RESUMO

The lung single photon emission tomographic (SPET) images of 985 consecutive patients referred for suspected pulmonary embolism were correlated with clinical outcome and angiography to evaluate the clinical usefulness of lung SPET compared to conventional planar ventilation/perfusion lung imaging. SPET interpretations followed the revised PIOPED criteria and clinical outcome was determined from referring physicians, hospital records, direct patient contact and county hall records. Patients were deemed to have had no clinically significant pulmonary embolism at the time of the SPET examination if, within the following 3 months: (1) the patient was alive and had no clinical evidence of pulmonary embolism or, (2) if deceased, pulmonary embolism was unlikely to have been the cause of death. Operating characteristics were based on the methods of Choi and of Simel. SPET interpretation was categorized as follows: high probability, 143 (14%); low probability, 840 (82%); intermediate, 41 (4%) (in contrast to PIOPED, with 39% intermediate interpretations). Pulmonary angiography was performed in only 4% of patients. Adequate follow-up data were available for 97% of patients. To facilitate comparison with PIOPED, either a high-probability or an intermediate-probability or an intermediate-probability study was considered to be a positive test, and either a low-probability or a normal study was considered to be a negative test. The sensitivity was 83% (PIOPED 82%), specificity 92% (PIOPED 52%), positive predictive value 62% (PIOPED 47%) and negative predictive value 97% (PIOPED 85%). The positive and negative predictive values have not been corrected for prevalence, which was approximately twice as high in the PIOPED study. Lung SPET provided accurate diagnostic information in 96% of patients and specificity was greatly improved compared to planar lung imaging reported in PIOPED. The diminished need for angiography greatly reduced the cost of evaluating patients suspected of having pulmonary embolism.

In a previous study we found no significant effects of acute exposure to the cotton gin environment on FEV1 in San Joaquin Valley gin workers compared with concurrent control subjects, nor was there an excess of obstructive lung disease among the same gin workers. The present study looked at the mean annual decline in FEV1, FEV1/FVC, and FEF25-75% in San Joaquin Valley cotton gin workers vs concurrent control subjects over four years. The FEV1 of gin workers completing all four years of the study declined 27.3 ml vs 35.5 ml in control subjects (p = 0.37); FEV1/FVC declined 0.21 percent in gin workers and 0.23 percent in control subjects (p = 0.86); FEF25-75% declined 0.058 L/s in gin workers and 0.066 L/s in control subjects (p = 0.68). Also, the mean annual rate of change of FEV1 was less among the gin workers who did not complete all four years than the gin workers completing all four years. The presence of "byssinosis" symptoms did not significantly influence the results. This prospective, longitudinal study failed to find any detrimental effect of the cotton gin environment on the rate of decline of FEV1, FEV1/FVC, and FEF25-75%. This result was independent of "byssinosis" symptoms and the effect of "dropouts."

Patients with seronegative and patients with seropositive rheumatoid arthritis (RA) have clinical, genetic, and immunologic differences. This study was undertaken to determine whether the two populations differ radiologically. Seventeen patients with seronegative RA were closely matched with seropositive control subjects. Radiographs of the hands and wrists were studied blindly, and disease severity was quantified with use of a modification of the Beaver Creek grading sheet. The following distinguishing features were also evaluated: osteosclerosis, new bone formation, carpal predominance, ankylosis (fusion), symmetry, and classical erosions. Seropositive patients had more severe disease, with larger and more numerous erosions, while the seronegative group had more osteosclerosis, carpal predominance, fusion, and new bone formation. Symmetry was equal in both groups. Although there were definite quantitative and qualitative differences between the two populations, radiologists should be cautious in using these criteria because of the great deal of overlap between the two groups.

No excess of obstructive airway disease was found in a group of 265 cotton gin workers when compared with other San Joaquin Valley agricultural workers. After an average of eight weeks' employment in San Joaquin Valley cotton gins, 125 workers showed no appreciable deterioration of pulmonary function compared with pre-employment measurements. In studies of pulmonary function during a workshift, cotton gin workers showed slightly greater mean decrements than control agricultural workers. These differences did not reach a level of statistical significance and were lower than those usually found in byssinosis. The unusual temporal pattern of employment in cotton gins in California precludes a simple approach to diagnosis by symptoms. No correlation was found in this study between symptoms of byssinosis and objective decrements in FEV1. The questionnaire as proposed by the Cotton Dust Standard was found to be of no value in detecting reactors in this study of gin workers.

A review of twelve cases in which disseminated coccidioidomycosis caused localized infection of the spine showed that eight of the twelve patients were alive and well with no evidence of active infection an average of eleven years after onset (range, two to thirty-five years). One patient who was followed for more than twenty-three years had a slowly developing neurological impairment in the lower extremities as a result of lumbosarcral destruction instability. One patient died early in the course of the disease from fulminating cervical spondylitis and quadriplegia. A second patient had a paraplegia from thoracic spondylitis. On patient had no evidence of active spondylitis five years after the onset of the disease, but then died of coccidioidal meningitis. All patients were treated with intravenous amphotericin at some time in the course of their illness, although its effect was not always dramatic. The three patients with neurological impairment did not undergo spine fusion, but most of the others had that operation. Surgical evacuation of abscesses and debridement of infected bone was also performed in many cases.

The clinical course of disseminated coccidioidomycosis is highly variable. Neither spontaneous cure nor spontaneous ankylosis has ever been demonstrated in an adult with the disease in one or more disseminated articular foci. Coccidioidomycotic arthritis may fluctuate in activity, and may be compatible with years of limited function and moderate morbidity. Amputation as well as arthrodesis accompanied by adequate excision of diseased tissue are generally reliable methods of treatment of infected joints, but the decision whether or not to "cover" such patients with systemic doses of amphotericin is still difficult, and the roles of synovectomy and topical amphotericin remain to be determined.

RESUMO

The phenotypes of serum alpha(1)-antitrypsin were determined by antigenantibody crossed electrophoresis. There were five homozygotes and 25 heterozygotes for the deficiency gene found in a group of 103 patients with obstructive lung disease. The frequency of heterozygotes was 14 and 9 percent in two control groups with different mean ages of 36 and 80. There was only one heterozygote among 39 healthy males over 70 years of age. Heterozygosity may be a predisposing factor in chronic obstructive lung disease, especially in the male population.
