RESUMO

Solitary fibrous tumor (SFT) of the lung is a rare neoplasm, usually originating from lung pleura. We present a case report of a 57-year-old male with no significant medical history who was incidentally diagnosed with an SFT of lung parenchyma on chest computed tomography scan. Radiological imaging revealed a well-defined mass in the left lower lobe of the lung. Biopsy and histopathological examination confirmed the diagnosis of solitary fibrous tumor. This case highlights the importance of considering SFT in the differential diagnosis of lung masses, as its clinical presentation and radiological features can mimic those of more common pulmonary malignancies.

Pseudoaneurysms are typically iatrogenic due to the increasing use of the artery for arterial interventions such as invasive vascular radiological procedures, invasive coronary artery procedures, arterial punctures for an arteriogram, or catheterization. Other reasons for pseudoaneurysm formation are intravenous drug use and penetrating trauma. They are more commonly observed in the lower limbs than in the upper limbs. In this case report, we present the occurrence of a brachial artery pseudoaneurysm (BAP) in a 73-year-old male patient who suffered a gunshot wound (GSW) 25 years ago during the war and was admitted to the hospital because of a fall episode. This case represents one of the few documented instances of a pseudoaneurysm formation following a GSW in the United States. Along with that, we describe the subsequent medical care provided to the patient.

Post-stroke recrudescence (PSR) is a clinical entity characterized by the acute transient recurrence of previously recovered focal stroke deficits. Various names have been used to describe PSR, which further complicates its diagnosis. Increased awareness of this condition is crucial for preventing inappropriate management and unnecessary testing. Mechanisms proposed for PSR include altered drug response in diseased brain areas and immune activation due to a compromised blood-brain barrier (BBB). Patients with PSR have a distinct vascular risk profile and fewer cardiovascular complications than those with transient ischemic attacks (TIAs). Accurate differentiation of PSR from other conditions that mimic stroke is essential for its appropriate management. Misdiagnosis may lead to unnecessary procedures and prolonged hospitalization. This article presents the case of a 56-year-old female with multiple episodes of PSR that were initially misdiagnosed in the emergency department. The patient had a history of hypertension and ischemic stroke, and her episodes of PSR were often triggered by elevated blood pressure. Future studies should focus on developing validated prediction scores to guide recurrent stroke workup. Enhancing awareness and understanding of PSR can optimize resource allocation and improve patient outcomes.

Recurrent and refractory bacterial vaginosis is a potentially hazardous condition that affects the age-bearing population of women. We report the case of a 33-year-old patient presenting with recurrent bacterial vaginosis after attempting multiple regimens for the past three years. The patient had a history significant for ectopic pregnancy and multiple sexually transmitted diseases. Successfully managing this condition in the female population is crucial to prevent uncommon complications. Furthermore, introducing healthy vaginal microbiota can be the best course of action amongst patients with long-term recurrence of bacterial vaginosis.

Myasthenia gravis (MG) belongs to a spectrum of autoimmune diseases in which anti-acetylcholine receptor antibodies damage neuromuscular junctions. It is a relatively rare disease with a higher incidence among the female population. The classical presentation is fatigable fluctuating diplopia or ptosis and, uncommonly, dysphagia or dysphonia. Even though it is rare, this condition can affect any skeletal muscle groups, including the neck or proximal limb muscles. There have been no reported cases of MG presenting as isolated neck weakness. An 81-year-old female patient presented with neck weakness associated with mild discomfort that progressively worsened throughout the day. Examination revealed reduced cervical muscular motor strength only. All imaging and laboratory investigations were within normal limits, except anti-acetylcholine receptor antibodies (binding Ab 12.04 nmol/L, blocking Ab 52% while modulating Ab 84%) with moderately elevated creatine phosphokinase (CPK) levels (350 U/l). The patient was prescribed Mestinon 60 mg QID (pyridostigmine), which led to rapid and significant relief of neck weakness. The patient has been stable on the medication for two years. MG typically presents in middle-aged female populations but, rarely, can also present with atypical symptoms among the elderly. Clinicians should have a high index of suspicion for myasthenia presenting with fatigable muscle weakness to reduce investigative costs and morbidity.
