RESUMO

Milia en plaque (MEP) is an uncommon skin condition identified as retroauricular confluent milium by Boulzer and Fouqet in 1903 [1]. It can be mistaken for other dermatoses like Favre-Racouchot nodular elastosis, steatocystoma multiplex, and nevus comedonicus. Milia en plaque can either be primary or secondary and is typically benign, often triggered by dermatological procedures like cryotherapy, as reported in this journal. In some cases, MEP can arise as a secondary manifestation of other diseases, including folliculotropic mycosis fungoides (FMF). Despite this association, there are few documented cases in the literature. Herein, we present a patient in whom MEP served as the initial clinical presentation of FMF; the treatment involved oral retinoids and phototherapy. Furthermore, we highlight distinctive features of both conditions. It is important to emphasize that early diagnosis and treatment of FMF are vital for the patient's quality of life. The presence of MEP can serve as a valuable indicator for identifying it.

Objetivo: Analisar a produção do cuidado de enfermagem às pessoas em sofrimento psíquico atendidas em um hospital geral na perspectiva da corresponsabilidade e integralidade. Método:Foi realizado estudo qualitativo, exploratório, por meio da análise documental, observação participante e entrevista semiestruturada com 12 técnicos de enfermagem e 15 enfermeiros do Hospital Municipal de Serrinha-Bahia/Brasil. Os dados foram analisados apartir análise de conteúdo. Resultados:O atendimento psiquiátrico no hospital geral é visto pela maioria da equipe como porta de entrada para o primeiro atendimento, porém demonstram sentir receio, insegurança e despreparo para lidar com estes pacientes,sinalizando a necessidade de educação permanente para elaboração e integração de novos saberes. Considerações finais: Para efetivação do novo modelo de assistência à saúde mental, faz-se necessário a promoção de reflexão direcionada a desconstrução de preconceitos e estigmas previamente estabelecidos. Nesse sentido, torna-se imprescindível a compreensão das novas práticas para o enfrentamento do processo de transição de paradigma, que demanda dos profissionais, a disponibilidade para rever suas próprias percepções e práticas diante dos desafios advindos do processo de aproximação da pessoa em sofrimento psíquico no serviço de saúde.


Objective: To analyze the production of nursing care for people in psychological distress treated in a general hospital from the perspective of co-responsibility and comprehensiveness. Method:A qualitative, exploratory study was carried out using document analysis, participant observation and semi-structured interviews with 12 nursing technicians and 15 nurses from the Municipal Hospital of Serrinha-Bahia/Brazil. The data was analyzed using content analysis. Results:Psychiatric care in the general hospital is seen by most of the team as the gateway to first aid, but they feel afraid, insecure and unprepared to deal with these patients, signaling the need for ongoing education to develop and integrate new knowledge.Final considerations: In order to implement the new mental health care model, it is necessary to promote reflection aimed at deconstructing previously established prejudices and stigmas. In this sense, it is essential to understand the new practices in order to cope with the process of paradigm transition, which requires professionals to be willing to review their own perceptions and practices in the face of the challenges arising from the process of approaching people in psychological distress in the health service.


Objetivo: analizar la producción de cuidados de enfermería a personas en situación de malestar psíquico atendidas en un hospital general desde la perspectiva de la corresponsabilidad y la integralidad. Resultados:La atención psiquiátrica en el hospital general es vista por la mayoría del equipo como la puerta de entrada a los primeros auxilios, pero se sienten temerosos, inseguros y poco preparados para tratar con estos pacientes, lo que señala la necesidad de una formación continua para desarrollar e integrar nuevos conocimientos. Consideraciones finales: Para implementar el nuevo modelo de atención en salud mental es necesario promover una reflexión orientada a deconstruir prejuicios y estigmas previamente establecidos. En este sentido, es esencial comprender las nuevas prácticas para hacer frente al cambio de paradigma, lo que requiere que los profesionales estén dispuestos a revisar sus propias percepciones y prácticas frente a los desafíos derivados del proceso de acercamiento a las personas que sufren enfermedades mentales en el servicio de salud.

