RESUMO

The present review focuses on the side effects that ex-obese patients face following bariatric surgery. We searched through the principal medical indexes (SCOPUS, Web of Science, PubMed, MEDLINE) using the following words, both alone and in combinations: bariatrics; bariatric surgery; anemia; vitamin B12; cobalamin; folate; folic acid; iron; iron supplements; gut microbiota; lactalbumin; α-lactalbumin. To perform exhaustive research, we considered articles published since 1985. Bariatric surgery induces states of nutritional deficiencies. In particular, the surgery results in a drastic fall in the levels of iron, cobalamin, and folate. Despite the dietary supplements which can counteract such decrease, some limitations exist in the nutraceutical approach. Indeed, the gastrointestinal side effects of supplements, the alterations in the microbiota, and the reduced absorption induced by the surgery may impair the effect of dietary supplements, exposing the patients to the risk of developing nutritional deficiencies. Recent literature reports the effect of promising molecules to counteract such limitations, which include α-lactalbumin, a whey protein with prebiotic activities, and new pharmaceutical forms of iron supplements, namely micronized ferric pyrophosphate. If on the one hand, α-lactalbumin enhances intestinal absorption and helps in restoring a physiological microbiota, micronized ferric pyrophosphate has a high tolerability and low or null risk of gastrointestinal side effects. Bariatric surgery represents a valid solution to obesity and obesity-related disease. However, the procedure may induce deficiencies in micronutrients. Data exists on the promising activities of α-lactalbumin and micronized ferric pyrophosphate, which may help in preventing bariatric-induced anemia.

OBJECTIVE: The human papilloma virus (HPV) is the etiological agent of cervical cancer in more than 95% of cases worldwide. Although most HPV infections clear up on their own and most pre-cancerous lesions spontaneously resolve, in some cases, they can persist, leading to lesions which may progress towards invasive cervical cancer. MATERIALS AND METHODS: We evaluated the effects of the association of epigallocatechin gallate (EGCG) + folic acid (FA) + vitamin B12 (B12) + hyaluronic acid (HA) on HPV-positive cervical cancer cells (HeLa). RESULTS: The association of EGCG + FA + B12 + HA induced a significant increase of apoptosis and p53 gene expression with a concomitant decrease of E6/E7 gene expression, a marker of HPV infection. CONCLUSIONS: This study provides for the first-time evidence on the potential additive activity of EGCG + FA + B12 + HA in counteracting HPV infection, by increasing apoptosis and p53 expression in HPV-infected cervical HeLa cells.

OBJECTIVE: Mood stabilizers like lithium (Li) and valproic acid (VPA) act via cellular depletion of inositol in the central nervous system (CNS). However, such depletion also involves peripheral tissues, thus exposing patients to various side effects. Preclinical and clinical studies revealed the effectiveness of inositol supplementation to recover such pathological conditions. Nevertheless, the risk of reducing the effectiveness of pharmacological therapies by raising inositol levels in the CNS, still represents a matter of concern. This study adds new insights on this aspect, highlighting the safety of a tailored dosage of inositol in patients taking Li or VPA. PATIENTS AND METHODS: We enrolled 15 patients over 18 years of age taking Li and/or VPA. They assumed 2 gr of myo-inositol (myo-ins) and d-chiro-inositol (d-chiro-ins) in the combined 80:1 ratio, plus 50 mg of α-lactalbumin (α-LA), twice a day for a total period of 6 months (T1). Evaluating the interference of such dietary supplementation with pharmacological therapy was the primary outcome. Monitoring blood levels of thyroid (fT3, fT4, TSH) and metabolic markers (fasting insulin, glucose, HOMA-IR index, triglycerides, HDL, LDL) were secondary outcomes. The analysis was carried out by comparing values at baseline (T0) and T1 (6 months). RESULTS: After 6 months, pharmacological therapy was still suitable for patients, requiring no changes nor adjustments. Instead, inositol treatment improved those borderline values about thyroid functionality and glucose and lipid metabolism. CONCLUSIONS: This pilot study demonstrated that the dosage of 4 gr/daily of inositol is safe in patients taking Li/VPA, as we recorded no interference with the pharmacological therapy. Moreover, such treatment may counteract or even prevent side effects, thus improving patients' quality of life.

