p < 0.05). The results of pre- and postoperative examinations demonstrate a high efficacy of such operations in patients with high progressive myopia combined with marked dystrophic changes in the tissues of the posterior segment of the eye.

Comprehensive ophthalmologic examinations of the anterior chamber angle were carried out in 130 patients (259 eyes) aged 16 to 39 (mean age 22.9 +/- 0.55 years) with myopia of 0.5 to 13.6 diopters. Study of the pattern of gonioscopic signs distribution has shown that the most characteristic factor related to myopia development and progress is the presence of goniodysgenesis signs, namely, expressed development of ligamentum pectinatum iridis, marked anterior position of Schlemm's canal combined with a wide anterior chamber corner, and pigmentation of anterior chamber corner. Analysis of gonioscopic signs, their mutual influences and relationships with other anatomicofunctional parameters will help assess the role of these signs in the development of complicated myopia, prediction of myopia course, and differential diagnosis of its pathogenetic forms.

Ophthalmic tone was examined by Goldman's tonometry in 186 subjects (372 eyes) aged 13-35 with myopic and emmetropic refraction, as was the status of relative accommodation reserve. The findings evidence that in progressive myopia the mean level of intraocular pressure is by 3.0 mm Hg higher than ophthalmic tone in the eyes with emmetropic and stationary myopia; the relative accommodation reserve index was found decreased in 80-87 percent of patients with slowly and gradually progressing myopia, whereas in those with emmetropic and stationary myopia it was normal. This lead the authors to a conclusion that relatively elevated intraocular pressure (greater than or equal to 16 mm Hg, measured according to Goldman) and poor accommodation are factors indicating the risk of myopia progress and essentially influencing the refraction enhancement at this age.

To stabilize weak and medium-severity progressive myopia, instillations of cycloplegics (homatropin or tropicamid) and of hypotensive agents (clofelin or thymolol) were administered into the conjunctival cavity once a day (at bedtime). The course of treatment lasted 30 days and was repeated every 2-3 months. Complete stabilization of myopia was achieved in 65-58% of cases, as evidenced by a follow-up of 3 years, on an average. Incomplete stabilization of the condition was observed in 35-42% of patients, the annual progress gradient being above 0.15 diopters. Simultaneously an accumulation of the relative accommodation reserves was observed, by 38.3% on an average, and a resolution of asthenopia. In a reference group of 50 subjects (100 eyes) spontaneous stabilization took place in 27% of cases, the relative accommodation reserve being unchanged.