BACKGROUND: A family history of arterial hypertension is a significant risk factor for the development of hypertension in youngsters. Thus, primary prevention has been emphasized in those subjects with a genetic predisposition. METHODS: This randomized clustered trial aimed to compare the effects of four modalities of aerobic training in postexercise hypotension. The primary outcomes were systolic blood pressure (SBP) and diastolic blood pressure (DBP). Secondly, peak oxygen uptake, heart rate, and subjective perceived exertion were analyzed. Nine normotensive men were randomized in four isocaloric sessions (200 kcal): high-intensity continuous training (HICT), moderate-intensity continuous training (MICT), high-intensity interval training (HIIT), and moderate-intensity interval training (MIIT). RESULTS: The area under the curve (AUC) showed a difference between sessions for SBP (F(3;8)=3.2; P=0.04), with MICT promoting a higher reduction than MIIT (P<0.05). In DBP there was also a difference (F(3;8)=15.3; P<0.001), with HICT reducing more than HIIT (P<0.05) and MIIT (P<0.05). Moderate-intensity protocols provided clinically relevant changes (CRC) in 11.1% of the individuals, and high-intensity protocols presented CRC in 50% of them (χ2=6.41; P=0.011) for SBP. For DBP, there was CRC in 27.8% of continuous conditions and none in the intervals (χ2=5.81; P=0.016). CONCLUSIONS: All training sessions promoted postexercise hypotension for DBP, and HICT provided higher reductions in the AUC. CRC for SBP were observed according to the intensity, while CRC for DBP were associated with modality.

The skin, by reflecting internal processes, externalizes what happens inside the body in many diseases. Thus, the skin, as an organ, extrapolates its functions of protection, barrier and signals the existence of systemic diseases, expanding the importance of the dermatologist beyond the skin surface. Thus, the dermatologist investigates diagnostic hypotheses for conditions related to all systems and refers patients to the appropriate specialty. Combined with examination by a trained eye, the skin, due to its easy access, is still the ideal place for performing biopsies, which often clarify the diagnosis. This manuscript is the second part of the article on cutaneous manifestations of systemic diseases. In the first part, the cutaneous manifestations of the main rheumatologic and granulomatous diseases were described, and vascular manifestations were also addressed. In the present article, it will be discussed how metabolic, cardiovascular, kidney, and gastrointestinal diseases can manifest themselves in the integumentary system. Malignant diseases and their cutaneous implications, will also be discussed. Pruritus and its clinical cutaneous correspondence will be discussed. Finally, an update on cutaneous signs of SARS-CoV2 coronavirus infection will be presented.

OBJECTIVES: To understand the implications of institutional racism in the therapeutic itinerary of patients with chronic renal failure (CRF) in the search for diagnosis and treatment of the disease. METHODS: Descriptive, qualitative study developed with 23 people with CRF in a regional reference hospital for hemodialysis treatment in Northeast Brazil. Two techniques of data collection were used: semi-structured interview and consultation to the NEFRODATA electronic medical record. For systematization and analysis, the technique of content analysis was used. RESULTS: Black and white people with CRF showed significant divergences and differences in their therapeutic itineraries: while white people had access to diagnosis during outpatient care in other medical specialties, black people were only diagnosed during hospitalization. In addition, white people had more access to private health plans when compared to black people, which doubles the possibility of access to health services. Moreover, even when the characteristics in the itinerary of black and white people were convergent, access to diagnosis and treatment proved to be more difficult for black people. CONCLUSIONS: The study showed the presence of institutional racism in the therapeutic itinerary of people with kidney disease in which black people have greater difficulty in accessing health services. In this sense, there is a need to create strategies to face institutional racism and to consolidate the National Policy for Comprehensive Health Care of the Black Population.

Changes in global and regional precipitation regimes are among the most pervasive components of climate change. Intensification of rainfall cycles, ranging from frequent downpours to severe droughts, could cause widespread, but largely unknown, alterations to trophic structure and ecosystem function. We conducted multi-site coordinated experiments to show how variation in the quantity and evenness of rainfall modulates trophic structure in 210 natural freshwater microcosms (tank bromeliads) across Central and South America (18°N to 29°S). The biomass of smaller organisms (detritivores) was higher under more stable hydrological conditions. Conversely, the biomass of predators was highest when rainfall was uneven, resulting in top-heavy biomass pyramids. These results illustrate how extremes of precipitation, resulting in localized droughts or flooding, can erode the base of freshwater food webs, with negative implications for the stability of trophic dynamics.