BACKGROUND: The advantages of barbed suture for tendon repair could be to eliminate the need for a knot and to better distribute the load throughout the tendon so as to reduce the deformation at the repair site. The purpose of this study was to evaluate the breaking force and the repair site deformation of a new barbed tenorrhaphy technique in an animal model. MATERIALS AND METHODS: Sixty porcine flexor tendons were divided randomly into three groups and repaired with one of the following techniques: a new 4-strand barbed technique using 2/0 polypropylene Quill™ SRS or 2/0 polydioxanone Quill™ SRS and a modified Kessler technique using 3/0 prolene. All tendons underwent mechanical testing to assess the 2-mm gap formation force, the breaking force and the mode of failure. The percentage change in tendon cross-sectional area before and after repair was calculated. RESULTS: The two-sample Student t-test demonstrated a significant increase in 2-mm gap formation force and in breaking force with barbed sutures, independently from suture material, when compared to traditional Kessler suture. Concerning the tendon profile, we registered less bunching at the repair site in the two barbed groups compared with the Kessler group. CONCLUSIONS: This study confirms the promising results achieved in previous ex vivo studies about the use of barbed suture in flexor tendon repair. In our animal model, tenorrhaphy with Quill™ SRS suture guarantees a breaking force of repair that exceeds the 40-50 N suggested as sufficient to initiate early active motion, and a smoother profile at the repair site. This experimental work is oriented to give a contribution to the knowledge of the relationship among surface roughness parameters and tribological properties of lubricated surfaces; it is well known that these surface properties are strictly related, but a complete comprehension of such correlations is still far to be reached. For this purpose, a mechanical polishing procedure was optimized in order to induce different, but well controlled, morphologies on Si(100) surfaces. The use of different abrasive papers and slurries enabled the formation of a wide spectrum of topographical irregularities (from the submicro- to the nano-scale) and a broad range of surface profiles. An AFM-based morphological and topographical campaign was carried out to characterize each silicon rough surface through a set of parameters. Samples were subsequently water lubricated and tribologically characterized through ball-on-disk tribometer measurements. Indeed, the wettability of each surface was investigated by measuring the water droplet contact angle, that revealed a hydrophilic character for all the surfaces, even if no clear correlation with roughness emerged. Nevertheless, this observation brings input to the purpose, as it allows to exclude that the differences in surface profile affect lubrication. So it is possible to link the dynamic friction coefficient of rough Si samples exclusively to the opportune set of surface roughness parameters that can exhaustively describe both height amplitude variations (Ra, Rdq) and profile periodicity (Rsk, Rku, Ic) that influence asperity-asperity interactions and hydrodynamic lift in different ways. For this main reason they cannot be treated separately, but with dependent approach through which it was possible to explain even counter intuitive results: the unexpected decreasing of friction coefficient with increasing Ra is justifiable by a more consistent increasing of kurtosis Rku.

The aim of this study was to verify whether different living conditions of Polychaeta are correlated with morphological and functional differences in the organization of the integument. For this purpose, we decided to study the epidermis of Timarete filigera, a non-tubicolous polychaete. With this objective in mind, we have identified the various cellular types responsible for mucous secretion in the epidermis of this species and defined the histochemical composition of the mucus produced by different types of gland cells. Three types of gland cells have been identified by histochemical and ultrastructural studies in the epidermis of this polychaete. The histochemistry was carried out using standard techniques and peroxidase-labelled lectins. In type 1 cells, the secretory granules contain neutral glycoproteins with glucosidic residues of GalNAc, Galbeta 1,3 GalNAc, glucosidic and/or mannosidic residues. In type 2 cells, the secretory granules contain acid glycoproteins mainly sulphated with glucosidic residues of GalNAc, Galbeta 1,3 GalNAc, glucosidic and/or mannosidic residues, and some terminal sialic acid. In type 3 cells, the residual granules have the same chemical composition as that of granules present in type 2 cells. The secretion of these glandular mucous cells consists of mainly sulphated acidic glycoproteins and GAG resistant to testis jaluronidase. In these cells, the residual granules have the same chemical composition as that of their secretion. The heterogeneity of mucus composition may be correlated with its different functions.

