RESUMO

A 21-year-old man visited the GP complaining of food getting stuck in the back of his throat. We saw a thin string of tissue connecting the uvula to the palatopharyngeal arch and removed it surgically. No such case has previously been described in the absence of a history of trauma, infection or operation.

Introducción: la calidad educativa en las IES (Instituciones de Educación Superior) ha sido avalada por procesos de evaluación y acreditación a nivel institucional y curricular, llevando a cabo un proceso de mejora continua, mediante recomendaciones sugeridas por los evaluadores, lo anterior garantizará la calidad institucional ante la formación de futuros profesionales. Objetivo: analizar las experiencias del profesorado de la carrera de enfermería de la Facultad de Estudios Superiores Iztacala, sobre el cambio curricular vivido. Material y métodos: estudio cualitativo con enfoque fenomenológico, mediante entrevista a profundidad a doce profesores de enfermería. Muestreo por conveniencia de acuerdo a la saturación de datos. En aspectos éticos y legales, se realizó carta de consentimiento informado. El análisis de datos fue basado en Miles y Huberman, con abordaje teórico según Van Manen. Resultados: surgieron dos categorías: 1. Sentimientos y afectaciones emocionales respecto al cambio curricular y laborales; 2. impacto del cambio curricular en los aprendizajes de los estudiantes, las cuales evidenciaron cómo el profesorado ha vivido los cambios curriculares en los 17 años desde que fue implementada la licenciatura. Conclusiones: los informantes narraron que en cada cambio curricular surgen emociones diferentes, entre incertidumbre, compromiso, responsabilidad y asumir empoderamiento para implementar la curricula, con ello, lograr los objetivos esperados por la misma, considerando perfil de egreso y rendimiento académico de los estudiantes.


Introduction: the educational quality at the higher level in schools and faculties has been endorsed by evaluation and accreditation processes at the institutional and curricular level, carrying out an improvement process, through the recommendations suggested by the evaluators, thus covering them in a certain time. The foregoing will guarantee the institutional quality before the training of future professionals. Objective: to analyze experiences of the teaching staff of the nursing career of the Iztacala Faculty of Higher Studies, on the curricular change experienced. Material and methods: Qualitative study with a phenomenological approach, through an in-depth interview with twelve nursing professors. Convenience sampling according to data saturation. In ethical and legal aspects, an informed consent letter was drawn up. The data analysis was through Miles and Huberman, with a theoretical approach based on Van Manen. Results: two categories emerged: 1. Feelings and emotional affectations regarding the curricular and work change; 2. impact of curricular change on student learning, which showed how teachers have experienced curricular changes in the 17 years since the degree was implemented. Conclusions: the informants narrated that in each curricular change different emotions arise, between uncertainty, commitment, responsibility and assuming empowerment to implement the curriculum, thereby achieving the objectives expected by it, considering the graduation profile and the academic performance of the students.

Medulloblastoma (MB) treatment is continuously evolving. Better treatment approaches, focused on particular molecular pathways involved in MB development and progression support new treatment strategies. This article explores the antiproliferative, proapoptotic and radiosensitizing effects of Methoxyamine (MX), a base excision repair (BER) inhibitor that has shown anticancer potential by sensitizing tumor cells to ionizing radiation and chemotherapy. The DAOY (a desmoplastic cerebellar-derived MB) and ONS-76 (classical MB) cell lines were treated with MX at different concentrations, either alone or combined with various chemotherapeutic compounds: cisplatin (CDDP), temozolomide (TMZ) and thiotepa (THIO). Additionally, cell lines were exposed to MX and treated at different ionizing radiation fractions. Measurement of cell growth by XTT assay, clonogenic assay and detection of apoptotic cell death through caspase activity was obtained. Exposure to MX significantly decreased cell proliferation (p<0.05) while increasing cell apoptosis (p<0.05). Growth reduction was concentration-dependent for both DAOY and ONS-76 cells lines. Conversely, MX failed to enhance the cytotoxicity of CDDP, TMZ, and THIO. Moreover, MX treatment radiosensitized both cell lines, with ONS-76 cells being more prone to radiation effects at higher doses of exposure. These data support the role of MX as a direct cytotoxic compound for pediatric MB cells by inhibiting the BER pathway. Nevertheless, an antagonism, rather than a synergic or additive effect of MX with different concentrations of CDDP, TMZ and THIO was observed. Likewise, the radiosensitizing effect on MB cell lines seems to depend on radiation doses and MB subtype. This information may be relevant for clinical study designs employing BER inhibitors for MB.

OBJECTIVE: To estimate the impact of allowing non-prescription sales of nicotine medications in the United States on increasing the numbers of smokers quitting. DESIGN: Sales and marketing data were used to compare the use of nicotine medications before and after non-prescription sales, and to estimate the impact of non-prescription sales on quit rates. SETTING: United States. MAIN OUTCOME MEASURES: Number of quit attempts using nicotine replacement therapy (NRT) products, number of smokers who quit smoking with over-the-counter (OTC) NRT or with NRT still sold by prescription, and incremental quits attributable to OTC NRT. RESULTS: Since the US Food and Drug Administration approved nicotine medications for OTC sale in 1996, use of the medications has increased by 152% compared with prior prescription use. With increased use of an efficacious treatment, OTC nicotine medications are estimated to yield from 114,000-304,000 new former smokers annually in the United States. CONCLUSIONS: The broader availability and promotion of effective treatments for tobacco dependence, specifically nicotine gum and patch, increase the number of smokers availing themselves of the medications. This increased use is estimated to contribute substantially to the number of former smokers in the United States.