Comprender las implicaciones del racismo institucional en el itinerario terapéutico de pacientes con insuficiencia renal crónica (IRC) en la búsqueda del diagnóstico y tratamiento de la enfermedad. Métodos. Estudio descriptivo, cualitativo, desarrollado con la participación de 23 personas con IRC en un hospital regional de referencia para tratamiento de hemodiálisis en el noreste de Brasil. Se utilizaron dos técnicas de recolección de datos: entrevista semiestructurada y consulta a la historia clínica electrónica NEFRODATA. Para la sistematización y el análisis, se utilizó la técnica de análisis de contenido. Resultados. Las personas negras y blancas con IRC mostraron divergencias y diferencias en sus itinerarios terapéuticos: mientras que las personas blancas tuvieron acceso al diagnóstico durante la atención ambulatoria con otra especialidad médica, las personas negras solo fueron diagnosticadas durante la hospitalización. Además, las personas blancas tenían más acceso a planes de salud privados en comparación con las personas negras, lo que les brindaba una doble posibilidad de acceso a los servicios de salud. También se agrega que, incluso, cuando las características en el itinerario de las personas blancas y negras eran convergentes, el acceso al diagnóstico y al tratamiento fue más difícil para las personas negras. Conclusión. El estudio evidenció la presencia de racismo institucional en el itinerario terapéutico de las personas con enfermedad renal, en el que las personas negras tienen mayor dificultad para acceder a los servicios de salud. En este sentido, es necesario crear estrategias para enfrentar el racismo institucional y consolidar la Política Nacional para la Salud Integral de la Población Negra.


Objetivo. Compreender as implicações do racismo institucional no itinerário terapêutico de pacientes com insuficiência renal crônica (IRC) na busca pelo diagnóstico e tratamento da doença. Métodos. Estudo descritivo, qualitativo, desenvolvido com 23 pessoas com IRC em um hospital de referência regional para tratamento hemodialítico no Nordeste do Brasil. Foi utilizado duas técnicas de coleta de dados: entrevista semiestruturada e consulta ao prontuário eletrônico NEFRODATA. Para sistematização e análise utilizou-se a técnica de análise de conteúdo. Resultados. As pessoas negras e brancas com IRC apresentaram divergências e diferenças em seus itinerários terapêuticos: enquanto as pessoas brancas tiveram acesso ao diagnóstico durante o atendimento ambulatorial com outra especialidade médica, as pessoas negras só foram diagnosticadas durante internação hospitalar. Ademais, as pessoas brancas tiveram mais acesso a planos privados de saúde quando comparados às pessoas negras, o que confere dupla possibilidade de acesso aos serviços de saúde. Acrescenta-se ainda que, mesmo quando as características no itinerário das pessoas negras e brancas foram convergentes, o acesso ao diagnóstico e tratamento se mostrou mais dificultado para pessoas negras. Conclusão. O estudo evidenciou a presença de racismo institucional no itinerário terapêutico de pessoas com doença renal, em que pessoas negras possuem maior dificuldade de acesso aos serviços de saúde. Nesse sentido, há necessidade de criar estratégias para o enfrentamento do racismo institucional e para consolidação da Política Nacional de Saúde Integral da População Negra.