A histochemical study is described that characterizes microgranular cells of the demosponge Cinachyra tarentina (C. tarentina) with the use of routine staining methods for mucosubstances, lectin histochemistry and electron microscopy. Microgranular cells are rare or absent in other species of sponges, but abundant in this species. Microgranular cells are present in both ectosome and mesohyl, particularly along the canal of the aquiferous system and around spicule holes. Inclusions of microgranular cells and the extracellular matrix were particularly positive for acidic glycoproteins with abundant sulfated ester groups and glycosidic residues containing GalNAc and Galbeta1,3GalNAc. Terminal L-fucose bound to the penultimate GalNAc residues and/or difucosylated oligosaccharides were present as well. Our results suggest that soybean lectin (SBA), peanut lectin (PNA), and winged pea lectin (WPA) are valuable markers for identifying microgranular cells of C. tarentina. Electron microscopy revealed some of the microgranular cells to contain small smooth cytoplasmic vesicles originating from the Golgi complex and few electron-dense granules, others were characterized by numerous secretory granules and vacuoles formed by vesicle fusion and connected with the plasma membrane. Our results suggest that microgranular cells in C. tarentina contribute to the synthesis of glycoprotein components of the extracellular matrix.

There is a Standard Objection to the idea that concepts might be prototypes (or exemplars, or stereotypes): Because they are productive, concepts must be compositional. Prototypes aren't compositional, so concepts can't be prototypes. However, two recent papers (Osherson and Smith, 1988; Kamp and Partee, 1995) reconsider this consensus. They suggest that, although the Standard Objection is probably right in the long run, the cases where prototypes fail to exhibit compositionality are relatively exotic and involve phenomena which any account of compositionality is likely to find hard to deal with; for example, the effects of quantifiers, indexicals, contextual constraints, etc. In this paper, we argue that the Standard Objection to prototype theory was right after all: The problems about compositionality are insuperable in even the most trivial sorts of examples; it is therefore as near to certain as anything in cognitive science ever gets that the structure of concepts is not statistical. Theories of categorization, concept acquisition, lexical meaning and the like, which assume the contrary simply don't work.

Stelletta grubii is an oviparous demosponge, which, during its reproductive period from summer to autumn, has small eggs (80-90 microns) dispersed uniformly in the mesohyl. The nucleolated nucleus is surrounded by dictyosomes containing small vesicles, which contribute to form reserve material. Vesicles, numerous food vacuoles, and groups of mitochondria are observed in the granular cytoplasm. Electron-dense yolk inclusions and lipids are found peripherally. The cortical portion of the egg cytoplasm possesses vacuoles with fibrillar contents. The egg forms pseudopodia, which could permit the capture of numerous bacteria present in the surrounding mesohyl. A thick layer of collagen fibrils, including lophocytes, separates the egg from the surrounding sponge mesohyl. Ultrastructural analysis has demonstrated the presence both of cellular components capable of autosynthetic activity (nutrient vesicles) and of phagocytosis mechanisms (pseudopod capture of bacteria) for the storage of nutrients by the egg.

103 school-age children (5 to 12 years) who survived mechanical ventilation for neonatal respiratory failure were evaluated for growth, neurological, intellectual, psychological and school function in order to determine those children most at risk for handicap. A major handicap occurred in seven children, preventing attendance at normal school or normal classes. Neurological sequelae were significantly associated with perinatal asphyxia and with birthweights of 1500g or less, and neurological sequelae and socio-economic factors were the major determinants of ability. The effects of the Neonatal Intensive Care Unit (NICU) experience on parents and subsequent parent-child relationships were also investigated: 67 per cent of the mothers were very upset by the experience and many continue to worry excessively about the health of their child. Parents who visited their child in the NICU frequently were significantly more anxious and overprotective, restricting many activities even when the child was of school age.
