RESUMO

PURPOSE: To compare the anatomic and functional outcomes of half-dose photodynamic therapy (PDT) and yellow 577-nm subthreshold micropulse laser (SMLT) in treating patients with chronic central serous chorioretinopathy (CSCR). DESIGN: Prospective, double-masked, randomized, controlled clinical trial. PARTICIPANTS: Patients with chronic CSCR confirmed by clinical features and multimodal imaging. METHODS: Eligible patients were randomized (1:1) to receive half-dose PDT or SMLT. The same treatment was repeated if persistent subretinal fluid (SRF) was observed. Treatment responses were evaluated 1 month after treatment and every 3 months until the end point at 12 months. MAIN OUTCOME MEASURES: The primary outcome measure was the complete resolution of SRF on OCT scan at month 12. Secondary outcomes included the changes in best-corrected visual acuity (BCVA), central macular thickness (CMT) as measured by OCT, retinal sensitivity as measured by microperimetry, and vision-related quality of life using the National Eye Institute 25-Item Visual Function Questionnaire. RESULTS: Between April 2017 and October 2020, 68 patients were recruited. At 1 month after treatment, SRF resolved in 8 (24.2%) of 33 patients receiving SMLT and in 20 (58.8%) of 34 patients receiving half-dose PDT. This increased to 23 (82.1%) of 28 patients in the SMLT group and 30 (90.9%) of 33 patients in the half-dose PDT group at 12 months of follow-up. Kaplan-Meier survival curves showed significantly faster resolution of SRF in the half-dose PDT group than the SMLT group (P = 0.016). Both groups showed significant improvement in BCVA (-0.12 ± 0.21, P = 0.005 for SMLT; -0.13 ± 0.12, P < 0.001 for half-dose PDT), CMT (-154.2 ± 105.6, P < 0.001 for SMLT; -140.8 ± 94.0, P < 0.001 for half-dose PDT), and retinal sensitivity (5.70 ± 5.02, P < 0.001 for SMLT; 6.05 ± 3.83, P < 0.001 for half-dose PDT) at 12 months compared with baseline. There was no significant difference between the 2 treatment groups at each time point in all investigations except BCVA at 3 months (P = 0.03). CONCLUSIONS: When comparing half-dose PDT to subthreshold SMLT, this study has shown both treatments to be viable options, with half-dose PDT achieving faster anatomic success and functional improvement. Purpose: The aim of this study was to evaluate and compare the electrical performance and properties of commercially available electroretinography (ERG) electrodes. Methods: A passive ionic model was used to measure impedance, noise, and potential drift in 10 types of ocular surface and skin ERG electrodes. Results: The impedance for silver-based ocular electrodes are generally lower (range, 65.35-343.3 Ω) with smaller phase angles (range, -6.41° to -33.91°) than gold-based electrodes (impedance ranged from 285.95 Ω to 2.913 kΩ, and phase angle ranged from -59.65° to -70.01°). Silver-based ocular electrodes have less noise (median line noise of 6.48 x 104nV2/Hz) than gold-based electrodes (median line noise of 2.26 x 105nV2/Hz). Although silver-based electrodes usually achieve a drift rate less than 5 µV/s within 15 minutes, gold-base ocular electrode cannot achieve a stable potential. The exception is the RETeval strip type of silver electrode, which had an unusual drift at 20 minutes. The noise spectral density showed no change over time indicating that noise was not dependent on the stabilization of the electrode. Conclusions: From the range of electrodes tested, lower impedance, lower capacitance, and lower noise was observed in silver-based electrodes. Stabilization of an electrode is effective against drift of the electrode potential difference but not the noise. Translational Relevance: Application of electrodes with optimized materials improve the quality of clinical electrophysiology signals and efficiency of the recording.

Pneumoconiosis, caused by inhalation of mineral dusts, is a major occupational disease worldwide. Currently, there are no effective drugs owing to a lack of potential therapeutic targets during either the inflammation or fibrosis molecular events in pneumoconiosis. Here, we performed microarrays to identify aberrantly expressed genes in the above molecular events in vitro and found a hub gene transforming growth factor-ß-activated kinase 1 (TAK1), which was highly expressed and activated in pneumoconiosis patients as well as silica-exposed rats with experimental pneumoconiosis. Genetic modulation of TAK1 by CRISPR (clustered regularly interspaced short palindromic repeats)/Cas9, RNA interference and overexpression indicated the important role of TAK1 in both inflammation and fibrosis in experimental pneumoconiosis. To achieve pharmacological TAK1 inhibition, we virtually screened out a natural product resveratrol, which targeted TAK1 at both N161 and A107 residues, and significantly inhibited TAK1 activation to attenuate inflammation and fibrosis in vitro. Consistently, in vivo prevention and intervention studies showed that resveratrol could inhibit pulmonary inflammation and fibrosis in silica-exposed rats.