PURPOSE OF REVIEW: The purpose of this meta-analysis is to compare the effects of moderate-intensity continuous training (MICT) and high-intensity interval training (HIIT) on blood pressure of hypertensive individuals. RECENT FINDINGS: Continuous aerobic training programs are successful in health promotion and are effective in systolic blood pressure (SBP) and diastolic blood pressure (DBP) modulation. However, HIIT seems to be superior to MICT to improvement of cardiorespiratory fitness. PubMed, ScienceDirect, and Google Scholar were searched for randomized clinical trials that compared chronic effects of HIIT and MICT on BP in hypertensive subjects. Pre- and post-intervention changes in maximal oxygen uptake (VO2max) between MICT and HIIT were analyzed. Both interventions presented significant differences in SBP (MICT: mean difference (MD), 3.7 mmHg [95% CI = 2.57, 4.82], p < 0.00001; and HIIT: MD, 5.64 mmHg [95% CI = 1.69, 9.52], p = 0.005) and in DBP (MICT: MD, 2.41 mmHg [95% CI = 1.09, 3.72], p = 0.0003; and HIIT: MD, 4.8 mmHg [95% CI = 2.9, 6.7], p < 0.00001) compared with the control group. No differences were found in the SBP values (MD, 1.13 mmHg [95% CI = - 0.01, 2.27], p = 0.05); however, differences were found between groups in DBP (MD, 1.63 mmHg [95% CI = 0.83, 2.44], p = 0.0001). In the secondary outcome, both interventions increased VO2max in comparison with control groups (MICT: MD, 1.30 ml/kg/min [95% CI = 0.92, 1.68], p < 0.00001; and HIIT: MD, 4.90 ml/kg/min [95% CI = 3.77, 6.04], p < 0.00001), and HIIT promoted greater improvement than MICT (MD, 2.52 ml/kg/min [95% CI = 1.90, 3.13], p < 0.0001). In conclusion, HIIT and MICT promote reduction in SBP in adults with hypertension, and HIIT showed a greater magnitude in DBP reduction. For hypertensive patients, HIIT may be associated with a greater improvement in VO2max than MICT might.

There is growing recognition that ecosystems may be more impacted by infrequent extreme climatic events than by changes in mean climatic conditions. This has led to calls for experiments that explore the sensitivity of ecosystems over broad ranges of climatic parameter space. However, because such response surface experiments have so far been limited in geographic and biological scope, it is not clear if differences between studies reflect geographic location or the ecosystem component considered. In this study, we manipulated rainfall entering tank bromeliads in seven sites across the Neotropics, and characterized the response of the aquatic ecosystem in terms of invertebrate functional composition, biological stocks (total invertebrate biomass, bacterial density) and ecosystem fluxes (decomposition, carbon, nitrogen). Of these response types, invertebrate functional composition was the most sensitive, even though, in some sites, the species pool had a high proportion of drought-tolerant families. Total invertebrate biomass was universally insensitive to rainfall change because of statistical averaging of divergent responses between functional groups. The response of invertebrate functional composition to rain differed between geographical locations because (1) the effect of rainfall on bromeliad hydrology differed between sites, and invertebrates directly experience hydrology not rainfall and (2) the taxonomic composition of some functional groups differed between sites, and families differed in their response to bromeliad hydrology. These findings suggest that it will be difficult to establish thresholds of "safe ecosystem functioning" when ecosystem components differ in their sensitivity to climatic variables, and such thresholds may not be broadly applicable over geographic space. In particular, ecological forecast horizons for climate change may be spatially restricted in systems where habitat properties mediate climatic impacts, and those, like the tropics, with high spatial turnover in species composition.

Background: Maximum oxygen consumption (VO2 max) in healthy individuals decreases approximately 10% per decade of life, and such decrease is more pronounced after the seventh decade. Objectives: To assess functional capacity of individuals aged 75 years or older, submitted to ergometric test and transthoracic echocardiogram exam, by means of metabolic equivalent (MET) and VO2 max measurements. Methods: A total of 381 patients (205 women; 79 ± 3.7 years) were evaluated. Exclusion criteria were: presence of left ventricular (LV) systolic dysfunction, LV diastolic dysfunction grade II and III, significant valve disease, or coronary artery disease with systolic LV dysfunction or dilatation. Associations between quantitative variables were analyzed by Pearson and Spearman correlation coefficients, and comparisons of quantitative data by Student's t-test for independent samples.Results: Increasing age was associated with a progressive decrease in the distance covered (p = 0.021), in the expected increase in HR (p < 0.001), in VO2 max (p < 0.001), and METs (p < 0.001) in both genders. There was no correlation of exercise test parameters with the echocardiographic parameters. Conclusions: Relatively healthy older individuals, with global systolic and diastolic functions of the left ventricle preserved, presented a progressive decrease in their functional capacity due to their natural aging process, comorbidities related to their age range and physical deconditioning

ABSTRACT Objective: To analyze the characteristics of nurses' work process in different countries. Method: We have used the integrative review method and selected 84 publications (articles, theses and dissertations) in national and foreign thesis banks and databases. We analyzed the evidence based on dialectical materialism. Results: The rejection of managerial tasks hides the singularity of nurses' work, due to the failure to understand the inseparable nature of managerial and healthcare tasks, given that it is what provides the expertise to coordinate the nursing work process and guide the healthcare work processes. The social and technical division is present in the work process in all countries studied, albeit in different ways. The nurse's position in the healthcare work process is subordinated to that of the physician. Conclusion: The characteristics are similar. The rejection of the dual nature of the work by nurses themselves due to alienation results in the non-recognition of their own work.

Método: Utilizamos el método de la revisión integradora y seleccionamos 84 publicaciones (artículos, tesis y disertaciones) en bases de datos y banco de tesis nacionales e internacionales. Las evidencias fueron analizadas en la perspectiva del materialismo dialéctico. Resultados: La negación del trabajo gerencial esconde la singularidad del trabajo de la enfermera, por la no comprensión de la naturaleza indisociable gerencial-asistencial, visto que eso confiere la experiencia para coordinar el proceso de trabajo en enfermería y direccionar los procesos de trabajo en salud. La división técnica y social está presente en el proceso de trabajo en todos los países estudiados, aunque bajo formas distintas. El lugar de la enfermera en el proceso de trabajo asistencial está subordinado al médico. Conclusión: Las características son semejantes. La negación de la propia naturaleza dual del trabajo por la enfermera, vía alienación, culmina en el no reconocimiento del propio trabajo.

RESULTS: The rejection of managerial tasks hides the singularity of nurses' work, due to the failure to understand the inseparable nature of managerial and healthcare tasks, given that it is what provides the expertise to coordinate the nursing work process and guide the healthcare work processes. The social and technical division is present in the work process in all countries studied, albeit in different ways. The nurse's position in the healthcare work process is subordinated to that of the physician. CONCLUSION: The characteristics are similar. The rejection of the dual nature of the work by nurses themselves due to alienation results in the non-recognition of their own work.

Phytotelmata in tank-forming Bromeliaceae plants are regarded as potential miniature models for aquatic ecology, but detailed investigations of their microbial communities are rare. Hence, the biogeochemistry in bromeliad tanks remains poorly understood. Here we investigate the structure of bacterial and archaeal communities inhabiting the detritus within the tanks of two bromeliad species, Aechmea nudicaulis and Neoregelia cruenta, from a Brazilian sand dune forest. We used metagenomic sequencing for functional community profiling and 16S sequencing for taxonomic profiling. We estimated the correlation between functional groups and various environmental variables, and compared communities between bromeliad species. In all bromeliads, microbial communities spanned a metabolic network adapted to oxygen-limited conditions, including all denitrification steps, ammonification, sulfate respiration, methanogenesis, reductive acetogenesis and anoxygenic phototrophy. Overall, CO2 reducers dominated in abundance over sulfate reducers, and anoxygenic phototrophs largely outnumbered oxygenic photoautotrophs. Functional community structure correlated strongly with environmental variables, between and within a single bromeliad species. Methanogens and reductive acetogens correlated with detrital volume and canopy coverage, and exhibited higher relative abundances in N. cruenta. A comparison of bromeliads to freshwater lake sediments and soil from around the world, revealed stark differences in terms of taxonomic as well as functional microbial community structure.

Climate change and biodiversity loss have been reported as major disturbances in the biosphere which can trigger changes in the structure and functioning of natural ecosystems. Nonetheless, empirical studies demonstrating how both factors interact to affect shifts in aquatic ecosystems are still unexplored. Here, we experimentally test how changes in rainfall distribution and litter diversity affect the occurrence of the algae-dominated condition in tank bromeliad ecosystems. Tank bromeliads are miniature aquatic ecosystems shaped by the rainwater and allochthonous detritus accumulated in the bases of their leaves. Here, we demonstrated that changes in the rainfall distribution were able to reduce the chlorophyll-a concentration in the water of bromeliad tanks affecting significantly the occurrence of algae-dominated conditions. On the other hand, litter diversity did not affect the algae dominance irrespective to the rainfall scenario. We suggest that rainfall changes may compromise important self-reinforcing mechanisms responsible for maintaining high levels of algae on tank bromeliads ecosystems. We summarized these results into a theoretical model which suggests that tank bromeliads may show two different regimes, determined by the bromeliad ability in taking up nutrients from the water and by the total amount of light entering the tank. We concluded that predicted climate changes might promote regime shifts in tropical aquatic ecosystems by shaping their structure and the relative importance of other regulating factors.