BACKGROUND: Age-associated decrease in type IIA/B human skeletal muscle fibers was detected in human biopsies in our previous study. The relationship between change in muscle fiber typing and bone mineral density (BMD) is, however, unknown either cross-sectionally or longitudinally. We therefore conducted a cross-sectional study to investigate their correlation using human muscle biopsies. METHODS: Forty human subjects aged (53.4 ± 20.2) years were recruited. Histomorphometric parameters of their muscle biopsies were measured by ATPase staining and image analysis, including average area percentage, fiber number percentage, mean fiber area, and area percentage of connective tissues. Hip and spine BMD was measured by dual-energy X-ray absorptiometry. Partial correlation with adjusting age was performed. RESULTS: Type IIB muscle fiber was found positively correlated with hip BMD irrespective to age and demonstrated significantly stronger relationship with BMD among all fiber types, in terms of its cross-sectional area (r = 0.380, P = 0.029) and size (r = 0.389, P = 0.025). Type IIA muscle fibers associated with hip BMD in mean fiber area only (r = 0.420, P = 0.015). CONCLUSIONS: Type IIB muscle fiber may play an important role in maintaining bone quality. This may also be a relatively more sensitive fiber type of sarcopenia and osteoporosis. These findings further consolidate the muscle-bone relationship.

This paper reports a new capillary zone electrophoresis method for the simultaneous chiral determination of the enantiomers of N,N-dimethylamphetamine (DMA), methamphetamine (MA), ephedrine (E), pseudoephedrine (PE) and methylephedrine (Me-E). In this study, a number of electophoretic parameters were examined and optimized including the choice of chiral selectors, the use of short-chain tetraalkylammonium cations, the effect of chiral selector concentration, buffer concentration, applied voltage and capillary temperature. Heptakis(2,6-di-O-methyl)-beta-cyclodextrin (DM-beta-CD) and tetrabutylammonium (TBA) being the best chiral selector and buffer cation, respectively, the optimized electrophoretic conditions were found to be: 20 mM DM-beta-CD, 50 mM TBAPO(4) at pH 2.5, applied voltage at 30 kV and temperature at 25 degrees C. Under the optimized conditions, all analytes were well separated with resolution factors between 3.3 and 24.0 achieved. Using phentermine as an internal standard, the intra-day (n=8) precisions for the relative migration times and peak areas of all analytes were below 1.09%, while the inter-day precisions (n=12, 6 days) for the relative migration times and peak areas of all analytes were under 3.77%. This method has been applied to the measurement of the enantiomeric purities of the seized DMA samples. The result of the measurement could provide important clues about the possible synthetic pathways of these samples.

Elderly individuals often fall because of poor muscle strength and reduced balancing ability related to muscle aging. However, it is unclear whether changes in muscle fiber types contribute to poor strength or imbalance. We studied age- associated changes in human skeletal muscle fibers using muscle biopsy specimens taken from 65 male and female Chinese patients aged 17-96 years. The muscle specimens were cryosectioned with alkaline triphosphatase staining at pH 4.4, followed by image analysis. We analyzed morphologic observations and performed quantitative analyses of the number, size, and area percentage of different types of skeletal muscle fibers and connective tissues. Types IIA and IIB muscle fibers decreased with age in the area percentage, fiber number percentage, and mean fiber area, whereas Type I fibers increased in area and number but not in size. Morphologically, Type II fibers appeared smaller and flatter. Our findings suggest deterioration in muscle quality and balancing coordination in elderly patients. We provide data to help determine treatments for reversing muscle fiber changes and reducing the number of falls and related fractures in patients.

Chiral C2-symmetric quaterpyridine L reacts with [Pd(eta3-C3H5)Cl]2 to form a chiral single-stranded helical binuclear palladium complex of formula [Pd2(eta3-C3H5)2(L)]2+; the complex can efficiently catalyze allylic substitution of 1,3-diphenylprop-2-enyl acetate with dimethyl malonate with enantioselectivity up to 85%.

A within-subject, randomized, and prospective intervention trial was done to evaluate the potential effect of low-intensity pulsed ultrasound on prevention of postmenopausal bone loss. Twenty healthy postmenopausal women between the ages of 51-81 years met the inclusion criteria. The treatment hand was randomly selected, and the contralateral site served as a control. Integral and trabecular bone mineral density were measured using highly precise multilayer peripheral quantitative computed tomography at the bilateral distal radius at baseline, 3 months after daily low-intensity pulsed ultrasound treatment, and 3 months after discontinuing treatment. Results showed that the rate of bone change (trabecular bone mineral density and integral bone mineral density) did not significantly differ between the site treated with low-intensity pulsed ultrasound and the contralateral control at either followup. Also, during the followup, bone mineral density did not change significantly in the contralateral control site. This was the first prospective and randomized study to show that low-intensity pulsed ultrasound at the current regime did not have significant effects on intact bone for prevention of postmenopausal bone loss in the distal radii of older Chinese women.

A clinical study was conducted to investigate the effect of low-intensity pulsed ultrasound (US) stimulation (LIPUS) on the healing of complex tibial fractures. Thirty complex tibial fractures were randomly assigned to the treatment with LIPUS (n = 16) or by a dummy machine (sham-exposed: n = 14). The fractures were immobilized by either internal or external fixations according to the clinical indications. LIPUS was given 20 min/day for 90 days. Fracture healing was monitored by clinical, radiological, densitometric and biochemical assessments. The LIPUS-treated group showed statistically significantly better healing, as demonstrated by all assessments. Complications were minimal in the LIPUS group. There were two cases of delayed union, with one in each group. There were two cases of infection in the control group. The delayed-union cases were subsequently treated by LIPUS and the infection cases were treated with standard protocol. Fracture healing in these patients was again treated by LIPUS.