MOTIVATION: Around 75% of all mass spectra remain unidentified by widely adopted proteomic strategies. We present DiagnoProt, an integrated computational environment that can efficiently cluster millions of spectra and use machine learning to shortlist high-quality unidentified mass spectra that are discriminative of different biological conditions. RESULTS: We exemplify the use of DiagnoProt by shortlisting 4366 high-quality unidentified tandem mass spectra that are discriminative of different types of the Aspergillus fungus. Climate change will alter the distribution of rainfall, with potential consequences for the hydrological dynamics of aquatic habitats. Hydrological stability can be an important determinant of diversity in temporary aquatic habitats, affecting species persistence and the importance of predation on community dynamics. As such, prey are not only affected by drought-induced mortality but also the risk of predation [a non-consumptive effect (NCE)] and actual consumption by predators [a consumptive effect (CE)]. Climate-induced changes in rainfall may directly, or via altered hydrological stability, affect predator-prey interactions and their cascading effects on the food web, but this has rarely been explored, especially in natural food webs. To address this question, we performed a field experiment using tank bromeliads and their aquatic food web, composed of predatory damselfly larvae, macroinvertebrate prey and bacteria. We manipulated the presence and consumption ability of damselfly larvae under three rainfall scenarios (ambient, few large rainfall events and several small rainfall events), recorded the hydrological dynamics within bromeliads and examined the effects on macroinvertebrate colonization, nutrient cycling and bacterial biomass and turnover. Despite our large perturbations of rainfall, rainfall scenario had no effect on the hydrological dynamics of bromeliads. As a result, macroinvertebrate colonization and nutrient cycling depended on the hydrological stability of bromeliads, with no direct effect of rainfall or predation. In contrast, rainfall scenario determined the direction of the indirect effects of predators on bacteria, driven by both predator CEs and NCEs. These results suggest that rainfall and the hydrological stability of bromeliads had indirect effects on the food web through changes in the CEs and NCEs of predators. We suggest that future studies should consider the importance of the variability in hydrological dynamics among habitats as well as the biological mechanisms underlying the ecological responses to climate change.

Abstract Steatocystoma multiplex is a rare genetic disorder characterized by the presence of hamartomatous malformations at the junction of the pilosebaceous duct. It consists of encapsulated cystic lesions in the dermis, with adjacent sebaceous gland. When associated with inflammation, resembling hidradenitis, it is called steatocystoma multiplex suppurativa, a condition rarely reported. This is the first case of steatocystoma multiplex suppurativa reported in the Brazilian literature. Female patient, 23 years old, with papular and nodular cystic lesions that started in the armpits and groin, later spreading to the trunk, lower limbs, anticubital fossa, face and scalp. The presence of papular-nodular lesions associated with disseminated hidradenitis-like lesions in flexural areas and the histopathological diagnosis of steatocystoma defined the diagnosis of steatocystoma multiplex suppurativa.

Abstract Gianotti-Crosti syndrome is a rare disease characterized by acral papular eruption with symmetrical distribution. It is a benign and self-limited disease; the symptoms disappear after two to eight weeks, without recurrences or scars. Skin lesions are usually asymptomatic. Prodrome might occur, suggesting upper respiratory infection, or constitutional symptoms. Diagnosis is eminently clinical, and this disease is associated with viral infections. Due to its rarity and low occurrence in adolescents and adults, we report a case of Gianotti-Crosti syndrome of a teenager.

BACKGROUND: Fratricide is the killing of one's own bother. It's a type of homicide rarely seen on psychiatric practice. This is still a theme which is poorly studied, and not well understood by the scientific literature. OBJECTIVE: To report a case of a men, with paranoid schizophrenia that murdered his own bother and had a psychiatric forensic evaluation to establish his penal responsibility. METHODS: A psychiatric interview was carried out and the psychiatric diagnosis was established based on the interview and analysis of forensic and medical records, using the DSM-IV-TR criteria. Literature review was held about the theme. RESULTS: The examinee was considered not guilty by reason of insanity, due to the presence of a mental disorder that affected her entire understanding and volition of the practiced act. CONCLUSIONS: The study of such cases may illustrate and identify motivating factors related to homicidal behavior in individuals with severe mental disorders.
